This study provides the first empirical evidence that the application of the posttraumatic stress disorder paradigm in the long-term evaluation of cardiac arrest survivors significantly contributes to defining a patient population at high risk for serious emotional disability.
ZusammenfassungDurch zunehmende Verbreitung der Frühdefibrillation hat die Ein-Helfer-Reanimation wieder an Bedeutung gewonnen, um den zweiten Helfer für die Vorbereitung der erweiterten Maûnahmen freizustellen. Unter Verwendung der Beutel-Masken-Beatmung ergibt sich die Notwendigkeit, neue, bisher nicht leitlinienkonforme Methoden zu entwickeln. Diese Studie vergleicht die ¹Überkopfmethodeª und die ¹Seitenmethodeª hinsichtlich ihrer Effektivität und Praktikabilität. Bei Probanden (¾rzten und Feuerwehrleuten) wurden Qualitätsparameter der Reanimation wie Drucktiefe, Frequenz, Atemzugvolumen und Atemminutenvolumen untersucht, die mit dem Resusci Anne SkillReporter (Fa. Laerdal) verifiziert wurden. Als Vergleich wurden die Grenzwerte in den Empfehlungen der ILCOR in der derzeit gültigen Fassung zugrunde gelegt. Obwohl die ¹Überkopfmethodeª eine effektivere Beutel-Masken-Beatmung ermöglicht als die ¹Seitenmethodeª (50 vs. 40,9 % suffiziente Beatmungen), war die Herzdruckmassage bei beiden Methoden nicht suffizient (39 vs. 43 % korrekte Kompressionen). Wegen der nicht ausreichendenden Effektivität der Herzdruckmassage sollten beide Methoden nicht eingesetzt werden.
AbstractThe distribution of the early defibrillation resulted in an increasing importance on the one-rescuer-resuscitation to set one rescuer free for the preparation of advanced measures. Using the bag-valve-ventilation, it is getting more important to develop new methods, previously not in conformity to the ILCOR-guidelines. This study compares the effectiveness and practicability of the ¹over-head-methodª and a ¹side-methodª. Quality parameters of the resuscitation that could be measured by an manikin (Resusci Anne SkillReporter [Laerdal]) as well as pressure depth, frequency, breath volume and breath minute volume were evaluated on physicians and firemen. As a comparison, the limits in the recommendations of the ILCOR in the presently valid version were taken. Although the ¹over-head-methodª shows a more effective bag-valve-respiration than the ¹side-methodª (50 vs 40.9 % sufficient respirations), the cardiac compressions in both methods were not sufficient (39 vs. 43 % correct compressions). Because of the ineffective cardiac massage should both methods not be performed.
