Introducción: En lactantes y preescolares, la alergia a las proteínas de la leche de vaca (APLV) es la más común. Existe una variabilidad en la presentación clínica, por lo cual el diagnóstico es difícil. Cuando no hay un diagnóstico de certeza, es común observar un manejo inadecuado. Objetivo: Describir el abordaje del diagnóstico y tratamiento de la APLV por pediatras en un hospital privado de la Ciudad de México en comparación con las guías internacionales. Material y métodos: Estudio transversal, observacional y descriptivo. Se recabaron datos por medio de una encuesta realizada a los padres de pacientes con diagnóstico de APLV. Se obtuvieron las frecuencias simples y porcentajes. Resultados: de 62 pacientes, 61.3% fueron hombres con una mediana de edad de 11 meses. El principal sistema afectado fue el gastrointestinal. Para la confirmación diagnóstica, en 61.3% de los casos se utilizó únicamente dieta de eliminación y la mejoría clínica del paciente posterior a ésta.
