Aneurysms of the subclavian-axillary segment are rare, but when diagnosed they must be treated. This article describes two cases of aneurysms of the upper extremities, one in a subclavian artery and the other in an axillary artery. The first case was a 71-year-old male with a pulsating supraclavicular bulge on the right and muscle weakness in the ipsilateral extremity. Duplex scanning and arteriography confirmed the diagnosis of aneurysm of the right subclavian artery and the patient underwent aneurysmectomy and end-to-end anastomosis. The second case was a 24-year-old female patient, with no history of comorbidities, who presented with a pulsating mass in the right axillary region and paresthesia of the ipsilateral extremity. Duplex scanning and arteriography confirmed an aneurysm in the right axillary artery, which was successfully treated with aneurysmectomy and end-to-end anastomosis. Pathology findings showed that the first case was an atherosclerotic aneurysm and the second was a congenital aneurysm.Keywords: aneurysm; subclavian artery; axillary artery; surgery. ResumoOs aneurismas do segmento subclávio-axilar são de ocorrência rara e, uma vez diagnosticados, devem ser tratados. Neste trabalho, relatamos dois casos de aneurismas de membros superiores, sendo um da artéria subclávia e outro da artéria axilar. No primeiro caso, o paciente de 71 anos, sexo masculino, apresentava abaulamento pulsátil supraclavicular direito associado à fraqueza muscular no membro ipsilateral. Foram realizados duplex scan e arteriografia, que confirmaram o diagnóstico de aneurisma da artéria subclávia direita, sendo o paciente submetido à aneurismectomia com anastomose término-terminal. No segundo caso, uma paciente de 24 anos, do sexo feminino, sem antecedentes mórbidos ou comorbidades, apresentava queixa de massa pulsátil na região axilar direita associada à parestesia no membro ipsilateral. Background: There are numerous access routes for inserting implants during breast augmentation surgery. In 1966, Pitanguy described the transareolopapilar route. The aim of this study was to assess the use of transareolopapilar incision during breast augmentation surgery at the Ivo Pitanguy Institute (Rio de Janeiro, RJ, Brazil), over the past 10 years. Methods: Retrospective analyses of the size of the implants used, indications for transareolopapilar incision, postoperative complications, and postoperative scarring were performed. Results: Fifty-three patients with a mean age of 33.54 years were included, and the mean follow-up period was 11.6 months. Most (60.4%) of the implants were <200 ml. Twelve patients required a second operation due to a breast lump (1 case); infection (1 case); capsular contracture (1 case); and dissatisfaction with breast shape (4 cases), volume (4 cases), and unilateral scarring (1 case). Sixteen (30.2%) patients developed some form of minor postoperative complication; 13 (24.5%) had one or more scarring issues, including hypochromia (18.9%), hypertrophy (1.9%), scar retraction (1.9%), and areola bifida (1.9%). Twenty (37.7%) patients underwent postoperative follow-up for more than one year and were satisfied with the postoperative scar. Conclusions: The transareolopapilar incision facilitates the insertion of small-to-moderate size implants with a low rate of postoperative complications and a low incidence of scarring, provided the correct surgical technique is used.
RESUMOIntrodução: A cirurgia de aumento mamário foi a cirurgia plástica mais realizada no Brasil, em 2008. Com a evolução da técnica e da qualidade dos implantes de silicone, a taxa de complicações tem diminuído progressivamente, tornando a cirurgia mais segura. Objetivo: Demonstrar o perfil das pacientes submetidas à troca de implante mamário, nos últimos cinco anos, na Clínica Ivo Pitanguy, analisando as características dos implantes de silicone e as causas que motivaram a troca. Método: Foram analisadas, retrospectivamente, todas as pacientes submetidas a procedimentos cirúrgicos de troca, retirada ou reinclusão de implantes mamários, nos últimos cinco anos, num total de 59 pacientes em 68 procedimentos e 129 mamas operadas. Resultados: O intervalo de tempo entre a colocação e a retirada dos implantes variou de 1 mês a 34 anos; 44 pacientes eram provenientes do nosso serviço e 24 de outros. Quarenta e três implantes eram, antes da troca, de poliuretano, 13 texturizados e seis lisos. O grande motivo de troca foi estético, com 50 casos, seguido por contratura capsular em 13 casos, e cinco casos de infecção, ruptura ou nódulo mamário. Conclusão: O principal motivo de troca de implantes foi estético, diferentemente da literatura. Os índices de contratura capsular e de infecção da loja do implante do presente estudo foram comparáveis aos apresentados em vários outros estudos.Descritores: Implante mamário. Reoperação. Próteses e implantes. ABSTRACT Background:The breast augmentation surgery was the most performed in Brazil in 2008. With the evolution of the technique and the quality of the implants, complications have diminished, leading to a more secure procedure. Objective: To assess the profile of patients submitted to implant exchange in the last five years at Ivo Pitanguy Clinic, analyzing the causes that led to implant removal. Methods: We have analyzed all patients submitted to exchange, removal or replacement of mammary implants in the last five years, at a total of 59 patients, 68 procedures and 129 breasts operated. Results: The time interval from the inclusion to the removal of the implants ranged from one month to 34 years; 44 patients have first operated at our service and 24 at other services. Forty three implants were, before the exchange, made of polyurethane, 13 texturized and six smooth. The major cause of the exchange was esthetic, with 50 cases, followed by capsular contracture, 13 cases and five cases of infection, rupture or breast nodule. Conclusion: The major reason of implants exchange was esthetic, differing from the literature. The capsular contracture and infection rates, at this study, were comparable to several others.
OBJETIVO: Analisar a freqüência e os fatores de risco associados à estenose carotídea acima de 70% em pacientes que serão submetidos a cirurgias de aorta abdominal. MATERIAL E MÉTODO: Foram analisados 94 pacientes que realizaram ultra-som Doppler de carótidas no pré-operatório de cirurgias de aorta abdominal entre janeiro de 2000 e janeiro de 2003, pela disciplina de Cirurgia Vascular da Santa Casa de São Paulo. RESULTADOS: Sessenta e sete pacientes (71%) eram homens. Dentre os 94 pacientes, 42 (44,6%) tinham doença oclusiva aorto-ilíaca, e 52 (53,4%), aneurismas da aorta abdominal (AAA). A análise dos dados mostrou uma prevalência de estenose de carótidas acima de 70% em 8,33% dos pacientes com AAA e em 13,51% dos pacientes com doença oclusiva aorto-ilíaca, diferença esta sem significância estatística (P = 0,5). Nos pacientes que apresentavam antecedente de isquemia cerebral - acidente vascular cerebral (AVC) ou ataque isquêmico transitório (AIT) -, houve uma prevalência estatisticamente maior de estenose carotídea entre 70 e 99%. Outros fatores de risco para aterosclerose, como sexo masculino, diabetes, hipertensão arterial e tabagismo, não foram preditivos da presença de estenose carotídea acima de 70%. CONCLUSÃO: A freqüência de estenose da carótida acima de 70% em pacientes no pré-operatório de cirurgia de aorta foi de 9,57%, e a presença de antecedente de AVC ou AIT na história foi preditiva de estenose acima de 70% neste grupo de pacientes.
