348CARTAS AL EDITOR ---Replantearse la posibilidad de la prueba genética si la paciente inicia sintomatología compatible que haga necesario un diagnóstico preciso, o bien si ella misma lo demanda cuando sea mayor de edad o presente madurez suficiente a criterio de los profesionales que la atienden, previo consentimiento informado y apoyo psicológico apropiado 6 . El sarcoma de Kaposi (SK), descrito por Moritz Kaposi en 1872, es una neoplasia vascular multifocal, asociada a la infección por el virus del herpes humano del tipo 8 (VHH8), con 4 variantes (clásica, endémica, epidémica y iatrogénica o asociada a la inmunosupresión) 1,2 . La variante iatrogé-nica es rara en la edad pediátrica, lo que dificulta su abordaje.Presentamos el caso de una adolescente de 12 años, de raza negra, ingresada con un cuadro de fallo multiorgánico. La biopsia renal sugería nefritis lúpica. Inició tratamiento de sustitución renal e inmunosupresora, con dosis altas de metilprednisolona y posteriormente con prednisolona. Se administró una vez por semana rituximab durante un mes y un ciclo mensual de ciclofosfamida durante 4 meses.Tres meses después de la admisión, se observaron a nivel de los miembros inferiores 5 lesiones cutáneas papulonodulares de coloración marrón y, posteriormente, 2 lesiones en la hemicara derecha ( fig. 1). No presentaba otras alteraciones en exploración física.En la biopsia cutánea se observó proliferación neoplásica fusocelular con diferenciación vascular (CD31 + ) y características morfológicas de SK (hendiduras vasculares rodeadas de células fusiformes con núcleos grandes e hipercromá-ticos). El estudio inmunohistoquímico fue positivo para el VHH8 ( fig. 2). Además, la amplificación del ADN del VHH8 fue positiva en sangre. Se excluyó la infección por el virus de la inmunodeficiencia humana y la ecografía abdominal y tomografía computarizada torácica no revelaron alteraciones.Se estableció el diagnóstico de SK con afectación cutánea exclusiva de probable etiología iatrogénica. La estrategia terapéutica incluyó la reducción gradual de la inmunosupresión y el uso de imiquimod tópico.Un año después de la reducción de la inmunosupresión las lesiones se encuentran en regresión completa.El SK iatrogénico se ha descrito de forma creciente en poblaciones adultas 2,3 . En la edad pediátrica se han relatado pocos casos asociados al trasplante renal, hepático y de médula ósea 4---7 . Su frecuencia varía en diferentes áreas geográficas, evidenciando la importancia de factores étnicos y ambientales. Predomina en países en vías de desarrollo, pudiendo estar asociada a una mayor prevalencia de infección por el VHH8. Los factores genéticos parecen determinantes dada su mayor incidencia en poblaciones «no blancas» en una misma área 1,2,8 . Ifosfamide is an antineoplastic drug widely used in the treatment of paediatric malignancies. However, in up to 50% of patients, it is associated with nephrotoxicity ranging from asymptomatic tubulopathy to overt renal failure. A two-year-old Caucasian boy was diagnosed with stage IV Burkitt lymphoma, with hepatic and renal and bone marrow involvement. Baseline evaluation showed GFR of 60 mL/min/1.73 m 2. He started chemotherapy with LMB 96 protocol group B with 5 courses of chemotherapy with vincristine, cyclophosphamide, doxorubicin, prednisone, and intrathecal administration of cytarabine and methotrexate. After the first chemo protocol, the patient was in remission and recovered normal renal function, but three months later he relapsed with involvement of the liver and kidneys. The patient initiated a salvage regimen with RICE (rituximab-ifosfamide, carboplatin, etoposide) intrathecal methotrexate and cytarabine and allogeneic stem cell transplantation was proposed. After achieving complete remission, he underwent allogeneic stem cell transplantation with busulfan and cyclophosphamide. Graft versus host disease prophylaxis was made with tacrolimus and methotrexate and infection prophylaxis with fluconazole and acyclovir. However, during treatment of the relapse, the patient presented downward crossing of weight and height centiles without recovery after treatment terminus and, following transplantation, consecutive laboratory testing suggested tubulopathy. Upon nephrologist referral, he was diagnosed with Fanconi syndrome and adequate supplementation was initiated with improvement of the patient's general condition and slow centile catch up. After literature review, the most probable causing agent was determined to be ifosfamide, as the nephrotoxic effects of the other medications prescribed tend to be reversible after stopping. Nephrotoxicity secondary to chemotherapy is a major cause of morbidity in paediatric cancer survivors. Our case represents a rare situation with unspecific clinical signs. Clinicians must be alert to the necessity of close monitoring to identify renal toxicity, mainly tubular dysfunction, as early as possible and allow adequate supplementation, which is crucial in preventing side effects.
