ZusammenfassungHintergrund: Die flächendeckende Gesundheitsversorgung Deutschlands ist in Gefahr, weil sich nicht mehr genügend Ärzte finden, die bereit sind, unter den gegenwärtigen Arbeitsbedingungen die Primärversorgung vor allem in ländlichen Gegenden sicherzustellen. Diese Situation wird sich verschärfen, da einerseits die Anzahl an Studienabgängern im Fach Medizin sinkt und andererseits zunehmend weniger der Absolventen bereit sind, in der primärärztlichen Versorgung tätig zu werden. In Anbetracht des wachsenden Nachwuchsproblems ist es von großem Interesse, welche Motive heute junge Menschen zum Medizinstudium bewegen und welche Berufsziele oder Hoffnungen sie mit der Wahl des Faches verbinden. Methodik: Alle Studienanfänger des Studienfachs Medizin (n = 344) im Wintersemester 2004/2005 wurden zu Beginn des Semesters mittels Fragebogen zu ihren Motiven, Zielen und Hoffnungen befragt. Ergebnisse: 94,8 % aller Erstsemester beteiligten sich. Als wichtigste Motive und Wünsche der Studenten zeigten sich das Arbeiten mit Menschen, die Hilfeleistung für andere und die Verwirklichung der eigenen Ideale. Nur ein geringer Prozentsatz der Teilnehmer plant, in der Allgemeinmedizin tätig zu werden, allerdings sind 48,2 % hinsichtlich ihrer Fachwahl noch unentschlossen. Schlussfolgerungen: Die Zahlen stehen im Einklang mit dem Trend zur zunehmenden Spezialisierung, der international zu Lücken in der Primärversorgung führt. This situation will get worse in the future because the number of graduates from medical schools is on the decline and in addition a decreasing number of graduates is willing to work in primary care. Taking into account the problem of an insufficient number of young physicians to replace the retiring ones it is of great interest which motives, goals, and hopes inspire young people to study medicine. Methods: We inquired all first year preclinical medical school students by questionnaire about their motives, goals, and hopes at the beginning of their first year (2004/2005). Results: 94.8 % of all first-year students responded. ZusammenfassungHintergrund: Fallbasierte Lehre ist eine effektive Methode, medizinisches Wissen und Fähigkeiten zu vermitteln. Virtuelle Patienten (VPs) können dabei ein wertvolles Instrument sein. Ziel dieses Übersichtsartikels ist es, die vorhandenen deutschsprachigen Patientensimulationsprogramme zu identifizieren, auf bereits vorhandene allgemeinmedizinische Inhalte zu überprü-fen und Anwendungsmöglichkeiten von VPs in der allgemeinmedizinischen Hochschullehre zu beschreiben. Methoden: Zur Identifizierung von Patientensimulationsprogrammen wurde eine Recherche der Literatur-Datenbanken Medline, EMBASE und ERIC durchgeführt, sowie der E-Learning-Datenbanken KELDAmed, LRSMed und Caseport. Zwischenergebnisse einer laufenden Telefonbefragung der allgemeinmedizinischen Vertreter deutscher Universitäten wurden zur Kontrolle ebenfalls mit aufgenommen. Informationen über Nutzungsmöglichkeiten der Programme wurden auf den entsprechenden Webseiten und durch telefonische Kontaktaufnahme geklärt. Ergebnisse: Es fanden sich sechs universitäre und zwei kommerzielle deutschsprachige Programme mit unterschiedlichen didaktischen Konzepten. Informationen über vorhandene allgemeinmedizinische Inhalte waren lediglich über die Telefonbefragung erhältlich. Schlussfolgerungen: Die Zahl deutschsprachiger Patientensimulationsprogramme ist überschaubar, die Auswahl des geeigneten Programms hängt jedoch von standortspezifischen Voraussetzungen ab. Bestrebungen zur Abstract Background: Case-based teaching is an effective method to train medical knowledge and skills. Virtual patients (VPs) can also facilitate this experience. The aim of this article is to identify existing patient simulation programmes in German language, to check for existing primary care specific content and to outline possibilities to integrate VPs in (undergraduate) medical education. Methods: To identify patient simulation software a search was conducted in the digital libraries and databases Medline, EMBASE und ERIC, and in the e-learning databases KELDAmed, LRSMed and Caseport. Interim results of a telephone survey of e-learning activities in primary care teaching at German universities were also included as a control measure. Information about accessibility and utilization of suitable programmes for other university departments was gained by the respective websites and telephone contact. Results: Six university and two commercial programmes were identified, displaying different didactic concepts. Content specific for primary care was only identified via the telephone survey. Conclusion: There is only a limited number of German patient simulation programmes, but a decision towards the most suitable programme depends highly on local circumstances.
