La Neumonía adquirida en la comunidad (NAC), es una de las principales patologías infecciosas a nivel respiratorio, con una amplia distribución y morbimortalidad a nivel mundial. Se caracteriza por un compromiso del parénquima pulmonar, causado por agentes externos al ámbito hospitalario, como bacterias, virus u hongos. La incidencia promedio es de 9.2 a 33 por cada mil habitantes, con mayor afectación en la población mayor a 65 años. Se estima que, al menos, 10% a 20% de casos, requerirán manejo en una unidad de cuidados intensivos, de acuerdo con la estratificación de riesgo individual. El diagnóstico es clínico y se confirma con estudios de imagen, análisis bioquímicos y microbiológicos. El tratamiento inicial se instaura empíricamente, de acuerdo con el historial clínico, uso previo de antibiótico y al patrón de sensibilidad del país o del centro.
La varicela es una enfermedad causada por el virus de varicela zóster y es altamente contagiosa. Se manifiesta por medio de un exantema pruriginoso maculopapular, con vesículas y costras. Se presenta principalmente en la infancia y brinda inmunidad para toda la vida. La varicela se presenta mundialmente y tiene un curso autolimitado y benigno en la mayoría de casos, sin embargo si se presenta una primoinfección en el embarazo hay riesgo de a neumonitis materna, síndrome de varicela congénito, varicela neonatal, las cuales presenta un índice de mortalidad del 30%. En la actualidad se cuenta con medidas preventivas, como es la vacuna contra varicela para mujeres en edad fértil, y en casos de mujeres gestantes y neonatos, se cuenta con la inmunoglobulina contra varicela zóster, además del tratamiento con antivirales como aciclovir y valaciclovir.
El traumatismo en general, corresponde a la tercera causa muerte en pacientes jóvenes y adultos menores de 40 años. El trauma torácico (TT), es considerado responsable de 20-35% de estos fallecimientos. La mayoría de las lesiones traumáticas a nivel del tórax son provocadas por colisiones automovilísticas, seguida por las heridas penetrantes. Clínicamente el TT, se clasifica en lesiones contusas o cerradas y penetrantes o abiertas. Dentro de las múltiples lesiones, se encuentran las fracturas de costales, fracturas de clavícula, esternón, escápula, tórax inestable, contusiones cardiopulmonares, neumotórax, hemotórax, lesiones vasculares, daño a órganos digestivos superiores, entre otras. El abordaje inicial, debe basarse estrictamente en los lineamientos de atención de pacientes poli traumatizados. Su tratamiento definitivo se dirige a restaurar el funcionamiento fisiológico de la cavidad torácica; menos del 10% de los traumatismos cerrados y sólo 15-30% de los penetrantes, requerirán procedimientos quirúrgicos avanzados.
El síndrome de HELLP es una afección del embarazo, el cual presenta la triada de hemólisis, elevación de enzimas hepáticas y trombocitopenia. Este síndrome se caracteriza por presentar una afectación multisistémica, además de presentar una alta morbimortalidad materno-fetal. Se estima que afecta entre 0,1 a 0,9% de los embarazos, entre las 27-37 semanas de gestación principalmente; y 10-20% también presentan preeclampsia. Estas pacientes refieren típicamente dolor en epigastrio o cuadrante superior derecho del abdomen, náuseas y/o vómitos. El diagnóstico se realiza por medio de la presencia de la tríada: anemia microangiopática, elevación de enzimas hepáticas y trombocitopenia. Se debe brindar manejo hospitalario y multidisciplinario debido a las posibles complicaciones de la madre y el feto. El tratamiento es sintomático y la realización del parto entre 24-48 horas posterior al diagnóstico.
El Síndrome de Stevens-Johnson y la Necrólisis Epidérmica Tóxica son enfermedades mucocutáneas severas con una alta morbi-mortalidad que forman parte de un espectro de gravedad y se distinguen entre sí por la extensión de piel afectada. Los medicamentos son el principal factor desencadenate dentro de las primeras semanas de tratamiento. Los agentes comúnmente implicados pueden variar de acuerdo con la población y sus diferencias genéticas. Los mecanismos fisiopatológicos implicados no están del todo esclarecidos, sin embargo se ha demostrado la participación de la granulisina, la vía de señalización del Fas y citocinas inflamatorias. Los pacientes con el síndrome de Stevens-Johnson o con Necrólisis Epidérmica Tóxica tienen alto riesgo de desarrollar infecciones bacterianas que podrían conllevar a sepsis y otras complicaciones. El diagnóstico requiere una correlación clínico-histopatológica y su manejo se basa principalmente en la suspensión del agente causal y la terapia de soporte temprana agresiva. Existe evidencia que sugiere beneficios con el uso de diversas terapias sistémicas inmunomoduladoras, sin embargo se requiere la evaluación de estudios prospectivos adicionales.
