Bei 20 gesunden Versuchspersonen (13 Frauen, 7 MÄnnern) im Alter zwischen 20 und 36 Jahren wurden die audiometrischen TonhÖrschwellen und die transitorisch evozierten otoakustischen Emissionen (TEOAE) vor und nach einer Schallbelastung (SchmalbandgerÄusch von 90 dB SL Über 5 min) gemessen. WÄhrend der Beschallung wurde die perstimulatorische LautheitsverÄnderung (Adaptation) bestimmt. Die kurzfristige akustische Belastung bewirkt eine temporÄre HÖrschwellenabwanderung, aber auch eine Amplitudenver-minderung der TEOAE. Zwischen der perstimulatorischen Adaptation und der Amplitudenabnahme der TEOAE konnte eine signifikante Korrelation nachgewiesen werden. Bei Personen mit einer kleinen perstimulatorischen Adaptation war die Verminderung der Amplitude der TEOAE grosser und die Erholung langsamer. Diese Untersuchungsmethode kann der Charakteri-sierung einer individuellen lÄrmabhÄngigen VulnerabilitÄt des HÖrorgans dienen.
In einer Longitudinalstudie wurden 254 Kinder (135 Knaben, 119 MÄdchen) mit beidseitiger sensorineuraler SchwerhÖrigkeit wÄhrend einer durchschnittlichen Beobachtungszeit von 8,6 Jahren audiometrisch kontrolliert. Dabei wurde vor allem der Verlauf der Innenohrleistung ausgewertet. Bei 145 Patienten (57%) konnte die Ätiologie der HÖrstÖrung festgestellt werden: HereditÄt (39%), RÖteln (8%), Meningitis (7%). Bei 109 Patienten (43%) konnte keine eindeutige Ursache gefunden werden. Zur Beurteilung der Progredienz wurden 163 Patienten ausgewÄhlt, deren durchschnittliche HÖrschwelle die 99-dB-Grenze zu Beginn der Beobachtung nicht Überschritten hatte. Die audiometrischen Kontrollen wurden mindestens 1 mal pro Jahr durchgefÜhrt. Eine Progredienz der SchwerhÖrigkeit liess sich in 64 Fallen (39,2%) nachweisen. Sie trat bei 20 MÄdchen und 44 Knaben auf. Dieselbe war bei Knaben eindeutig starker ausgeprÄgt als bei MÄdchen und kam am hÄufigsten in der Ätiologiegruppe «hereditÄre SchwerhÖrigkeit» vor. Überdies unterstÜtzen die Ergebnisse dieser Studie die Ansicht, dass ein HÖrgerÄt durchaus zur Progredienz der SchwerhÖrigkeit (LÄrmschÄdigung) beitragen kann. Die meisten schwerhÖrigen Patienten (148 = 90,8%) waren HÖrgerÄtetrÄger. Unter den 15, die kein HÖrgerÄt trugen, kam die Verschlechterung des GehÖrs nur 1 mal vor. Unter den 148 HÖrgerÄtetrÄgern fanden wir eine Progredienz in 63 Fallen (38%), davon 12mal beidseitig. Unsere Ergebnisse stÜtzen die Ansicht, dass ein HÖrgerÄt bei schwerhÖrigen kindlichen Patienten eine schÄdliche LÄrmwirkung haben kann.
