ZusammenfassungDer demografische Wandel bringt eine wachsende Gruppe hochbetagter und geriatrischer Patienten mit sich. Da die Inzidenz nicht-melanozytärer Hauttumoren bei Hochbetagten sehr hoch ist und schlechte Versorgung sowie fehlendes Bewusstsein gegenüber dieser Tumorentität zu Verzögerungen bei der ärztlichen Konsultation führen kann, wird man in der Dermatologie immer wieder mit sehr großen Hauttumoren konfrontiert. Obwohl mit zunehmendem Alter die Operationsrisiken steigen, ist die mikrografisch kontrollierte chirurgische Tumorexzision dennoch die Therapie der Wahl. Wir stellen einen 91-jährigen Patienten mit einem riesigen Plattenepithelkarzinom der Haut vor, bei dem durch ein radikales dermatochirurgisches Vorgehen sowohl Tumorfreiheit als auch maximale Funktionalität und eine Wiederherstellung der Lebensqualität erreicht werden konnten. Dieser Fall beschreibt einige der Anforderungen an eine effektive und patientenorientierte Therapie bei großen Hauttumoren in dieser Altersgruppe.
