• O autores declaram que não há conflito de interesse. ResumoObjetivo: relatar caso clínico de tumor odontogênico adenomatoide (TOA), enfatizando suas características e diagnósticos diferenciais. Relato de caso: paciente masculino, 16 anos, apresentando lesão radiolúcida, unilocular, associada ao dente 24 incluso. Sob as hipóteses de TOA, cisto dentígero, tumor odontogênico cístico calcificante e ameloblastoma unicístico, foi realizada biópsia excisional. Na microscopia, observou-se parênquima contendo estruturas tubulares e áreas semelhantes à roseta e cribriformes, estabelecendo o diagnóstico de TOA. Conclusão: o reconhecimento das características clínicas, radiográficas e histopatológicas do TOA é essencial para guiar o processo para se estabelecer o diagnóstico definitivo e, consequentemente, permitir o tratamento adequado do paciente.Palavras-chave: Tumores odontogênicos; Diagnóstico diferencial; Cistos odontogênicos; Características radiográficas. AbstRActObjective IntroduçãoO tumor odontogênico adenomatoide (TOA) é um tumor odontogênico benigno, de origem epitelial, que, desde a sua primeira descrição, exibiu diferentes nomenclaturas -adamantioma, adenoameloblastoma, tumor adenomatoide ameloblástico e ameloblastoma adenomatoide. 1,2 Durante um período, foi considerado uma variante do ameloblastoma, no entanto,em 1971, foi incluído como um tumor odontogênico distinto sob a nomenclatura TOA na primeira edição do livro de Tumores de Cabeça e Pescoço elaborado pela Organização Mundial de Saúde (OMS), sendo mantido e definido em sua versão mais recente do ano de 2005 como um tumor odontogênico composto apenas por epitélio odontogênico em uma variedade de padrões histoarquiteturais. 2,3O TOA é um tumor odontogênico incomum com frequência relativa variando entre 2 e 7% de todos os tumores odontogênicos.2,4 Clinicamente, a lesão é caracterizada por um aumento de volume, de crescimento lento e progressivo, podendo haver ou não a presença de sintoma.5 Acomete principalmente a região anterior da maxila de pacientes do sexo feminino na segunda década de vida, especialmente adolescentes, 4-6 e geralmente é descoberto em exame radiográfico de rotina como uma lesão radiolúcida unilocular envolvendo a coroa de um dente canino incluso. 7O diagnóstico definitivo é obtido pelo exame microscópico, que revelará uma proliferação de epitélio odontogênico em uma variedade de padrões arquiteturais, sendo o mais comum composto por células epiteliais fusiformes, colunares ou cuboidais, formando ninhos ou estruturas semelhantes a ductos e a rosetas. 1,2,5 O aspecto clínico e radiográfico do TOA é relativamente constante e pode ser sugestivo, mas, ainda assim, geralmente inclui uma variedade de cistos e tumores odontogênicos em seu diagnóstico diferencial.
