Introdução A migrânea é uma doença prevalente e incapacitante, cuja fisiopatologia engloba secreção de citocinas como IL-1β e inflamação neurogênica. Objetivo Avaliar a associação entre as variantes genéticas +3953 C>T e -511 C>T do gene IL1β com a suscetibilidade e efeitos clínicos da migrânea. Material E Métodos Estudo prospectivo observacional tipo caso-controle, composto por 156 participantes (81 com migrânea e 75 controles), paredos por sexo, idade, etnia e IMC. Projeto aprovado pelo Comitê de Ética em Pesquisa, no 98316718.7.0000.0020. Foi realizada entrevista estruturada e obtenção de dados demográficos, clínicos, antropométricos e relacionados à migrânea e suas características, além de questionário validado sobre incapacidade da migrânea (MIDAS). O DNA foi obtido por amostra de sangue periférico ou material da mucosa oral. Genotipagens foram feitas por PCR-SSP. Suscetibilidade à migrânea foi determinada por regressão logística binária e análise das características clínicas por teste de Qui-quadrado e Mann Whitney. Considerou-se diferença estatística p≤0,05. Resultados O alelo C da variante +3953, menor produtor da citocina, foi associado a menor suscetibilidade a migrânea (OR=0,50; p=0,02) no modelo alélico, bem como o genótipo CC (OR=0,37; p=0,048) no modelo codominante após ajuste para sexo, idade, IMC e etnia. Não houve diferença nos modelos dominante e recessivo da variante +3953 C>T. A variante -511 C>T não influenciou a suscetibilidade à migrânea. O alelo C da variante +3953 apresentou maior prevalência de aura (p=0,04) e menor prevalência de osmofobia (p=0,002). O alelo T da variante -511, maior produtor da citocina, foi associado à maior frequência de fonofobia (p=0,011) e incapacidade relacionada à migrânea (p=0,041). Conclusão A variante +3953 C>T pode influenciar a suscetibilidade à migrânea (genótipo CC associado a chance 67% menor de doença). As variantes estudadas podem influenciar características clínicas da migrânea.
IntroductionThe main mechanisms in the physiopathology of migraine are cortical spreading depression and trigeminal activation with the release of CGRP. Neurogenic inflammation and neuroinflammation can exert an influence over both these mechanisms, but there are still a number of gaps in our understanding. Interleukin (IL)- 1β is a pro-inflammatory cytokine whose levels of plasma increase during the attack phase of migraine. Thus far, its genetic variants have not been well studied. ObjectiveTo investigate the association between the genetic variants IL1B +3954C>T (rs1143634) and -511C>T (rs16944) and susceptibility to migraine and its clinical characteristics. Subjects and MethodsCase control study comprising 307 participants, of whom 152 had a diagnosis of migraine and 155 were healthy controls, paired by sex, age, ethnicity and BMI. The clinical and demographic data were evaluated. The patients with migraine were interviewed using a structured form containing information about the type of migraine (with or without aura, episodic or chronic), age at onset of the disease, frequency of attacks, accompanying symptoms that triggered headaches. The patients also answered validated questionnaires to evaluate incapacity (Migraine Disability Assessment - MIDAS) and impact (Headache Impact Test - HIT-6) for migraine, the presence of allodynia (ASC-12), as well as symptoms of anxiety (State Anxiety Inventory - STAI 1 and 2), depression (Beck Depression Inventory) and a hyperacusis scale.The genetic variants IL1B +3954C>T (rs1143634) and -511C>T (rs16944) were identified using polymerase chain reaction (PCR) and the fluorescence levels of PCR products were evaluated using a Step One thermocycler (Applied Biosystems). The analyses were conducted using the dominant, codominant, recessive and over- dominant genetic models. Categorical data were evaluated via the chi-squared test or Fisher’s exact test, and continuous data were evaluated using the Mann-Whitney test. Objetivos Analisar a influência das variantes do gene IL6 -174 G>C e -597 G>A na suscetibilidade e parâmetros clínicos da migrânea. Material E Métodos Trata-se de um estudo prospectivo observacional do tipo caso-controle, composto por 92 participantes, sendo 44 pacientes com migrânea e 48 controles saudáveis, pareados para sexo, idade, etnia e IMC. O projeto foi aprovado pelo Comitê de Ética em Pesquisa, nº 98316718.7.0000.0020. Foram realizadas entrevistas estruturadas e obtidos dados demográficos, clínicos, antropométricos e relacionados à migrânea e suas características. Os participantes responderam aos questionários validados referentes a incapacidade da migrânea (MIDAS) e alodinia (ASC-12). O DNA foi obtido por extração de leucócitos do sangue periférico e material da mucosa oral. As genotipagens foram feitas por PCR-SSP. Para determinar a suscetibilidade à migrânea, foi realizada regressão logística binária e, para análise das características clínicas, o teste de Qui-quadrado e Mann Whitney. Foi considerada diferença estatística quando p ≤ 0,05. Resultados Indivíduos com o genótipo CC da variante -174 G>C, menor produtor da citocina pró-inflamatória, apresentaram menor suscetibilidade à migrânea, quando comparados a indivíduos com o genótipo GG ou GC no modelo recessivo (OR = 0,14; p = 0,026). Não houve influência da variante -597 G>A na suscetibilidade à migrânea. Ambas as variantes não demonstraram associação com a forma episódica ou crônica, presença de aura, pósdromo, pródromo, fonofobia, fotofobia, osmofobia, incapacidade e alodinia. Contudo, este resultado pode ter sido limitado pelo pequeno número amostral. Conclusões O genótipo CC da variante -174 G>C do gene IL6 demonstrou exercer efeito protetor ao desenvolvimento da migrânea, com chance 86% menor de migrânea quando comparado aos genótipos GG ou GC.
