Dans le contexte culturel malien, notamment au sujet du désir d’enfants, de la grossesse et de la place de l’enfant à venir, il est important de prendre en compte la question de la polygamie. Enfanter peut en effet positionner une femme à la place de favorite. À travers l’illustration clinique de Mme M., nous verrons de quelle manière l’enfant, avant même sa naissance, peut être investi comme objet de vengeance envers l’autre parent. Nous analyserons les représentations du fœtus et la place de l’invisible dans un contexte culturel donné et dans l’histoire personnelle de cette femme. Nous essaierons enfin, à travers une analyse transculturelle, de le mettre en lien avec le « syndrome de Médée ».
Au Burundi et plus généralement dans les pays d’Afrique, il peut arriver que père et fils n’aient pas le même nom. Le nom est choisi par le devin-guérisseur avant la naissance, en fonction de plusieurs facteurs, tels que la famine, la guerre, les circonstances de la conception et bien d’autres. A travers une situation clinique, nous essaierons de comprendre quel impact ces nominations peuvent avoir sur l’avenir de l’individu sans pour autant considérer que ce déterminisme, pour important qu’il soit, ait un caractère absolu.

L’objet transitionnel est défini par Winnicott comme un objet ayant une fonction de défense contre l’angoisse. Nous proposons une réflexion transculturelle sur sa place et sa forme en Afrique de l’Ouest en nous appuyant sur les concepts « d’objets et de phénomènes transitionnels » de Winnicott. Nous avons cherché dans nos souvenirs d’enfance maliens des équivalents d’objets pouvant s’apparenter, pour le jeune enfant, au modèle occidental. Nous retrouvons, au moment du sevrage et des premières expériences de séparation, des objets et des figures d’attachement à valeur transitionnelle dans le contexte malien. L’usage et la fonction de ces processus semblent dépendre de l’investissement parental, et doit être lu à la lumière du caractère groupal de l’appartenance au Mali et des mouvements des enfants dans un modèle de parenté multiple. À notre génération, le processus transitionnel semble être présent en Afrique de l’Ouest, ce peut être un objet, une personne ou un symbole. Ce processus doit être replacé dans son contexte et prend des formes plus variées que dans le monde occidental.
Le contexte culturel confère à la maladie mentale des caractéristiques qui sont susceptibles de modifier la symptomatologie dans toute société et culture. Ici, il s’agit d’un patient originaire du Mali convaincu d ’être atteint de la lèpre. L’approche culturelle semble être le précurseur de l’instauration de l’alliance thérapeutique d’une part, et d’autre part permet d’accompagner de manière adaptée le patient jusqu’à la critique de sa symptomatologie.
