Perfil do gestor e práticas de gestão municipal no Sistema Único de Saúde (SUS) de acordo com porte populacional nos municípios do estado do Rio Grande do SulManager profile and practices of county health management in the Unified Health System (SUS) according to population size in the municipalities of the Rio Grande do Sul state Resumo O estudo buscou conhecer quem são os gestores municipais de saúde e práticas realizadas no âmbito de gestão. Possuiu delineamento transversal. A metodologia contou com o envio de um questionário eletrônico às secretarias de saúde dos 497 municípios do estado do Rio Grande do Sul (RS). Destes, 297 retornaram (59,75%). Contudo, foram considerados na análise estatística apenas os questionários com mais de 50% de respostas válidas (n = 264). A análise contou com o cálculo da distribuição das variáveis e com testes de qui-quadrado, onde o porte populacional foi considerado variável independente. A maioria foi composta por mulheres, com escolaridade alta e distintas formações, média de idade de 43,76 anos, cor branca. A média de gastos com saúde foi de ≈20% das receitas. Houve consenso quanto na insuficiência dos recursos. Quanto ao porte populacional, 79,92% possuíam ≤ 20.000 habitantes. Diferenças significativas foram observadas na gestão de municípios de distintos portes, tanto nas prioridades, quanto nas dificuldades. O estudo permitiu conhecer parcialmente a realidade da gestão municipal. Além disso, os resultados sugeriram a necessidade de novas pesquisas sobre a feminilização da gestão, avaliação de processos de trabalho e impactos da política de austeridade. It is based on a cross-sectional design. The methodology involved the use of electronic questionnaire, which was sent to the health departments of the 497 municipalities of the state of Rio Grande do Sul (RS). Of these, 297 were answered (59.75%). However, only those with more than 50% of valid answers (n = 264) were included in the statistical analysis. The analysis included the calculation of the distribution of variables and chi-square testes, taking population size as an independent variable. Most managers were women; graduated; from different professional backgrounds; average age of 43.76 years-old, and white. The average health expenditure in the municipalities was ≈ 20% of revenues. There was a consensus on the insufficiency of resources. With regards to population size, 79.92% presented with ≤ 20,000 inhabitants. Statistically significant differences were found in the management of municipalities of different sizes, both in terms of priorities and difficulties. The study allowed to partially understanding the reality of municipal management. These results indicate the need to further investigate the feminization of health management, working process assessment, and the impacts of economical austerity policy.
Objetivo: traçar o perfil clínico e epidemiológico de gestantes e puérperas internas em unidade de terapia intensiva COVID-19 em uma maternidade de alta complexidade na cidade de Teresina-PI. Métodos: trata-se de um estudo de cunho quantitativo, descritivo, retrospectivo de revisão de prontuário realizado em uma maternidade localizada na cidade de Teresina-Pioram englobados prontuários de gestantes com diagnóstico clínico ou sorológico de COVID-19. Foram excluídos prontuários com muitos dados incompletos (acima de 20), pacientes sem diagnóstico de COVID-19 e que os sintomas apareceram após o parto. Resultados: A amostra total desse estudo foi composta por 43 prontuários de pacientes que atenderam aos critérios estabelecidos, sendo a maioria mulheres na faixa de 18 a 30 anos de idade (60,4%; N=26), com um peso maior que 65kg (60,4%; N=26), multigestas (55,8%; N=24), que realizaram consultas pré-natais (88,3%; N=38) e apresentaram, no mínimo, uma intercorrência na gravidez (67,4%; N=29). As manifestações clínicas mais relatadas foram dispneia (95,3%; N=42) e tosse (81,3%; N=35). Além disso, a pesquisa revela alto índice de parto prematuro (80%; N=20) e cesariano (100%; N=25). 69,7% (N=30) da amostra precisou de suporte ventilatório. Faz-se necessária a realização de mais estudos para entender o verdadeiro perfil deste público e compreender as demais consequências que a doença traz para gestantes.
Neste estudo, propõe-se a produção de artefatos para o cuidado de usuários com lesões crônicas de pele na atenção primária à saúde (APS). Empreendeu-se pesquisa de natureza aplicada, abordagem qualitativa, com ancoragem teórica em princípios da gestão da clínica e sistematizada metodologicamente com base em técnicas e ferramentas de experimentação do design. A análise do conteúdo produzido integra dados de revisão integrativa da literatura, e de diálogos em grupos focais, envolvendo a equipe multiprofissional da APS. Desta análise, emergiram quatro categorias temáticas, nas quais são apresentados os respectivos artefatos produzidos, com vistas à qualificação do cuidado na APS a usuários com lesões crônicas de pele: mapa conceitual, com representação gráfica que permite uma visão geral das atribuições da equipe; algoritmo de atendimento para apoio a decisões clínicas, monitoramento e avaliação do cuidado; briefing que reúne as principais condutas terapêuticas de recurso mínimo e/ou opção de coberturas especiais fundamentais ao manejo clínico e social; e estrutura de registro para emprego de padrões de monitoramento, avaliação e suporte à decisão nas ações de cuidado. A pesquisa contribui para a implementação de princípios da gestão da clínica nos serviços de APS, na medida em que favorece a incorporação de práticas baseadas em evidências, bem como a orientação das ações clínicas compartilhadas e interprofissionais, com base nas necessidades de usuários com lesões crônicas de pele, e em padrões de qualidade e segurança, com foco no empreendimento de melhores práticas ao usuário no encontro com o serviço.
