O Hipotireoidismo Subclínico (HS) está associado a disfunções autonômicas cardíacas, comaumento da atividade simpática e diminuição da atividade parassimpática. Este desequilíbriono Sistema Nervoso Autônomo (SNA) pode gerar instabilidade elétrica cardíaca, sendo assim,fator de risco para eventos cardiovasculares fatais, como fibrilação e taquicardia ventricular.Dessa forma, o presente estudo propôs analisar o SNA de pacientes diagnosticados com HS,através da análise da Variabilidade da Frequência Cardíaca e Duração do Intervalo QT. Osindivíduos foram submetidos a avaliação eletrocardiográfica domiciliar por 24 horas, comregistros em 12 canais, através do gravador Holter DMS 300-12. A função autonômicacardíaca foi avaliada pelos parâmetros: SDNN (Standard Deviation of all normal NN interval),SDANN (Standart Deviation of the Average NN Interval), pNN50 (Percent of normal-normalNN intervals whose difference exceeds 50 ms) e a duração e duração do intervalo QT emmilissegundos. A pesquisa qualitativa incluiu 3 pacientes com o seguinte perfil clínico:feminino; média idades 53 anos; IMC médio 30,1; TSH médio 5,5; T4L médio 1. Constatou-se,que ⅔ dos indivíduos apresentaram aumento da duração do intervalo QT (554 e 534 ms) erestante dos parâmetros de inicial interesse normais, considerando os critérios de riscopré-estabelecidos. Contudo, observaram-se outros achados significativos na avaliaçãocardiológica dos pacientes como: bloqueio atrioventricular de 1º grau (PR até 0,24 seg)durante o sono, pausas sinusais com até 4.2 seg de duração, períodos deinfradesnivelamento de ST (-1,5MM) de V4 a V6, durante taquicardia sinusal e bloqueio deramo esquerdo intermitente com alteração de repolarização ventricular. A despeito dosachados expressivos, não se pode afirmar que estes estão correlacionados ao HS,devendo-se primeiramente excluir outras patologias prévias. Ademais, se faz necessário oacompanhamento cardiológico meticuloso desses indivíduos, devido aos riscos que podemadvir dessas condições. Infelizmente, devido a pandemia, a pesquisa encontrou dificuldadesna execução do projeto, principalmente devido ao fechamento prolongado da clínicaresponsável pelo exame cardiológico e pelo receio de contaminação dos pacientes eprofissionais, motivos que limitaram o desdobramento da pesquisa. Além disso, a maioriados portadores de HS são assintomáticos, o que prejudica o diagnóstico e recrutamento.Ainda são poucos os estudos que abordam esse tema, tanto na literatura nacional, quantointernacional. Portanto, é de interesse dos pesquisadores dar continuidade ao projetoobjetivando-se recrutar um número maior de participantes, a fim de evitar fatoresconfundidores, visto que alguns trabalhos internacionais já demonstraram disfunçãoautonômica nesses indivíduos, sendo necessário o acompanhamento da função cardíaca,considerando que as doenças cardiovasculares são a maior causa de mortalidade no mundo.
