Caso clínico de un hombre de 20 años, procedente de área rural de un municipio de Nariño, que consulta a un centro de salud después de 36 horas de haber ingerido de modo no intencional 20-30 mL de gramoxone (dicloruro de paraquat) mientras estaba en estado de embriaguez, con síntomas digestivos, hiperbilirrubinemia, elevación de azoados, leucocitosis y neutrofilia, por lo que es referido a un hospital de alta complejidad en la ciudad de Pasto. Durante su hospitalización, presenta epistaxis, falla renal con requerimiento de hemodiálisis, quemaduras orales, hipertermia y dos episodios de crisis convulsivas tónico-clónico generalizadas. Se toman paraclínicos: azoados, gases arteriales, electrolitos, glicemia, entre otros, cuyos resultados se enmarcan dentro de la normalidad durante los episodios convulsivos. Es relevante proporcionar elementos para construir un criterio clínico que explique el compromiso neurológico, ya que, es raro y complejo en intoxicaciones por herbicidas como el paraquat.
The bacterial diversity retained in Al/Fe-pillared interlayered clay and Fe-functionalized granular activated carbon absorbent was characterized after their saturation with mature landfill leachate either before and after a tertiary, catalytic wet peroxide oxidation treatment. The 454 pyrosequencing analyses showed bacteria belonging to Phyla: Proteobacteria, Bacteroidetes and Actinobacteria. Flavobacterium was the most representative genus detected in three out of four studied adsorbents: the pillared clay before the catalytic oxidation and the granular activated carbon at both stages before and after the catalytic oxidation, whereas Haliscomenobacter, Rhodococcus Pseudomonas, Thermomonas, Aequorivita, and Acidovorax, were also found according to the type of absorbent. It was demonstrated the efficiency of the oxidizing treatment in the elimination of the immobilized leachate's microorganisms when in the presence of the Al/Fe-pillared clay adsorbent; this exhibited the highest catalytic response, since no DNA was detected on this material after its catalytic treatment. Only in the case of the functionalized activated carbon, it was found the presence of microorganisms of environmental interest after the advanced oxidation stage. Although the bacterial community detected in the activated carbon after oxidizing treatment showed lower voltage output than that one before oxidation, such resistant bacteria could be potentially useful driving microbial fuel cells for degradation of more complex and harmful substrates. Thus, further studies should be done assessing the degradation of toxic and hazardous substances by microbial fuel cells in the presence of catalytic wet peroxide oxidation-and other advanced oxidation-resistant bacteria, including contaminants of emerging concern widely spread in wastewaters.
El coronavirus tipo 2 es un β-coronavirus cuya infección se caracteriza por clínica de predominio respiratorio; sin embargo, la sintomatología neurológica está cobrando gran interés asociada a la infección pulmonar e invasión directa del virus al sistema nervioso central. Siendo posible la asociación entre el síndrome de Guillain-Barré y la infección por virus SARS-CoV-2. En este reporte se describe dicha asociación en una paciente femenina de 44 años de edad, con clínica clásica de síndrome de Guillain-Barré al ingreso y clínica respiratoria seis días previos a la instalación de los síntomas neurológicos. Reportándose serologías IgG e IgM para SARS-CoV-positivas y nexo epidemiológico de contacto directo con personas infectadas por SARS-CoV-2. Requirió manejo en UCI por riesgo de falla respiratoria, manejo con inmunoglobulinas, sin requerimiento de ventilación mecánica, presentando mejoría y otorgándose salida. Un mes después reconsulta, se considera recaída de Guillain-Barré, se hospitaliza, se inicia manejo hasta evolución y resolución de síntomas
