The findings should be used when designing new models applied in different healthcare systems within each country, with a focus on strategy aimed at the welfare of the patient and his family. Findings give a possible solution for financially restricted healthcare systems, regarding the patient's care in the home environment.
Aim. The study highlights the different concepts of dementia care models which are used in different care settings and determine the effectiveness and contribute to a better quality of life for people with dementia. Methods. This literature review use PRISMA process for data collection and analysis. Scientific and professional publications in English were searched through international specialized databases; EBSCO HOST, CINAHL, MEDLINE, PubMed and Wiley Online Library. The review results were founded by key words and then selected according to the publication time frame and other shaped criteria. Results. In total 1998 references were identified for the thematic content analysis, in the final review we included 29 studies. Two thematic areas were identified according to the keywords and literature search strategy: dementia care models and geriatric models of care. Further, we formed four concepts highlighted in models that reflect the specific characteristics and principles of care: biopsychosocial approach, person-centred care practice, environment of care, and Montessori for dementia care. Conclusions. Most of featured models advocated the implementation of comprehensive, integrated and person-centred care, focused to honour and integrate an individual’s life story with their needs into daily care as much as possible in an appropriate and customized environment. Currently the most innovative model is based on implementation of Montessori approach into person’s daily activities.
Uvod: Pomen patronažne zdravstvene nege narašča s pospešenim razvojem primarnega zdravstvenega varstva, preprečevanja bolezni in približevanja zdravstvene oskrbe integrirani podpori pacientom in njihovim družinam znotraj lokalnih skupnosti. Namen raziskave je bil proučiti, kaj je predmet raziskovanja izvajalcev zdravstvene nege ter katera so glavna vprašanja in problemi patronažne zdravstvene nege v Sloveniji. Metode: Uporabljen je bil sistematičen pregled literature. Zajemal je zbiranje znanstvene in strokovne literature v Vzajemni bibliografsko-kataložni bazi podatkov COBIB.SI od januarja do maja 2017. Literatura je bila iskana po korenu ključne besede patronaž*. Začetna merila za tematsko analizo vsebine je izpolnjevalo 144 enot. V seznam za analizo empiričnih podatkov se jih je uvrstilo 45. Za prikaz pregleda in odločanja o uporabnosti pregledanih enot je bila uporabljena metoda PRISMA. Rezultati: S pregledom literature so bila ugotovljena raziskovalna področja v patronažni zdravstveni nege v Sloveniji; izboljšanje kakovosti, varnost in pogoji dela, opolnomočenje pacienta in integrirana oziroma celovita obravnava pacienta. Zaznan je problem dostopnosti do objav posameznih raziskav, kakovost načrtov in metod raziskovanja. V zadnjih treh letih preučevanega obdobja so prevladujoči vir podatkov v raziskavah patronažne medicinske sestre. Diskusija in zaključek: Nujno je ciljno ukrepanje s povečanjem interesa in znanja patronažnih medicinskih sester za raziskovanje. Za boljše prepoznavanje njihove vloge v primarnem zdravstvenem varstvu ter krepitev interdisciplinarnega raziskovanja na mikro-, mezo- in makroravni sistema zdravstvenega varstva je potrebno uvajanje kompleksnejših raziskovalnih načrtov.
IZVLEČEKUvod: Demenca je kronično obolenje s tendenco upadanja kognitivnih sposobnosti obolelega. Uporaba metod za obvladovanje in trening osnovnih življenjskih aktivnosti, ki omogočajo daljše obdobje samostojnega življenja pacienta z demenco, je v tujini uveljavljena z različnimi novejšimi komplementarnimi pristopi. Ena izmed uveljavljenih metod je pristop Marie Montessori, prilagojen odraslim pacientom z demenco. V članku je predstavljena metoda montessori pri obravnavi pacienta z demenco in njeni učinki na kakovost življenja pacienta z demenco. Metode: Uporabljena je bila deskriptivna raziskovalna metodologija s pregledom domače in tuje literature. Za prikaz pregleda petih preko spleta dostopnih podatkovnih baz in odločanja o uporabnosti pregledanih virov je bila uporabljena metoda PRISMA. V končni pregled literature je bilo vključenih 19 člankov, objavljenih do maja 2016. Za obdelavo podatkov je bil uporabljen model analize konceptov. Večina zajetih raziskav je bila izvedena v Združenih državah Amerike. Rezultati: Po pregledu raziskav so bila identificirana tri tematska področja: (1) vpliv metode montessori na sodelovanje in prizadevanje, (2) vpliv metode montessori na vedenje, povezano s hranjenjem, in (3) vpliv metode montessori na širšo skupino kognitivnih sposobnosti. Diskusija in zaključek: Kljub majhnemu številu člankov, ki opisujejo uporabo metode montessori pri obravnavi pacientov z demenco, metodo lahko predstavimo kot učinkovito. Avtorji raziskav ugotavljajo, da pristop ne le omogoča ohranjanje kognitivnih sposobnosti, temveč le-te celo izboljšuje. ABSTRACTIntroduction: Dementia is a chronic disease with the tendency of declining patient's cognitive and mental abilities. Treatment is primarily focused on prolonging the period of independent life. Various newer complementary approaches and methods that are aimed at maintaining and encouraging cognitive abilities have been implemented across the globe. One of these methods is the approach of Maria Montessori, which has been adapted to adult persons suffering from dementia. The article presents the Montessori Method in the treatment of people with dementia and its effects on the patient's quality of life. Methods: The descriptive research methodology was used to review of domestic and foreign literature. The PRISMA method was used to review five databases accessible online and make decisions on the applicability of the reviewed sources. The conceptual model by Walker and Avant was used for data analysis. The final review included 19 studies published before May 2016. The included studies have been conducted mainly in the United States of America. Results: The three thematic areas have been identified based on reviews: (1) the effects of the Montessori-based activities on engagement and effort, (2) the impact of the Montessori-based activities on eating behaviours and (3) the benefits of the Montessori-based activities on the broader group of cognitive abilities. Vsak dan kompleksnejša obravnava pacientov z vse večjim številom kroničnih obolenj/stanj zahteva vključevanje vse večjega števila strokovnjakov različnih zdravstvenih in nezdravstvenih področij. Vse pogosteje v obravnavi pacientov sodelujejo različni zdravniki specialisti, medicinke sestre, fizioterapevti, dietetiki, farmacevti, psihologi in drugi strokovnjaki. Izveden je bil pregled literature, s katerim smo želeli ugotoviti, kako se povezujejo in sodelujejo različni zdravstveni delavci na vseh ravneh zdravstvenega varstva. Ključna besedna zveza za iskalni niz je bila kombinacija znanja in spretnosti (»skill mix«), postavljeni pa so bili tudi vključitveni kriteriji. Potek raziskave je prikazan v PRIZMA diagramu. Na podlagi pregleda izbranih virov smo oblikovali štiri vsebinske kategorije: »skill mix« v bolnišničnem okolju, »skill mix« v institucionalnem varstvu in skrbi za starejše odrasle, »skill mix« na primarni zdravstveni ravni ter »skill mix« v času pandemije covida-19. Timsko delo, ki zajema prenos/kombinacijo znanj in spretnosti, postaja velik izziv za zdravstveni sistem. Na podlagi ugotovitev sklepamo, da lahko prinese revolucionarne spremembe v oskrbi pacientov na vseh ravneh zdravstvenega in socialnega varstva. Ključni elementi za uspeh so sodelovanje, spoštovanje znanj in spretnosti vseh vključenih in dobra organizacija v času uvajanja sprememb.
