A pancreatite aguda (PA) é uma patologia de causa multifatorial, caracterizada por um ataque inflamatório ao pâncreas de início súbito. A classificação de Atlanta a divide em forma leve, moderada e grave, sendo esta última o principal foco do presente estudo. A PA grave está associada a um alto índice de mortalidade, visto que leva à falência orgânica e complicações locais, como necrose. A fisiopatologia da PA consiste na ativação intracelular de zimogênios, liberação de citocinas inflamatórias e lesão vascular. Sintomas como dor súbita e intensa em epigástrio, náuseas e febre, são sugestivos e, se associado a marcadores laboratoriais e exames de imagem, podem confirmar o diagnóstico de PA. O tratamento clínico é fundamentado em medicamentos para supressão de sintomas, nutrição enteral, oxigenoterapia e reposição de fluidos. Entretanto, para coleções infectadas ou sintomáticas - após tratamento conservador duradouro - a intervenção cirúrgica é indicada. A necrosectomia aberta é a técnica tradicional, porém, abordagens menos invasivas têm sido aplicadas, como a laparoscópica, percutânea e endoscópica, apresentando menores taxas de complicações e mortalidade. As complicações pós-operatórias podem se dividir em precoces, como hemorragias e perfurações, e tardias, como hérnias de parede abdominal, distúrbio funcional do pâncreas e fístulas pancreato cutâneas, entéricas e colônicas. Sendo assim, as novas técnicas parecem produzir melhores resultados, entretanto, novos estudos ainda são necessários para determinar sua eficácia, custo-benefício e qualidade de vida.
O íleo biliar é uma rara obstrução mecânica do trato gastrointestinal decorrente da impactação de cálculos biliares, podendo ocorrer em qualquer parte deste trato. Sua ocorrência aumenta proporcionalmente à idade e, em decorrência do aumento da expectativa de vida da população, o íleo biliar apresentou crescimento na sua incidência. Manifestada tipicamente por dor em cólica, distensão abdominal, náuseas e vômitos, seus sintomas inespecíficos dificultam um diagnóstico rápido e preciso, resultando em altas taxas de morbimortalidade. A tomografia computadorizada é considerada, atualmente, o padrão ouro no diagnóstico do íleo biliar, entretanto, o diagnóstico também pode ser dado por radiografia, a partir da presença da tríade de Rigler (pneumobilia, distensão de alças intestinais e presença de cálculos radiopacos intraluminais). O principal objetivo terapêutico consiste na extração do cálculo agressor, após a estabilização clínica do paciente. Embora a cirurgia aberta tenha sido estabelecida como método padrão de tratamento, nos dias de hoje, o reparo por endoscopia tornou-se bem aceito, principalmente em pacientes mais frágeis. Entretanto, nos casos em que o método conservador falha ou em cenários de emergência, a cirurgia deve ser realizada prontamente. Esta, por sua vez, pode ser realizada por uma abordagem única, com a extração do cálculo biliar simultânea à colecistectomia e ao reparo da fístula, ou em dois estágios, ou seja, apenas a extração do cálculo biliar e posteriormente o reparo da fístula e a colecistectomia. De qualquer forma, a escolha da melhor conduta deve sempre ser individualizada, levando sempre em consideração a situação do paciente e a localização dos cálculos.
A úlcera péptica é definida pela ruptura da barreira defensiva da mucosa epitelial do estômago e duodeno, causada pelo desequilíbrio entre fatores de proteção e de destruição desta. Normalmente, são resultadas de infecções por Helicobacter pylori ou da ação de anti-inflamatórios não esteróides. Essa afecção manifesta-se, tipicamente, por epigastralgia, plenitude pós-prandial, distensão abdominal, náuseas, vômitos, melena e hematêmese. A úlcera péptica é diagnosticada através de seus sinais, sintomas e com a esofagogastroduodenoscopia. Uma vez diagnosticada, a úlcera pode ser tratada por diversas estratégias, a depender de sua etiologia e evolução, podendo variar de mudanças no hábito de vida, como alimentação e cessação do tabagismo e etilismo, a cirurgia, como a vagotomia e gastrectomia. No geral, o tratamento medicamentoso é suficiente para a cicatrização, sendo os inibidores da bomba de prótons os medicamentos de primeira linha. Porém, nos casos em que há infecção por H. pylori, o uso de antibióticos, como claritromicina e amoxicilina, também são indicados. Raramente a cirurgia é necessária, exceto em casos de complicações, como perfuração, suspeita de malignidade e obstrução gastrointestinal, ficando o manejo cirúrgico reservado a esses casos mais complexos.
A síndrome cardiorrenal (SCR) é definida como um distúrbio combinado do coração e dos rins, que interagem e produzem entre si uma disfunção de forma aguda ou crônica. Os sistemas cardiovascular e renal possuem uma relação bidirecional para manter as funções fisiológicas, de forma que a falência de um desses órgãos causa alterações patológicas no outro. A SCR é dividida em cinco subtipos, de acordo sua patologia primária e secundária. Os tipos 1 e 2 possuem a insuficiência cardíaca (IC) como causadora da insuficiência renal (IR), sendo que o tipo 1 possui curso agudo e o tipo 2 crônico. Já nos tipos 3 e 4, o cenário se inverte, ou seja, a IR causa a IC, podendo ser agudo (tipo 3) ou crônico (tipo 4). Por fim, no tipo 5, uma condição sistêmica que causa as disfunções cardíacas e renais. A fisiopatologia da SCR é multifacetada e envolve mecanismos hemodinâmicos, como aumento da pressão venosa central, neuro-hormonais, como hiperativação do sistema renina-angiotensina-aldosterona, inflamatórios, como liberação de endotelina-1 e consequente fibrose, entre outros. Os pacientes com SCR apresentam sinais e sintomas típicos da IC, como dispneia, edema, ascite, estertores crepitantes, que variam de acordo com o tipo da congestão, ou seja, pulmonar, sistêmica ou ambas. Atualmente, o diagnóstico clínico é insuficiente e requer biomarcadores, como peptídeos natriuréticos, galectina-3 e cofilina-1 urinária, e métodos de imagem, como ultrassonografia, tomografia computadorizada e ressonância magnética. O tratamento da SCR tem como objetivo a redução da congestão, mantendo boa perfusão e controle das patologias cardíaca e renais. Inicialmente, são preferidos os diuréticos de alça, podendo ser associados a antagonistas de vasopressina, vasodilatadores diretos, fármacos inotrópicos, entre outros. Em último recurso, para os pacientes refratários aos diuréticos, pode ser utilizada a ultrafiltração. Entretanto, apesar dos resultados clínicos terem melhorado nas últimas décadas, os pacientes com disfunção simultânea dos rins e coração ainda possuem um prognóstico ruim, apresentando altas taxas de hospitalização e mortalidade.
O presente artigo apresenta uma revisão narrativa de literatura, cuja finalidade é abordar o mecanismo fisiopatológico da sepse secundária a COVID-19. A sepse raramente era atribuída a agentes etiológicos virais, porém, com o surgimento do SARS-CoV-2, criou-se um novo debate, exigindo a consideração desse vírus como causador de tal quadro. Apesar da maioria dos pacientes infectados por esse vírus apresentarem-se assintomáticos ou com sintomas leves, uma pequena parte pode evoluir para quadros mais graves, principalmente aqueles que apresentam comorbidades associadas. Embora a patogênese da COVID-19 não esteja completamente esclarecida, percebeu-se que a hiperativação do sistema imune e a produção excessiva de citocinas, revelada através de seus níveis séricos, se assemelhavam aos encontrados na sepse. Além disso, acredita-se que a disfunção microvascular, ocasionada pela hipóxia e distúrbios circulatórios pode causar a falência de múltiplos órgãos. Por fim, o tratamento, preconizado pela Surviving Sepsis Campaign (SSC), se baseia na ventilação, hemodinâmica e terapia, sendo, este último, composto por corticosteróides, imunoterapia, anticoagulantes e antimicrobianos.
