Reflujo gastroesofágico (RGE) fisiológico, es el paso del contenido depositado en el estómago, de forma retrograda a la luz del esófago; en ocasiones puede llegar a la faringe o boca, e incluso puede ser exteriorizado en forma de vómito, pero la mayoría de las veces este regresa al estómago sin producir síntomas respiratorios, digestivos o generales.(1,2) Es frecuente la regurgitación y el vómito de variada frecuencia e intensidad, que en la mayoría de los lactantes de 6 a 12 meses desaparece asociado al crecimiento, la bipedestación y la ablactación . (3,6,11,12). Reporte de un caso clínico de RGE en un lactante menor, previamente manejado con inhibidor de bomba de protones, sin mejoría clínica. Se indica un abordaje integral de los problemas de salud, desde un enfoque preventivo, recomendaciones de puericultura y medidas generales antirreflujo, sin ordenar ningún estudio ni medicamento, con evolución clínica favorable. Introducción: La neurocisticercosis (ncc) es la infección parasitaria más común del sistema nervioso central causada por la larva de la Taenia solium. Esta patología es endémica en los países en vías de desarrollo y presenta su mayor incidencia en pacientes escolares y adolescentes de la población pediátrica. Muestra una diversidad clínica y hallazgos patológicos, en general, se presenta con formas intraparenquimatosas asociadas con convulsiones o formas extraparenquimatosas relacionadas con hipertensión endocraneana. La sospecha clínica y epidemiológica de la enfermedad obliga a solicitar estudios de tomografía axial computarizada (tac) y/o resonancia magnética, además de los estudios serológicos específicos. Presentación de caso: se describe un caso clínico de una niña preescolar con neurocisticercosis sucedido en el Hospital Infantil Napoleón Franco Pareja (hinfp) en Cartagena de Indias, Colombia, el cual se caracteriza por presentar un cuadro clínico que debuta con crisis tónicas en extremidades, por lo que se solicita estudio de tac cerebral, que muestra imágenes de encefalitis compatible con cistercosis cerebral, se decide hospitalizar iniciándose manejo cesticida, anticonvulsivante y antiinflamatorio. Conclusión: Los hallazgos imagenológicos y la evolución clínica de la neurocisticercosis en la infancia están asociados con el estadio de la enfermedad y la respuesta inmunológica del huésped.
In pediatric severe Traumatic Brain Injury (TBI) the appearance of refractory intracranial hypertension is related to an unfavourable prognosis. Severe TBI (ECG <8) is present in 60% of polytrauma children and is associated with high morbidity and mortality. The monitoring of intracranial pressure (ICP) and the treatment of intracranial hypertension have been subjects of attention in neurotrauma for years in the evidence-based guidelines currently being a topic of discussion. Currently the guides based on evidenceshowing acceptance of some guidelines for the management of intracranial hypertension, within the most accepted of sedatives/analgesics, hyperosmolar agents (hypertonic saline solution HTS and mannitol) and barbiturates, although the level of evidence of these recommendations is insufficient to standardize such management in the centers of attention.
Objetivo: Determinar la presencia de cambios en los diagnósticos electroencefalográficos pre y post operatorios de niños de cero a 18 años llevados a cirugía de corazón en una institución de referencia en Colombia entre los meses de agosto a noviembre del año 2017. Materiales y métodos: Se tomó electroencefalograma una hora previa al procedimiento quirúrgico cardiaco y otro los siete días posterior al procedimiento, se recolectaron datos clínicos asociados a presencia de anormalidades en el electroencefalograma. Los datos fueron sometidos a análisis descriptivo, bivariado y multivariado. Resultados: Se encontró cambios en los trazados electroencefalográficos del preoperatorio al post operatorio de dos maneras, una con deterioro de los trazados de normales a anormales y la otra con empeoramiento del trazado anormal. Conclusiones: Los pacientes son llevados a cirugía a pesar de tener trazados alterados. La población llevada a cirugía cardiaca en el centro del estudio es muy diversa lo que ocasiona resultados neurológicos variados e influyen en los cambios en los trazados post operatorios.
