Zusammenfassung Hintergrund Studien zufolge müssen Pflegepersonen im Krankenhaus u. a. aufgrund von Ressourcenknappheit notwendige Pflegetätigkeiten weglassen bzw. können diese nur mit einer für die Patient*innen nachteiligen Verzögerung durchführen. Dieses Phänomen wird auch als Missed Nursing Care (MNC) beschrieben (Kalisch et al. 2009). In Österreich fehlt es an einer validen Datengrundlage dazu. Ziele Missed Nursing Care in österreichischen Krankenhäusern zu erheben und Einflussfaktoren darauf zu identifizieren. Unterschiede bzw. Zusammenhänge in MNC nach Krankenhaus- und Stationscharakteristika sowie nach soziodemografischen und beruflichen Merkmalen von Pflegepersonen zu untersuchen. Methode Deskriptiv-komparative quantitative Querschnittstudie mittels onlinebasierter Fragebogenerhebung anhand des Revised-MISSCARE-Austria-Instruments. Zielgruppe waren alle Pflegepersonen, die in diesem Zeitraum auf Allgemeinstationen in österreichischen Krankenhäusern tätig waren. Die Datenanalyse erfolgte mittels deskriptiver Statistik, Korrelationskoeffizienten und nichtparametrischen Gruppenvergleichstests. Ergebnisse 1006 Pflegepersonen haben an der Studie teilgenommen und die Einschlusskriterien erfüllt. 84,4 % (n = 849) davon gaben an, dass in ihren Teams in den letzten 2 Wochen mindestens eine der erfassten Tätigkeiten in der akuten Patient*innenversorgung weggelassen wurde. Dies betrifft vornehmlich Interventionen, bezogen auf emotionale Unterstützung, auf Beratung und Schulung sowie auf Gesprächsführung. Vor allem jüngere Pflegepersonen (p < 0,001), Pflegende auf Stationen mit mehr Betten (p < 0,001), Personen mit einer Vollzeitanstellung (p = 0,036), mit weniger Berufserfahrung (p = < 0,001) und diplomierte Gesundheits- und Krankenpflegepersonen in der direkten Patient*innenversorgung (p = 0,003) berichten über eine höhere Häufigkeit von MNC. Die häufigsten Einflussfaktoren auf MNC sind Multitasking, häufige Unterbrechungen, Personalmangel, gesteigerter Versorgungsaufwand, gesteigerte Aufnahme- und Entlassungsdynamiken sowie die eigene Erschöpfung und mangelnde Unterstützung von den direkten Vorgesetzten. Schlussfolgerung MNC wurde erstmalig in österreichischen Krankenhäusern erfasst. Die Ergebnisse geben deutliche Hinweise auf implizite Rationierung der akuten Versorgung. Die identifizierten Einflussfaktoren sollten bei der zukünftigen Entwicklung von Strategien für die Sicherung der Versorgungsqualität im akuten Bereich berücksichtigt werden.
Zusammenfassung. Hintergrund: Fragebogenübersetzungen in der deutschsprachigen Pflegewissenschaft setzen trotz zunehmender Kritik auf Methoden mit Erst- und Rückübersetzung. Dem entgegen wird die TRAPD-Methode als best practice anerkannt. Erfahrungswerte mit der Anwendung dieser Methode in der deutschsprachigen Pflegewissenschaft fehlen jedoch. Ziel: Die Anwendung der TRAPD-Methode am Bespiel der Übersetzung vom revised MISSCARE Survey vom Englischen ins Deutsche darzustellen sowie notwendige Adaptierungen, Vorteile und Limitationen der Methode zu diskutieren. Methode: Eine adaptierte Version der teambasierten Übersetzungsmethode TRAPD durch die Schritte Vorbereitung, Übersetzung, Review, Adjudikation, Prätest und Dokumentation wurde umgesetzt, basierend auf den GESIS Guidelines für Fragebogengestaltung und Übersetzung. Ergebnisse: Das revised MISSCARE Austria Instrument besteht aus 85 Items. Für die Mehrzahl der Items wurden äquivalente Termini gefunden, die eine eindeutige Übersetzung ermöglichten. Bei einzelnen Items wurde eine Adaptierung aus kulturellen, Messung- und Konstrukt-bezogenen Gründen notwendig. Die Äquivalenz der Übersetzung wurde bei komplexen Items mit der Entwicklerin geprüft und durch mehrmalige kognitive Prätests mit Pflegepersonen gefördert. Schlussfolgerungen: Die Studie bestärkt die Aussage, dass die TRAPD-Methode für die Übersetzung von Messinstrumenten ebenfalls in der deutschsprachigen Pflegewissenschaft geeignet ist. Sie zeigt aber auch, dass weitere Erfahrungen für eine Weiterentwicklung von methodischen Spezifika für unsere Disziplin erforderlich sind.
Zusammenfassung Hintergrund Eine sichere und qualitativ angemessene Versorgung im Krankenhaus erfordert den Einsatz einer adäquaten, den Bedarfen von Patient*innen und ihrer Angehörigen entsprechenden Anzahl an qualifizierten Pflegepersonen. Für Allgemeinstationen in österreichischen Krankenhäusern gibt es hierzu keine explizite Orientierung. Die Datenlage zum Pflegepersonaleinsatz auf Allgemeinstationen und dessen Angemessenheit ist hierzulande limitiert. Diese beiden Faktoren dienen als Prädiktoren für das Weglassen pflegerischer Versorgung – Missed Nursing Care (MNC). Ziele Eine erste Datenlage zum Einsatz von Pflegepersonalressourcen auf Allgemeinstationen in österreichischen Krankenhäusern mittels Patient-to-Nurse Ratio (PtN) vorzustellen; die Einschätzung der Angemessenheit der Pflegepersonalbesetzung aus Sicht von Pflegepersonen zu erheben und den Einfluss beider Variablen auf MNC zu diskutieren. Methode Es wurde eine quantitative Querschnittstudie mit dem Revised-MISSCARE-Austria-Instrument zwischen Mai und Juli 2021 durchgeführt. Zielgruppe waren Pflegepersonen, die in diesem Zeitraum auf Allgemeinstationen in österreichischen Krankenhäusern in der direkten Patient*innenversorgung tätig waren. Die Datenanalyse erfolgte mittels deskriptiver Statistik sowie Korrelationskoeffizienten, nichtparametrischen Gruppenvergleichstests und univariater multifaktoriellen Varianzanalyse. Ergebnisse In der Datenanalyse wurden 846 Fälle berücksichtigt. PtN-Werte auf konservativen und/oder operativen Allgemeinstationen in österreichischen Krankenhäusern situieren sich im internationalen Vergleich in einem hohen Bereich: Durchschnittlich werden ca. 15 Patient*innen/Pflegeperson am Tag- bzw. 22 Patient*innen/Pflegeperson im Nachtdienst versorgt. Unterschiede nach Krankenhauslokalisation und -größe sowie nach Berufsgruppe in der Pflege zeigen sich statistisch signifikant. 67,8% der Befragten geben an, dass die Pflegepersonalbesetzung in den letzten 3 Monaten selten oder nie angemessen war. Beide Variablen zeigen einen statistisch signifikanten Einfluss auf MNC. Schlussfolgerung Entsprechend dieser Ergebnisse zeigt sich eindeutig die Notwendigkeit der Einführung eines Monitorings zur aktuellen Pflegepersonalsituation auf Allgemeinstationen in österreichischen Krankenhäusern. Hier erscheint nicht nur die PtN, sondern auch die Einschätzung der Angemessenheit der Pflegepersonalbesetzung aus Sicht von Pflegepersonen für die Beurteilung der Pflegepersonalsituation relevant.
