Background: Patient undergoing surgery may be afraid and concerned about the diagnosis, the treatment, the procedure, the postoperative care, and the surgical recovery. Good communication between staff and patients can minimize or prevent this situation. This study aimed to evaluate the effectiveness of a Telecare nursing intervention, "Telephone consultation", in reducing the "Delayed surgical recovery" nursing diagnosis in patients undergoing laparoscopic cholecystectomy and hernia repair. Methods: This study was performed in two different institutions located in Rio de Janeiro, Brazil. A total of 43 patients were enrolled. The experimental group consisted of 22 patients who had access to the telephone followup intervention, and the control group consisted of 21 patients who received conventional treatment without telephone follow-up. This was a randomized controlled study with patients who were 60 years or older and awaiting operative procedures of hernia repair and laparoscopic cholecystectomy who had a mobile or landline phone and were available for telephone contact. Results: There was a reduction in "loss of appetite with nausea" (p = 0.013); "need help to complete self-care" (p = 0.041); "pain" (p = 0.041); and "postoperative sensation" (p = 0.023). The experimental group showed a significantly larger decrease in factors related to the "Delayed surgical recovery" diagnosis, suggesting a positive effect of the intervention compared to the effect in control group.\. Conclusion: Telephone consultation identified factors that increased the risk of complications after surgery, recognized potential patients for delayed surgical recovery and helped perioperative nurses provide accurate interventions to prevent or mitigate delayed recovery. Analisar a literatura sobre o uso do telecuidado como Intervenção de Enfermagem na assistência ao idoso com Alzheimer e seus cuidadores. Trata-se de revisão sistemática da literatura nas bases LILACS, PubMed, CINAHL e Elsevier, baseada na proposta PRISMA. Os critérios de inclusão foram: artigos que abordassem diretamente ações de telecuidado por enfermeiros; estudos de coorte e ensaios clínicos randomizados com sigilo de alocação; artigos indexados; publicados nos idiomas português, inglês e espanhol; recorte temporal de 2007 a 2017. Foram encontrados dois artigos sobre telecuidado como Intervenção de Enfermagem no suporte e apoio aos cuidadores familiares de idosos com demência. O uso do acompanhamento por telefone foi realizado associado a outra tecnologia assistida. O acompanhamento por telefone foi associado ao uso de câmeras, e os próprios cuidadores realizavam as filmagens da rotina; ainda foram realizadas teleconferências em chamadas de vídeo entre cuidadores e profissionais. O telecuidado pode ser considerado uma prática avançada da enfermeira e alternativa acessível na assistência aos idosos com Alzheimer e seus cuidadores.
Introdução: A tecnologia pode se apresentar como uma aliada dos profissionais da saúde para possibilitar a continuidade do cuidado em domicílio. A utilização dela se justifica pelo aumento da complexidade do estado de saúde dos indivíduos que cada vez mais possuem doenças crônicas e agudas, a crescente expectativa de vida, a dificuldade de deslocamento nas grandes cidades e nas áreas rurais e, sobretudo, ao aumento da adesão as diversas redes sociais que possibilitam a facilidade e rapidez na comunicação. Objetivo: discutir estratégias do uso de tecnologias para a continuidade de cuidados à distância aos idosos. Métodos: foram apresentados três estudos de intervenção sobre a aplicabilidade de tecnologias para continuidade de cuidados à distância. Resultados: em pacientes submetidos a cirurgia de catarata, o telecuidado influenciou na diminuição do tempo de recuperação cirúrgica, na menor ocorrência de complicações e as intervenções mais realizadas foram referentes ao desconforto, proteção dos olhos, curativo e autocuidado. Nospacientes submetidos a cirurgia cardíaca, observa-se que o discurso demonstra retorno ao hospital para acompanhamento dos exames de sangue, preocupação com a cicatriz cirúrgica, bom estado geral, apesar das dores, e satisfação por receber o contato via Whatsapp®. Sobre idosos com demencia e seus cuidadores, a ligação telefônica reduziu o risco para declínio funcional nos idosos e a sobrecarga dos cuidadores de idosos. Conclusão: o telecuidado foi testado e demonstrou resultados positivos em três cenários, mas destaca-se a necessidade de ampliar essa tecnologia em mais modalidades com outras tecnologias integradas que possam melhorar a qualidade de vida e autonomia dos idosos e familiares.
Teleconsultation can be classified as an advanced practice nursing that requires nurses’ clinical reasoning based on a consistent theoretical framework to use in the nursing process. Our study conducted a theoretical-reflective analysis, based on Callista Roy’s Adaptation Model of Nursing and Chick-Meleis’ Transition Theory, about the contribution of teleconsultation as an advanced practice nursing in the care of older adults with chronic diseases during the COVID-19 pandemic. We reflect on this in two moments: “nursing theories and dealing with COVID-19” and “ways of adapting to new care models and advanced practice nursing,” based on communication and information technologies. The worsening of the pandemic in Brazil changed life cycles, health/disease and organizational processes, demanding the development of an adaptive-transactional state by users and health care providers. Thus, information and communication technologies combined with advanced practice nursing can relieve social distancing and its repercussions on health care.
RESUMO A teleconsulta pode ser definida como uma prática avançada de enfermagem que requer do enfermeiro raciocínio clínico fundamentado em um arcabouço teórico consistente para aplicação no processo de enfermagem. No presente estudo, realizou-se uma análise teórico-reflexiva, fundamentada no Modelo de Adaptação de Callista Roy e na Teoria de Transição de Chick-Meleis, sobre a contribuição da teleconsulta como prática avançada de enfermagem no atendimento a pacientes idosos e com doenças crônicas no contexto da pandemia de covid-19. A reflexão é apresentada em dois momentos: “teorias de enfermagem e o enfrentamento da covid-19” e “formas de adaptação a novos modelos de assistência e as práticas avançadas em enfermagem”, norteados pelas tecnologias de comunicação e informação. O agravamento da pandemia no Brasil trouxe mudanças relacionadas aos ciclos de vida, aos processos saúde/doença e organizacionais, demandando o desenvolvimento de um estado adaptativo-transacional por parte dos usuários e profissionais de saúde. Neste contexto, as tecnologias da informação e comunicação aliadas à prática avançada de enfermagem representam papel fundamental para atenuar o distanciamento social e suas repercussões na assistência à saúde.
