Objetivo: Desenvolver um modelo simulador de baixo custo para treinamento de técnicas de Septoplastia para médicos residentes em Otorrinolaringologia. Métodos: Trata-se de uma pesquisa tecnológica com desenvolvimento experimental de um protótipo de cirurgia. O modelo foi constituído por um septo artificial produzido com materiais de baixo custo e fácil aquisição, associado à um suporte em formato de hemi-crânio desenvolvido e impresso em impressora 3D. Adaptações foram realizadas para garantir que o resultado final fosse o mais objetivo e prático possível, facilitando assim sua utilização. Resultados: Com o modelo, foi possível treinar as etapas iniciais de um procedimento de septoplastia, podendo ser realizado em variados ambientes, devido seu baixo peso, facilitando seu transporte. Permite que o usuário se familiarize com os instrumentais de septoplastia e o uso de fotoforo em um espaço cirúrgico limitado que é a cavidade nasal, dando-lhes a oportunidade de melhorar a destreza manual, essencial durante um procedimento real. Conclusão: O presente protótipo, oferece uma nova opção para treinamento e para melhorar as habilidades cirúrgicas do médico residente, conferindo maior segurança e destreza, mediante treinamento prévio em um modelo específico, para posterior transferência destas habilidades para um ambiente cirúrgico real.
Objetivo: Correlacionar alterações clínicas e questionários de sono com achados polissonográficos em pacientes com Apnéia Obstrutiva do Sono (AOS) atendidos em ambulatório de sono de um hospital de referência. Métodos: Estudo observacional, transversal, realizado em um hospital de referência em otorrinolaringologia. Foram selecionados 35 pacientes com queixa de roncos e que já haviam realizado polissonografia. Os pacientes preencheram três questionários de sono: Escala de Sonolência de Epworth (ESE), Questionário de Berlim e Questionário STOP-BANG. Resultados: AOS foi encontrada em 83% dos pacientes, sendo 55,2% homens e 44,8% mulheres, maioria na sexta década de vida (41.4%). A média do Índice de Massa Corporal (IMC) foi de 32,6 (± 4,9 DP) e da circunferência cervical, foi de 39,7 cm (± 4,9 DP). Quanto à sensibilidade e especificidade dos questionários em predizer risco de AOS, obteve-se que a ESE apresentou sensibilidade de 68,97 % e especificidade de 33,33%, STOP-BANG apresentou sensibilidade 93.1% e especificidade 16,6% e o Questionário de Berlim, sensibilidade 96,5% e especificidade 0%. Conclusão: Os questionários ESE, Berlim e STOP-BANG, identificaram pacientes de alto risco para AOS, mas não excluíram com precisão os de baixo risco.
Objetivo: Demonstrar um modelo de simulador cirúrgico, prático e de baixo custo, para a prática de timpanotomia com colocação de tubo de ventilação. Método: Desenvolvimento experimental de um modelo para treinamento cirúrgico, construído a partir de uma infra-estrutura com duas plataformas de madeira, uma como base e outra como suporte para o molde, interligadas por uma haste articulada de metal. O molde foi formado a partir de seringas serradas em um comprimento de 2,5 cm, com um anel membranoso de Policloreto de Vinil fixado com um amarrilho de plástico em sua ponta, simulando respectivamente conduto auditivo externo e membrana timpânica. Resultado: O simulador após montado permite a prática do procedimento desejado, além de outros como aspiração, retirada de cerume e corpos estranhos do conduto adutivo externo, podendo ser executados com auxilio tanto do microscópio, como do endoscópio rígido. Conclusão: O modelo é uma alternativa barata e prática para treinamento de timpanotomia com colocação de tubo de ventilação e outros procedimentos da rotina otorrinolaringológica, e o treinamento com o uso do simulador permite ao usuário dominar as habilidades necessárias e se familiarizar com os equipamentos e instrumentais para a execução destes procedimentos no ambiente cirúrgico real.
Objetivo: Avaliar a presença de disfagia e seu impacto na qualidade de vida de pacientes portadores de doença de Parkinson (DP), correlacionando o estadiamento da doença, a severidade da disfagia e a auto avaliação do risco de disfagia. Métodos: Estudo observacional e transversal, avaliando 24 pacientes portadores de DP através da escala para sintomas motores de Hoenh e Yahr, da avaliação endoscópica da deglutição (VED), de questionário sobre a qualidade de vida (The 39 item Parkinson´s Disease Question - PDQ-39) e de um instrumento de auto avaliação do risco de disfagia e aspiração (Eating Assessment Tool - EAT-10). Resultados: Na VED, 87,5% dos pacientes apresentaram disfagia, com gravidade independente da severidade motora da doença. Não houve significância entre a gravidade da disfagia e a auto avaliação de disfagia e aspiração. No PDQ-39 houve pior percepção sobre a qualidade de vida com o avançar da doença (p=0,003), com significância no domínio da cognição nos disfágicos graves. Conclusão: a disfagia é um sintoma comum e independente da severidade motora da DP, devendo ser investigada e reabilitada por equipe multiprofissional precocemente a fim de reduzir o impacto na qualidade de vida destes pacientes.
