O câncer gástrico ainda constitui importante problema de saúde pública no estado do Pará, onde as taxas de mortalidade apresentam valores acima da média brasileira. As bases históricas de ocupação de espaço na Amazônia, bem como as relações econômico-sociais estabelecidas ao longo desse processo, marcaram o padrão alimentar desse Estado. Este trabalho tem por objetivo reconstruir as principais características do padrão alimentar dessa população no século passado. Procurou-se identificar todas as publicações científicas disponíveis que relacionavam dados sobre os hábitos alimentares dos residentes do Pará no século XX. A partir dessas publicações, foi possível caracterizar o padrão alimentar da população paraense no século XX, que incluía um elevado consumo de sal, utilizado na conservação de carnes e frutos do mar, um consumo relativamente reduzido e irregular de legumes e verduras e uma importante ingestão de glicídios, a partir da farinha de mandioca, muitas vezes adicionada de corantes artificiais. O padrão de dieta retratado, provavelmente comum a outras regiões da Amazônia, pode ser caracterizado, com base na literatura científica, como potencialmente favorecedor de certas condições associadas à carcinogênese gástrica. Nesse sentido, ele poderia ter contribuído, em parte, para a alta mortalidade por essa neoplasia registrada na região Norte.
RACIONAL: O câncer gástrico vem apresentando uma diminuição da incidência e da mortalidade em vários países, inclusive no Brasil. Entretanto, ainda se constitui em importante problema de saúde pública, especialmente no Estado do Pará, onde as taxas de mortalidade apresentam valores acima da média brasileira. OBJETIVO: Analisar os padrões de distribuição da mortalidade por câncer gástrico no Estado do Pará e em Belém, nas duas últimas décadas. MATERIAL E MÉTODOS: Os dados de mortalidade foram obtidos no Sistema de Informações sobre Mortalidade do Ministério da Saúde e as informações populacionais a partir dos dados do IBGE (Instituto Brasileiro de Geografia e Estatística). Foram calculadas taxas de mortalidade por câncer gástrico para o conjunto da população e segundo sexo, ajustadas por idade, para cada triênio no período 1980-97, para o Estado do Pará e para Belém. A tendência das taxas padronizadas de mortalidade por câncer de estômago foi analisada com base em modelos de regressão linear. RESULTADOS: As taxas padronizadas de mortalidade por neoplasia de estômago, em Belém, apresentaram magnitudes bastante elevadas (27,2 a 15,8/100.000), comparativamente a outras capitais brasileiras e países. A magnitude das taxas do sexo masculino foi bastante superior a das mulheres, sendo que, em Belém, a razão de sexo manteve-se próxima a 3,0 em todos os períodos analisados, com exceção de 1995-97. No Estado do Pará, observa-se tendência de queda das taxas de mortalidade em ambos os sexos, mais marcante no sexo masculino, no período de estudo. Em Belém, também se configurou uma tendência de declínio em todas as faixas etárias. Apesar do declínio observado, as taxas de mortalidade por câncer de estômago em Belém, no final da série (1995-97) era ainda superior àquelas existentes, em 1980-82, em algumas capitais de Estados da federação.
