We were unable to demonstrate any association between the GST genotypes studied and the risk of ovarian cancer but the inheritance of a heterozygous Arg/Pro genotype of p53 increased the risk of ovarian cancer more than 2.5 times (OR = 2.571; 95% CI = 1.453-4.550). There was no association of the studied genes to any clinical or pathological feature of the patients or to their response to chemotherapy.
IntroductionRecently, studies have demonstrated that the addition of bevacizumab to chemotherapy could be associated with better outcomes in patients with advanced non-small cell lung cancer (NSCLC). However, the benefit seems to be dependent on the drugs used in the chemotherapy regimens. This systematic review evaluated the strength of data on efficacy of the addition of bevacizumab to chemotherapy in advanced NSCLC.MethodsPubMed, EMBASE, and Cochrane databases were searched. Eligible studies were randomized clinical trials (RCTs) that evaluated chemotherapy with or without bevacizumab in patients with advanced NSCLC. The outcomes included overall survival (OS), progression-free survival (PFS), response rate (RR), toxicities and treatment related mortality. Hazard ratios (HR) and odds ratios (OR) were used for the meta-analysis and were expressed with 95% confidence intervals (CI).ResultsWe included results reported from five RCTs, with a total of 2,252 patients included in the primary analysis, all of them using platinum-based chemotherapy regimens. Compared to chemotherapy alone, the addition of bevacizumab to chemotherapy resulted in a significant longer OS (HR 0.89; 95% CI 0.79 to 0.99; p = 0.04), longer PFS (HR 0.73; 95% CI 0.66 to 0.82; p<0.00001) and higher response rates (OR 2.34; 95% CI 1.89 to 2.89; p<0.00001). We found no heterogeneity between trials, in all comparisons. There was a slight increase in toxicities in bevacizumab group, as well as an increased rate of treatment-related mortality.ConclusionsThe addition of bevacizumab to chemotherapy in patients with advanced NSCLC prolongs OS, PFS and RR. Considering the toxicities added, and the small absolute benefits found, bevacizumab plus platinum-based chemotherapy can be considered an option in selected patients with advanced NSCLC. However, risks and benefits should be discussed with patients before decision making.

In this large retrospective study, the standard prognostic factors impacted on survival whereas the timing of adjuvant therapy did not. Patients with delayed adjuvant chemotherapy may have worse prognostic factors which could play a major role in their poor outcome.
A síndrome de Li-Fraumeni (SLF) consiste em um distúrbio autossômico dominante, associado a mutações no gene da proteína tumoral p53. Apresenta uma maior expressão em mulheres jovens e está associada à ocorrência de determinados tipos de câncer, tais como sarcomas, leucemias, tumores cerebrais, câncer de mama e carcinomas adrenocorticais. Esta síndrome não apresenta quadro clínico ou fenótipo específico, suas manifestações clínicas estão em linha com o tipo de câncer apresentado e seu diagnóstico é baseado em critérios clínicos e confirmado por testes genéticos que analisam a mutação do gene p53. Dessa forma, a detecção precoce da mutação, a realização de testes de rastreamento e o aconselhamento genético podem ser capazes de alterar a história natural da doença e, consequentemente, reduzir a mortalidade associada aos principais tipos de neoplasias associadas à síndrome. Assim como no quadro clínico, não há tratamento específico para tal diagnóstico. A abordagem oncológica será, portanto, de acordo com o tipo de câncer que o paciente desenvolve. Pacientes com SLF parecem apresentar uma maior sensibilidade para desenvolver neoplasias induzidas por radiação, devendo-se então, avaliar individualmente indicações de alguns exames diagnósticos de imagem e propostas terapêuticas envolvendo radiação. Diante do contexto de novas perspectivas diagnósticas, de rastreio e seguimento trazemos aqui uma atualização sobre a SLF, ilustrada sob o panorama de um caso clínico norteador.
Resumo: A polimixina B é um antimicrobiano polipeptídico com atividade bactericida contra bactérias aeróbias gram-negativas, inclusive contra bactérias multirresistentes. A neuro e a nefrotoxicidade são efeitos colaterais consagrados, porém foi observado presença de hiperpigmentação cutânea marrom-acastanhada em face, pescoço, em região posterior do tronco, nádegas e região posterior e proximal dos membros inferiores cinco dias após o início da administração deste antibiótico em uma paciente internada na Enfermaria de Clínica Médica de um hospital do interior do Estado do Ceará. A hiperpigmentação cutânea induzida por Polimixina B é um efeito colateral considerado pouco descrito na literatura médica. Os mecanismos etiopatogênicos que causam esta alteração rara permanecem desconhecidos, mas podem envolver reações de estresse oxidativo, provocando uma liberação de histamina, que tem efeito melanogênico, culminando com a deposição de melanina na derme. Ainda que não seja um efeito adverso com risco de vida, a hiperpigmentação cutânea pode gerar danos estéticos importantes, além de distúrbios psicológicos, porém várias questões a respeito dessa reação permanecem obscuras, como possíveis terapêuticas que poderiam ser utilizadas e prognóstico. Diante disso, o objetivo deste Trabalho foi relatar um caso de hiperpigmentação cutânea induzida por Polimixina B em paciente internada no setor de Enfermaria de Clínica Médica em um hospital do interior do Estado do Ceará, além de revisar a literatura sobre o mesmo assunto. Para isso, foi realizada uma busca ativa dos dados clínico-epidemiológicos e socioambientais presentes nos arquivos do hospital em questão, no prontuário da paciente, além de busca e leitura de periódicos nas bases de dados PubMed e SciELO no ano de 2022.Palavras-chave: Hiperpigmentação cutânea; Polimixina B. Abstract: Polymyxin B is a polypeptide antimicrobial with bactericidal activity against aerobic gram-negative bacteria, including multidrug-resistant bacteria. Neurotoxicity and nephrotoxicity are consecrated side effects, but brown-brown cutaneous hyperpigmentation was observed on the face, neck, posterior region of the trunk, buttocks and posterior and proximal regions of the lower limbs five days after the beginning of the administration of this antibiotic in a patient hospitalized in the Internal Medicine Infirmary of a hospital in the interior of the State of Ceará. Polymyxin B-induced cutaneous hyperpigmentation is a side effect considered to be poorly described in the medical literature. The etiopathogenic mechanisms that cause this rare alteration remain unknown, but they may involve oxidative stress reactions, causing a release of histamine, which has a melanogenic effect, culminating in the deposition of melanin in the dermis. Although not a life-threatening adverse effect, cutaneous hyperpigmentation can cause significant aesthetic damage, in addition to psychological disorders, but several questions regarding this reaction remain unclear, such as possible therapies that could be used and prognosis. Therefore, the objective of this work was to report a case of cutaneous hyperpigmentation induced by Polymyxin B in a patient admitted to the Internal Medicine Infirmary in a hospital in the interior of the State of Ceará, in addition to reviewing the literature on the same subject. For this, an active search was carried out for the clinical-epidemiological and socio-environmental data present in the files of the hospital in question, in the patient's chart, in addition to searching and reading journals in the PubMed and SciELO databases in the year 2022.Keywords: Skin hyperpigmentation; Polymyxin B.
O câncer de próstata é uma das malignidades mais comuns ao redor do mundo. O rastreamento e o diagnóstico precoce seguem sendo pilares para cura dos pacientes de risco desfavorável. No entanto, há uma preocupação a respeito do tratamento excessivo dos tumores de menor risco, onerando o sistema de saúde e causando potenciais efeitos colaterais irreversíveis em homens cuja qualidade de vida e sobrevida não seriam afetados pela doença. A vigilância ativa (VA), estratégia iniciada nas últimas duas décadas, é o tratamento de escolha nos pacientes com câncer de próstata de risco muito baixo,baixo e em alguns selecionados com risco intermediário favorável. Os principais protocolos de VA contemporâneos variam entre si nas suas coortes, manejos no monitoramento e no melhor momento para a intervenção. No entanto, os reportes de mortalidade câncer-específica e taxas de doença metastática são extremamente baixos no seguimento mediano de médio prazo (5-10 anos). Estes desfechos parecem estar intimamente associados aos diferentes critérios de seleção, monitoramento e intervenção, sugerindo que a estratégia da VA pode ser individualizada baseando-se no nível de risco aceitável para os pacientes, levando em consideração suas preferências pessoais. Novas tecnologias, como a ressonância nuclear magnética multiparamétrica e testes moleculares parecem tornar a VA mais segura. A utilização da VA, embora em crescimento, permanece altamente variável nos diferentes países e a educação continuada para segurança desta estratégia segue um desafio para que seu uso seja cada vez mais difundido.
