O envelhecimento populacional, o aumento da morbi-mortalidade secundária as doenças crônicas não-transmissíveis e a incorporação de novas tecnologias à medicina vêm desafiando os sistemas de saúde. Modelos de case-mix têm sido desenvolvidos com a finalidade de propiciar uma metodologia de alocação de recursos capaz de também mensurar e avaliar o desempenho de prestadores de saúde. Numerosas alternativas de classificações de pacientes para mensurar o case-mix têm sido propostas, seja no contexto hospitalar, ambulatorial, de unidades especializadas, dentre outros. No nosso meio, essas metodologias ainda são escassas ou pouco avaliadas. Este estudo apresenta uma escala clínico-funcional que delineia um sistema de case-mix de casos complexos, baseando-se na experiência de um grande serviço de atenção domiciliar em uma operadora de plano de saúde do tipo cooperativa médica em Belo Horizonte, Minas Gerais, Brasil. Trata-se de um instrumento, simples, acessível a múltiplas especialidades de profissionais de saúde e passível de aplicação em várias modalidades de assistência a idosos e portadores de doenças que levam à incapacidade funcional.
