Abstract Objective: The postoperative pain after heart surgeries had been often reported. Meager reports about respiratory function and pain correlation had been reported. The aim of this study is to assess the pain intensity and location during hospital stay and its effect on respiratory function in patients undergone elective heart surgery.Methods: Respiratory function (lung volumes, respiratory muscle strength and peak expiratory flow) was assessed at the preoperative and postoperative times (1, 3 and 5 days) by ventilometer, manovacuometer and peak flow meter measurements. The assessment of pain intensity was performed with a visual analogue scale for pain.Results: The majority of pain site was on sternotomy incision (50% of patients) and the intensity was higher at the first postoperative day (8.32 by visual scale measurement). All respiratory variables remained lower than to preoperative period at fifth postoperative time (P > 0.05), with exception for respiratory rate. The pain and maximal inspiratory pressure showed a negative correlation at the first postoperative day (P = 0.019).Conclusion: Postoperative pain decreased respiratory function in patients precluding deep inspirations, in special, at the first postoperative day. Objective: To evaluate the diagnostic performance of the rapid shallow breathing index (RSBI) in predicting extubation failure among adult patients in the intensive care unit and to determine the appropriateness of the classical RSBI cut-off value. Methods: This was a prospective study conducted in the adult intensive care unit of the Botucatu School of Medicine Hospital das Clínicas. The RSBI was evaluated in 73 consecutive patients considered clinically ready for extubation. Results: The classical RSBI cut-off value (105 breaths/min/L) presented a sensitivity of 20% and a specificity of 95% (sum = 115%). Analysis of the receiver operator characteristic (ROC) curve revealed a better cut-off value (76.5 breaths/min/L), which presented a sensitivity of 66% and a specificity of 74% (sum = 140%). The area under the ROC curve for the RSBI was 0.78. Conclusions: The classical RSBI cut-off value proved inappropriate, predicting only 20% of the cases of extubation failure in our sample. The new cut-off value provided substantial improvement in sensitivity, with an acceptable loss of specificity. ResumoObjetivo: Avaliar o desempenho diagnóstico do índice de respiração rápida e superficial (IRRS) na predição do insucesso da extubação de pacientes adultos em terapia intensiva e verificar a adequação do valor de corte clássico para esse índice. Métodos: Estudo prospectivo realizado na unidade de terapia intensiva de adultos do Hospital das Clínicas da Faculdade de Medicina de Botucatu, através da avaliação do IRRS em 73 pacientes consecutivos considerados clinicamente prontos para extubação. Resultados: O IRRS com valor de corte clássico (105 ciclos/min/L) apresentou sensibilidade de 20% e especificidade de 95% (soma = 115%). A análise da curva receiver operator characteristic (ROC) demonstrou melhor valor de corte (76,5 ciclos/min/L), o qual forneceu sensibilidade de 66% e especificidade de 74% (soma = 140%), e a área sob a curva ROC para o IRRS foi de 0,78. Conclusões: O valor de corte clássico do IRRS se mostrou inadequado nesta casuística, prevendo apenas 20% dos pacientes com falha na extubação. A obtenção do novo valor de corte permitiu um acréscimo substancial de sensibilidade, com aceitável redução da especificidade. O valor da área sob a curva ROC indicou satisfatório poder discriminativo do índice, justificando a validação de sua aplicação.
Objetivo. Verificar os efeitos de um protocolo fisioterapêutico composto por técnicas manuais de mobilização torácica, na função respiratória de pacientes com paralisia cerebral (PC). Método. Foi realizado um estudo piloto com 05 pacientes com PC quadriplégica com idade média de 8,6 ± 3,5 anos. A função respiratória foi avaliada com um ventilômetro de Wright (FerrarisR MK8) adaptado a uma máscara facial para verificação do volume minuto (VM), frequência respiratória (FR) e volume corrente (VC). O padrão respiratório foi avaliado através da inspeção do paciente. As avaliações foram feitas pelo mesmo avaliador antes e após 10 sessões de fisioterapia utilizando técnicas manuais de mobilização torácica. Foi realizada análise estatística das variáveis numéricas entre os períodos pré e póstratamento através do teste de Wilcoxon. Resultados. Após o tratamento, houve aumento na FR de 22,9%, aumento significativo de 23,13% no VM (p=0,043) e redução de 14,62% no VC. Em todos os pacientes, o padrão respiratório inicial foi costal e alteraram para padrão respiratório misto após o tratamento. Conclusão. os pacientes estudados obtiveram aumento significativo no VM e alteração do padrão respiratório.
Introdução: A pleurotomia durante a cirurgia cardíaca pode danificar a função respiratória durante o período pós-operatório, podendo interferir no tempo de internação hospitalar. Objetivo: Comparar os dias de internação hospitalar em pacientes submetidos à cirurgia de revascularização do miocárdio com e sem realização de pleurotomia. Métodos: A pesquisa foi composta por 20 pacientes submetidos à cirurgia de revascularização do miocárdio com circulação extracorpórea, sendo divididos em dois grupos: G1 – pacientes que realizaram pleurotomia (n=10) e G2 – pacientes que não realizaram pleurotomia (n=10). Resultados: Os resultados mostraram que os pacientes do grupo G1 quando comparados aos pacientes do grupo G2 obtiveram média de internação semelhante (7,2±0,78 versus 6,8±0,42 dias). Conclusão: Não houve diferença significativa no tempo de internação entre os grupos de pacientes com e sem pleurotomia.
Testes de função pulmonar são usados para determinar a gravidade, as conseqüências funcionais e o progresso de disfunções pulmonares e neuromusculares. As variações metodológicas com o bocal e a máscara facial e a cooperação do paciente podem interferir no desempenho das manobras e afetar as medidas. O objetivo deste estudo foi comparar os valores de volumes pulmonares (volume corrente [Vc], volume-minuto [Vm]), freqüência respiratória (FR) e capacidade vital (CV) em indivíduos saudáveis quando avaliados por meio do bocal e da máscara facial. Participaram do estudo 60 voluntários saudáveis, 14 homens e 46 mulheres, com média de idade de 22,9±7,1 anos. Para a avaliação com bocal, utilizou-se uma peça semirrígida descartável e clipe nasal; a máscara usada foi facial plástica com borda pneumática inflável. As medidas obtidas foram comparadas estatisticamente (nível de significância 5%). Os valores obtidos com o bocal e com a máscara facial foram, respectivamente: FR, 15 rpm x 13 rpm (p<0,01); Vm, 14,87 l/min x 10,02 l/min (p<0,01); Vc, 0,93 l x 0,8 l (p<0,01); CV, 3,85 l x 3,52 l (p<0,01). Os valores de volumes pulmonares e capacidade vital mostraram-se pois significativamente inferiores quando avaliados com a máscara facial em comparação ao bocal.
Purpose: This study aimed at assessment of respiratory muscle performance (RMP), functional capacity (FC), feeling of fatigue, and quality of life (QOL) in liver transplant candidates and to correlate these variables with each other and with the Model for End-Stage Liver Disease (MELD). Methods: A prospective study carried out from January/2008 to June/2009 including 130 liver transplant candidates on waiting–list at the São Paulo Health Transplant System, followed at the Unit of Liver Transplantation - Unicamp with cirrhosis of any etiology, independent of gender, age, and degree of liver function impairment as classified by the MELD score. Respiratory muscle performance was determined by respiratory muscle strength (RMS - PIMAX and PEMAX), vital capacity (VC), and FC measured during six-minute walk tests (6MWT). Fatigue was evaluated using the Fatigue Severity Scale (FSS), and QOL was assessed using the SF-36 questionnaire. Results: Expressed as the mean SD and percentage of the predicted value: MELD=16+4, VC=4.0+0.9L - (97%+16%), PIMAX=77+28cmH2O -(71+25%), PEMAX=76+26cmH2O (76+26%), 6MWT=453.9+81.2m (73+13%), FSS=34+17, and compromised QOL, particularly in physical (48+41) and emotional (57+41) aspects, vitality (58+26) and general state (58+24). Predicted VC was negatively correlated with MELD (p=0.0034) and positively correlated with RMS and 6MWT (p=0.0217). Greater distances in the 6MWT were associated with a higher RMS and QOL and less fatigue. Greater FSS scores resulted in lower QOL scores. Conclusion: A decrease in VC was related to the severity of liver disease and a decrease in RMS and FC, which, in turn, correlated with greater fatigue and lower QOL.
