The aim of the present study was to analyze adherence to drug treatment in hypertensive patients enrolled on the Family Health Program in Ribeirão Preto, São Paulo State. This transversal study was conducted between August and December 2006, in which 109 patients were interviewed using the Morisky and Green Test to measure adherence level to pharmacotherapy. Variables that may be related to adherence level were also investigated, such as demographic characteristics, health care team and health service related factors, and pharmacotherapy related factors. The test scores were analyzed by two criteria: patients were categorized as "more adherent" if they had a score from 3 to 4 (criterion 1) or a score of 4 (criterion 2); patients with other scores were categorized as less adherent. Of all patients, 79.8% and 43.1% were classified as "more adherent" according to criterion 1 and criterion 2, respectively. With regard to the possible causes of non-adherence to treatment, statistically significant (p<0.05) associations were identified between "trust in the doctor" or "number of antihypertensive drugs used" and the level of adherence, according to criterion 2. These results indicated adequate adherence levels to drug treatment by the patients, and highlighted the importance of professional/patient interaction, trust in the doctor and the attitude by health professionals toward users. Este estudo teve como objetivo estudar a adesão ao tratamento farmacológico de pacientes hipertensos seguidos no Programa de Saúde da Família em Ribeirão Preto, SP. Realizou-se um estudo transversal entre agosto e dezembro de 2006, em que foram entrevistados 109 pacientes, utilizando-se o Teste de Morisky e Green para mensurar o grau de adesão ao tratamento. Variáveis que podem se relacionar com o grau de adesão também foram estudadas, tais como: características demográficas, fatores relacionados à equipe e ao serviço de saúde e à terapia medicamentosa. Os resultados do teste foram avaliados por dois critérios: os pacientes foram classificados como "mais aderentes" se apresentassem pontuação de 3 a 4 (critério 1) ou de 4 (critério 2); pacientes com outras pontuações foram classificados como "menos aderentes". Dos pacientes, 79,8% e 43,1% foram "mais aderentes" de acordo com os critérios 1 e 2, respectivamente. Em relação às possíveis causas da não adesão, observaram-se associações significativas (p<0,05) entre as variáveis "confiança no médico" ou "quantidade de medicamentos anti-hipertensivos que utiliza" e "grau de adesão", pelo critério 2. Os resultados do presente trabalho indicam que os pacientes apresentam graus satisfatórios de adesão ao tratamento medicamentoso, a importância da interação profissional/paciente, a confiança no médico e a atitude dos profissionais de saúde em relação ao usuário.Unitermos: Adesão ao tratamento. Although most patients finished their treatment, TIC led to trastuzumab discontinuation in a significant proportion of patients suggesting the need of a closer cardiac monitoring. None of the covariates influenced on the incidence of TIC, which can be due to the relatively small sample. Thus, larger scale studies should be conducted in order to establish which specific factors are associated with the development of TIC in order to avoid it.

BackgroundMetastatic colorectal cancer imposes a substantial burden on patients and society. Over the last years, progresses in the treatment have been made especially due to the introduction of monoclonal antibodies, such as bevacizumab which, on the other hand, has considerably increased the costs of treatment. We performed a cost-effectiveness analysis of bevacizumab plus XELOX in comparison with XELOX alone in metastatic colorectal cancer in first-line therapy, from the perspective of a public hospital school in Brazil.MethodsThis was a cost-effectiveness analysis performed by a decision tree and Markov models. Costs were expressed in local currency and outcomes were expressed in months of life gained. The model was constructed using the TreeAge Pro 2013® software.ResultsThe incremental difference in years of life gained was 2.25 months, with an extra cost of 47,833.57 BRL, resulting in an incremental cost-effectiveness of 21,231.43 BRL per month of life gained.ConclusionsAlthough the XELOX plus bevacizumab regimen is a more expensive and more effective treatment than XELOX, it does not fit the reimbursement values fixed by the public healthcare system in Brazil.
Background: This study applied time-driven activity-based costing (TDABC) to accurately assess costs and to demonstrate how it is possible to estimate cost-saving opportunities by exploring the results achieved with the method.Methods: This is a cohort retrospective and multicenter cost analysis research, which included data from video-assisted cholecystectomy patients from five public-funded hospitals. The TDABC was applied according to recommendations in the literature for data collection and analyses. Descriptive cost analysis was followed by an estimate of the labor cost that could be saved by redesigning the surgical pathway based on the benchmark care cycle identified in the hospitals studied.
Summary Background Worldwide, healthcare facilities face high and often inappropriate consumption of antimicrobials. However, there are limited antimicrobial stewardship (ASP) studies from low- and middle-income countries that include restrictive measures and their impacts on antimicrobial consumption. Aim This study aims to assess the impact of a restrictive bundle (preauthorization requirements and offering consultation with an infectious diseases physician 24 hours a day) on antimicrobial consumption, in a tertiary hospital in Brazil. Methods We conducted an interrupted time series analysis (ITS) with pharmacy-based data from January 2010 to December 2018 to evaluate changes of level and slope in antimicrobial consumption (expressed in DDD/1,000 patient-days) after the implementation of the restrictive bundle in September 2014. Fourteen restricted antimicrobials (amphotericin B deoxycholate, liposomal amphotericin B, micafungin, voriconazole, ganciclovir, amikacin, ampicillin/sulbactam, daptomycin, ertapenem, levofloxacin, linezolid, piperacillin/tazobactam, polymyxin B, and tigecycline) and eight unrestricted were analysed. Findings Among the 14 restricted antimicrobials, four presented a significant negative level change: micafungin (-2,14, p=.027), voriconazole (-14.72, p<.001), ertapenem (-1.89, p=.007) and amikacin (-13.98, p<.001). The only negative slope change was observed for the liposomal amphotericin B, -0.532 (p=.009). The restricted antibiotics group presented an increased consumption trend (1.068, p=.002) compared to the pre-intervention period, a similar change was observed for the unrestricted antibiotics group (1.360, p<.001). Conclusion Preauthorization and 24 hour expert consultation were partially effective as a restrictive antimicrobial stewardship bundle in a Brazilian tertiary-care hospital. Increased consumption of unrestricted antimicrobials was observed as a side effect of the intervention.
Na última década, houve uma significativa melhora nas taxas de resposta, sobrevida livre de progressão e sobrevida global no tratamento do câncer colorretal metastático, resultante do desenvolvimento de novas combinações de quimioterapia padrão e surgimento de drogas alvo-específicas. Considerando o alto custo das terapias e os recursos disponíveis cada vez mais limitados para a atenção à saúde, este estudo teve como objetivo realizar uma análise de custo-efetividade do protocolo XELOX isoladamente (estratégia 1) em comparação ao XELOX acrescido de bevacizumabe (estratégia 2) em tratamento de 1ª linha para pacientes com câncer colorretal metastático sob a perspectiva de um hospital público voltado para a assistência e o ensino. Trata-se de uma avaliação econômica completa do tipo custo-efetividade, empregando-se árvore de decisão simples associada a Modelo de Markov. Os custos foram expressos em unidade monetária local (R$) e os desfechos em meses de vida ganhos. Para a construção do modelo, foi utilizado o software TreeAge Pro 2013 ® . Foi elaborado um modelo de estados de transição de Markov, em horizonte temporal de 60 meses, sendo que cada ciclo do modelo correspondeu a três meses. Os dados de custos foram coletados retrospectivamente, por microcusteio, e obtidos por meio do sistema interno de dados eletrônico do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo. Os dados de efetividade e as probabilidades de transição entre os estados de saúde foram calculados utilizandose dados de estudos clínicos selecionados por revisão sistemática. A diferença incremental em meses de vida ganhos foi de 2,25 para um custo extra de R$47 833,57, o que resultou em uma razão de custo-efetividade incremental de R$21 231,43 por mês de vida ganho. Considerando-se um limiar de custo-efetividade de três vezes o valor do Produto Interno Bruto per capita, segundo recomendação da Organização Mundial de Saúde, a adição de bevacizumabe ao esquema XELOX não foi considerada custo-efetiva. Pela análise de sensibilidade, a variável que causou maior impacto foi a efetividade para o estado de saúde "suporte clínico" na estratégia 1. Quando este parâmetro foi inserido no modelo com o valor mínimo e o valor máximo, apresentou uma razão de custo-efetividade incremental de R$7 814,47 e R$-29 614,12 respectivamente. Assim, a estratégia 2 tornou-se dominada pela estratégia 1. Objective: The study aimed at evaluating and comparing healthcare-related costs for metastatic colorectal cancer treatment among patients receiving systemic (chemotherapy or biotherapy) therapy, considering both costs per treatment cycle and total costs per patient. Methods: This was a cross-sectional, econometric study conducted in a public, tertiary referral hospital in Brazil, which is supported by national and state level funding. Data were collected between January 01, 2009 and October 31, 2013. Micro-costing method was employed to estimate costs related to medication, laboratory tests, imaging tests, drug preparation and administration. Results: The XELOX plus bevacizumab therapy presented the highest average cost per cycle, 7,701 Brazilian reals, followed by FOLFIRI plus bevacizumab, with an average cost of 6,927 Brazilian reals. Costs of treatment regimens containing capecitabine and/or monoclonal antibody (capecitabine monotherapy, cetuximab, cetuximab plus irinotecan, FOLFIRI plus bevacizumab, XELIRI, XELIRI plus bevacizumab, XELOX and XELOX plus bevacizumab) had a greater impact on total cost of therapy. In the mFOLFOX6 and FOLFIRI regimens, the total cost of therapy was mostly influenced by the cost per cycle (51%) and the cost related to drug administration (50%). Conclusions: the Brazilian universal healthcare system does not cover monoclonal antibody therapies and the chemotherapy regimens FOLFIRI and mFOLFOX6 for colorectal cancer, since they surpass the monthly reimbursement amounts provided by the system. On the other hand, XELOX regimen fits within the budget established by the system, representing a promising alternative for colorectal cancer treatment, especially taking into account current economic limitations.
