OBJECTIVE To investigate the association between C-reactive protein (CRP) and high blood pressure (BP) in overweight or obese children and adolescents. METHODS Cross-sectional study with 184 overweight or obese children and adolescents aged from two to 18 years old, from April, 2009 to April, 2010. The classification of nutritional status used the body mass index (BMI). Based on the Centers for Disease Control and Prevention curve, individuals were classified as: overweight (BMI between the 85th-95th percentiles), obesity (BMI between 95th-97th percentiles) and severe obesity (BMI >97th percentile). Abnormal values were considered for systolic BP (SBP) and/or diastolic (DBP) if ≥90th percentile of the BP curve recommended for children and adolescents in the V Brazilian Guidelines on Hypertension, for waist circumference (WC) if ≥90th percentile of the curve established by the National Cholesterol Education Program, and for high sensitive CRP (hs-CRP) if >3mg/dL. To evaluate the association of inadequate values of CRP and the studied groups, chi-square test and analysis of variance were applied, using the Statistical Package for the Social Sciences version 17.0 and adopting a significance level of 5%. RESULTS Among the evaluated sample, 66.3% were female, 63.5%, non-white, 64.1% had severe obesity, 78.3% had altered WC and 70.6% presented high BP. There was a significant association of CRP high levels with altered WC and BMI ≥97th percentile. In adolescents, high CRP was related to high SBP. CRP mean values were higher in individuals with elevated SBP. CONCLUSIONS Inadequate values of hs-CRP were associated with severe obesity and high SBP in the studied population. These markers can be used to identify children and adolescents at higher risk for developing atherosclerosis.
INTRODUÇÃO: O hipotiroidismo congênito (HC) e a Fenilcetonúria (PKU) são as causas mais comuns de retardo mental preveníveis e têm os seus cursos naturais dramaticamente modificados, dependendo da época do diagnóstico e da instituição de tratamento adequado. OBJETIVOS: Avaliar o Programa para Rastreamento de HC e PKU, através do "teste do pezinho" na região de Campina Grande, entre 01/03/00 e 28/02/01. MÉTODO: Avaliamos a cobertura do rastreamento de HC e PKU na cidade de Campina Grande no período; incidência de HC e PKU, idade da coleta; tempo do recebimento do resultado e início do tratamento. RESULTADOS: A cobertura do Programa foi de 32,2%. Não foi diagnosticado nenhum caso de HC ou PKU no período do estudo. No serviço público, a média (±DP) de idade para a coleta foi de 18,2±12,2 dias, e de recebimento 56,7±27,4 dias. CONCLUSÃO: A cobertura do programa foi muito baixa no período estudado. O tempo decorrido entre o nascimento e recebimento dos exames é inadequado. O programa deve seguir as normas estabelecidas pelo Ministério da Saúde.
RESUMOIntrodução: O diabetes mellitus tipo 1 (DM1) freqüentemente se acompanha de outras doenças autoimunes, principalmente doença autoimune da tireóide (DAT). Objetivo: Determinar a prevalência de DAT em pacientes com DM1 e investigar possível relacionamento com outros fatores. Métodos: Em 126 pessoas com DM1, foram mensurados TSH, T 4 livre, anticorpo anti-peroxidase e hemoglobina A1c. Resultados: DAT foi encontrada em 26 pacientes (20,6%), sendo 11 (8,7%) com hipotireoidismo clínico, 6 (4,8%) com hipotireoidismo subclínico e 9 (7,1%) com tireoidite sem disfunção tireoideana. Em um paciente (0,8%), a DAT (hipertireoidismo) precedeu o DM1. Houve relação entre DAT e idade atual e no diagnóstico do DM1. Não foram encontradas significâncias para sexo, grupo racial, duração do DM, paridade e local de moradia. Conclusões: A prevalência de DAT é bastante alta para justificar seu rastreamento e está relacionada com a idade atual e a idade do diagnóstico do DM1. (DM1) is frequently associated with other immune diseases, especially autoimmune thyroid disease (ATD). Objective: To determine the prevalence of ATD in subjects with DM1 and to investigate possible association with other factors. Methods: TSH, free T 4 , antiperoxydase antibody, and hemoglobin A1c were measured in 126 DM1 patients. Results: ATD was found in 26 (20.6%) patients being 11 (8.7%) with overt hypothyroidism, 6 (4.8%) with subclinical hypothyroidism and 9 (7.1%) with thyroiditis without thyroid dysfunction. In one patient (0.8%), ATD (hyperthyroidism) preceded DM1. ATD was associated with current age and age at diagnosis of DM1. No associations of ATD with sex, race, duration of DM1, parity, and home place were observed. Conclusions: The prevalence of ATD in DM1 patients is high enough to justify screening and is related to current age and to the age at the diagnosis of DM1. Essa associação de endocrinopatias autoimunes num mesmo indivíduo (síndrome poliglandular) aparentemente não tem relação com o controle do DM1 e sim com o status imunogenético do paciente e/ou com fatores ambientais.
