Eosinophilic esophagitis is an immune-allergic pathology of multifactorial etiology (genetic and environmental) that affects both pediatric and adult patients. Its symptoms, which include heartburn, regurgitation, and esophageal stenosis (with dysphagia being more frequent in eosinophilic esophagitis in young adults and children), are similar to those of gastroesophageal reflux disease, causing delays in diagnosis and treatment. Although endoscopic findings such as furrows, esophageal mucosa trachealization, and whitish exudates may suggest its presence, this diagnosis should be confirmed histologically based on the presence of more than 15 eosinophils per high-power field and the exclusion of other causes of eosinophilia (parasitic infections, hypereosinophilic syndrome, inflammatory bowel disease, among others) for which treatment could be initiated. Currently, the 3 “D”s (“Drugs, Diet, and Dilation”) are considered the fundamental components of treatment. The first 2 components, which involve the use of proton pump inhibitors, corticosteroids, immunosuppressants and empirical diets or guided food elimination based on allergy tests, are more useful in the initial phases, whereas endoscopic dilation is reserved for esophageal strictures. Herein, the most important aspects of eosinophilic esophagitis pathophysiology will be reviewed, in addition to evidence for the various treatments.

Background & aims Hepatorenal syndrome is a rare entity that is part of the complications of liver cirrhosis in its more severe stages. Without treatment, its mortality rate increases significantly. Terlipressin is considered to be the therapy of choice until the need of a liver transplant. The aim is to determine its prevalence, define patients' characteristics, triggers and 90-day survival, according to the type of managements established. Method This was a retrospective cohort study conducted in Colombia. It included patients with cirrhosis and acute kidney injury who met hepatorenal syndrome criteria, reaching 28 patients from 2007 to 2015. Groups were categorized according the type of hepatorenal syndrome and treatment. Demographic and trigger factors were evaluated to characterize the population. Treatment outcomes with terlipressin vs norepinephrine were analyzed up to a 90-day survival, using log Rank test. Continuous variables needed Student's T and Mann Whitney's U tests and categorical variables, Chi2 test. A value of p <0.05 and a power of 85% was considered. The data was analyzed in the SPSS version 23 software. Results 117 patients with cirrhosis developed renal injury; of these 23.9% were diagnosed with Hepatorenal Syndrome (67.8% type1; 32.1% type2). The presence of ascites was 100% in HRS2 and 84% in HRS1 (p = 0.296). The main trigger in both types was paracentesis greater than 5 liters in the last 4 weeks (39.3%). In total, 35% of the patients received renal replacement therapy and 14% underwent a hepatic transplant. Type 1 was more frequent (63% received terlipressin; 21% norepinephrine). Introducción: la fistula biliar poscolecistectomía es poco frecuente. El manejo principalmente es endoscópico, pero en la literatura no hay consenso en la técnica de primera línea entre papilotomía, prótesis biliar o su combinación. Metodología: se realizó un estudio observacional tipo serie de casos en el que se incluyeron todas las CPRE realizadas en el Hospital Universitario San Ignacio en Bogotá, Colombia, entre enero de 2010 y marzo del 2021 por fistula biliar posterior a colecistectomía. Se registraron las características demográficas, manifestaciones clínicas, resolución, eventos adversos y estancia hospitalaria según la técnica endoscópica. Resultados: se incluyeron 24 pacientes con fistula biliar poscolecistectomía que se manejaron con CPRE. La mediana de edad fue de 59 años (rango intercuartílico [RIC]: 53,5-67). En el 75% el tipo de cirugía fue laparoscópica. La manifestación clínica más frecuente fue aumento del drenaje biliar > 150 mL/24 horas (50%), seguido de dolor abdominal (39%). La principal localización fue el conducto cístico en el 40%. El manejo con papilotomía fue del 25%; con prótesis biliar, 8,4%, y combinado, 66%; la resolución de la fístula ocurrió en el 100%, 50% y 87%, respectivamente, con menor estancia hospitalaria en el manejo combinado de 3,5 días frente a 4 días en papilotomía. Solo se presentó 1 evento adverso de hemorragia en el grupo de papilotomía. Conclusión: la papilotomía y la terapia combinada son opciones terapéuticas con buenas tasas de resolución y baja estancia hospitalaria para el manejo de las fistulas biliares poscolecistectomía. Se requerirán estudios prospectivos, aleatorizados y multicéntricos para definir la técnica con mejores desenlaces clínicos.

La manometría anorrectal de alta resolución es una prueba diagnóstica utilizada para los trastornos motores y sensitivos anorrectales. Consta de una medición del tono basal y de contracción anal; también valora la maniobra de pujo, reflejo rectoanal inhibitorio (RAIR) y parámetros sensitivos rectales. La interpretación convencional de la manometría anorrectal se enfoca en describir aisladamente la región anatómica disfuncional. Sin embargo, con la clasificación de Londres se busca estandarizar el informe de estos resultados, agrupándolos en trastornos mayores, menores y hallazgos no concluyentes, similar a la clasificación de Chicago para trastornos motores esofágicos.
