ObjectivesTo evaluate function among patients with postoperative recurrence of rotator cuff injuries that was treated arthroscopically (case series) and compare this with function in patients without recurrence (control group); and to compare function among patients with recurrence of rotator cuff injuries that were greater than and smaller than 3 cm.MethodsThis was a retrospective evaluation of patients who underwent arthroscopic revision of rotator cuff injuries using the ASES, Constant & Murley and UCLA scores and a visual analog pain scale, in comparison with patients in a control group who underwent primary rotator cuff repair.ResultsThe size of the rotator cuff injury recurrence had a statistically significant influence on the result from the arthroscopic surgical treatment. The functional scores showed worse results than those from the first procedure.ConclusionArthroscopic surgical treatment of renewed tearing of rotator cuff injuries showed worse functional scores than those from primary repair of the injury.
ObjectiveTo demonstrate the relationship between the size, degree of retraction and topography of rotator cuff injuries and the degree of rise of the humeral head, and to evaluate the influence of gravity, using magnetic resonance imaging (MRI).MethodsWe evaluated 181 shoulder MRIs from 160 patients aged over 45 years, between November 2013 and July 2014. The patients were divided into two groups: one control (no lesion or partial damage to the rotator cuff); and the other with complete tears of the rotator cuff. We measured the acromiohumeral distance in the sagittal plane, and established the shortest distance between the apex of the head and the acromion.ResultsIn this study, 96 examinations on female patients (53.04%) and 58 on male patients (46.96%) were evaluated. The mean age was 63.27 years: in the control group, 61.46; and in the group with injuries, 65.19. From analysis on the measurements of the subacromial space, we observed significantly higher values in the control group (7.71 mm) than in the group with injuries (6.99). In comparing the control group with some specific subgroup, i.e. posterosuperior (6.77), anteroposterior-superior (4.16) and retraction Patte III (5.01), we confirmed the importance of topography and degree of retraction in relation to the rise of the humeral head.ConclusionThe rise of the humeral head was directly related to the size, degree of retraction and topography of the rotator cuff injuries, with greater degrees of rise in cases of superior and posterior lesions and anteroposterior-superior (massive) lesions. The assessment using MRI was not influenced by the force of gravity.
ResumoAvaliar o resultado funcional da técnica de remplissage, a cicatrização da capsulotenodese do tendão infraespinal no defeito de Hill-Sachs, o grau de infiltração gordurosa do músculo infraespinal e sua força de rotação lateral no pós-operatório.Foram avaliados 25 pacientes com luxação anterior recidivante do ombro e lesão de Hill-Sachs com índice de Hardy maior do que 20%, submetidos à técnica artroscópica de remplissage com seguimento mínimo de um ano. Os pacientes foram submetidos a avaliação clínica (escores funcionais de Carter-Rowe e Walch-Duplay, medição de amplitude de movimento e força) e exame de ressonância magnética no ombro operado.Dos pacientes, 88% e 92% apresentaram resultados bons ou excelentes nas avaliações funcionais pelos escores de Carter-Rowe e Walch-Duplay, respectivamente. Identificou-se diferença média de 1 kg a menos de força do membro operado em relação ao contralateral (p < 0,001) e diferença média de 10∘ em rotação lateral 1 e 2 (p < 0,001), novamente com o uso como referência do lado contralateral. Todos os pacientes submetidos a ressonância magnética apresentaram preenchimento de alto grau da lesão de Hill-Sachs pela capsulotenodese, assim como ausência ou mínima infiltração gordurosa no músculo infraespinal.A técnica de remplissage apresentou resultados bons/excelentes nos escores funcionais, apesar da perda discreta de força e amplitude de rotação lateral com significância estatística. Foram observados resultados excelentes quanto à cicatrização da capsulotenodese e ao preenchimento do defeito de Hill-Sachs.
Resumo
Objetivo Analisar o resultado funcional dos pacientes submetidos ao tratamento cirúrgico videoartroscópico da ruptura completa do manguito rotador do ombro, com seguimento mínimo de 10 anos.
Métodos Foram avaliados 63 pacientes (63 ombros) submetidos ao reparo cirúrgico videoartroscópico da ruptura completa do manguito rotador com seguimento mínimo de 10 anos. O resultado funcional no pós-operatório desses pacientes foi avaliado pelos escores de Constant e UCLA.
Resultados Os valores médios foram de 26 pontos no escore UCLA e de 93 no escore de Constant. Para o escore de Constant, 91% foram considerados satisfatórios; e para o UCLA, 62% dos pacientes apresentaram escores satisfatórios.
Conclusão O reparo artroscópico da ruptura completa do manguito rotador mostrou-se efetivo mesmo a longo prazo (seguimento mínimo de dez anos). A idade dos pacientes antes da cirurgia, o tamanho da lesão, o grau de infiltração gordurosa e a avaliação do trofismo muscular foram importantes preditores de prognóstico.
Resumo
Objetivo Realizar a tradução e adaptação cultural à língua portuguesa do Long Head of Biceps Tendon (LHB).
Métodos O processo envolveu a produção de traduções por indivíduos com domínio da língua-alvo, retrotraduções de maneira independente, criação de um comitê para comparar as versões original e traduzida, realização de pré-teste com a versão final, e elaboração da versão final.
Resultados O questionário foi traduzido e adaptado conforme a metodologia proposta. Na primeira versão em português (VP1), houve divergências na tradução de doze termos. A retrotradução da VP1 apresentou, quando comparada à versão original, divergência em oito termos. A segunda versão em português (VP2), elaborada por um comitê, foi aplicada a um grupo pré-teste constituído por 30 participantes, e ao final chegou-se à terceira versão em português, denominada LHB-pt.
Conclusão A tradução e adaptação do LHB foram concluídas com sucesso.
