Introducción: Se define fistula anal como la comunicación anormal entre el conducto anorrectal y la piel y es un proceso infeccioso crónico que en su fase aguda forma un absceso anal. La mayoría de las fistulas anales tienen un origen criptoglandular. Después de un absceso anorrectal, el 7-40% de los casos evolucionan a una fase crónica manifiesta por fístula anal. Las fístulas se clasifican en simples y complejas. Las simples se definen como trayectos interesfinterianos o transesfinterianos en los que se identifica el orificio interno, por debajo del anillo anorrectal. Metodología: El estudio fue observacional, descriptivo, retrospectivo, con muestreo no probalistico de casos consecutivos en pacientes admitidos al servicio de cirugía general del Hospital Militar Central durante el periodo 2017-2019. La muestra corresponde a 322 fichas de pacientes con enfermedades periorificiales. Resultados: La prevalencia de fistula perianal en el servicio de cirugía del Hospital Militar Central fue del 28% , fue predominante en el sexo masculino en un 78%, la edad comprendida entre los 30 a 40 años fue la más afectada en un 60%. El trayecto fistuloso más frecuente fue el interesfinterico con el 61 %, seguido del trayecto transesfinteriano 26 %, Extraesfinteriano 9 % y por ultimo supraesfinteriano 4%. El 69% de los pacientes presentaron el antecedente de absceso anal. El método diagnóstico más utilizado fue la exploración física con un 79% seguido de la ecografía endoanal con un 13 % y resonancia magnética en un 8 %. La técnica más implementada fue la fistulectomia en un 63%. La tasa de recidiva fue de 9 % todas en fistulas complejas y de incontinencia anal en un 6%. Conclusiones: Las fístulas anorrectales constituyen una patología frecuente en la consulta del cirujano coloproctólogo, con un 28 % de prevalencia en nuestro estudio, siendo así la segunda causa de consulta entre las patologías periorificiales en nuestro servicio, con una complejidad que es mal interpretada en cuanto a su diagnóstico y tratamiento. El objetivo del tratamiento de las fístulas anorrectales no solo es curarla, sino además preservar la función de la continencia anal, minimizar los defectos de la cicatrización, ofreciendo al paciente una recuperación rápida, y evitar la recurrencia.
Introducción: La colonoscopia realiza una exploración visual de la mucosa del colon, dentro de sus objetivos está la localización de lesiones colonicas, obtener biopsias para su análisis, realizar diversos procedimientos terapéuticos o diagnosticar lesiones pequeñas no detectables por métodos imagenológicos tradicionales. El objetivo del presente estudio es determinar la prevalencia de Patologías Colorrectales y sus características en pacientes sometidos a colonoscopías en el Servicio de Cirugía General del Hospital Militar Central de las FFAA durante el periodo 2018 a 2019. Metodología: estudio observacional, descriptivo, retrospectivo de corte transverso. Resultados: Fueron estudiados 355 historias clínicas e informes de colonoscopias. Del total el 56% correspondieron al sexo Femenino. El promedio de edad fue de 54 años, el grupo etario más prevalente fue de 50 a 60 años. El principal motivo de indicación de colonoscopia fue Screninng de Cáncer Colorrectal, vista en 51% de los pacientes, seguido de hemorragia digestiva baja en 27% de los pacientes. Los hallazgos más frecuentes en la colonoscopia fueron Hemorroides en 36% de los pacientes, seguido de Polipos 27% y Divertículos 18%. En cuanto a la localizaciones más frecuentes: el canal anal 40 %, Colon sigmoides 32% y colon descendente 18 %. Los hallazgos histológicos, en pólipos el tipo Hiperplasico en un 43 % y en tumores fue el adenocarcinoma de colon en un 82% de los casos. Se realizaron tratamientos quirúrgicos en 79 % de pacientes con enfermedad hemorroidal y en el 100 % de pacientes con tumores. Conclusión: La colonoscopia es un instrumento de gran utilidad al momento de diagnóstico, seguimiento y tratamiento de las patologías Colorectales, nos permite determinar el tipo de lesión, su localización y toma de biopsia. Ayuda a disminuir la incidencia y morbimortalidad causada por el Cáncer Colorrectal, y así además realizar el seguimiento endoscópico de las lesiones precursoras.
Introducción: La colecistectomía laparoscópica es uno de los procedimientos quirúrgicos realizados con más frecuencia a nivel mundial en el campo de la cirugía general. La colecistectomía laparoscópica difícil se refiere a la extracción quirúrgica de la vesícula cuando existen algunas condiciones asociadas del mismo órgano o de sus órganos vecinos o del paciente, que no permiten una disección fácil, rápida y cómoda de la vesícula, y que se traducen en prolongación del tiempo quirúrgico. Materiales y Métodos: Estudio observacional descriptivo, prospectivo de corte trasverso en pacientes adultos de ambos sexos portadores de colecistitis aguda en el Servicio de Cirugía General del Hospital Militar Central de las Fuerzas Armadas de Paraguay de noviembre 2019 a marzo de 2020. Resultados: 31 pacientes que fueron intervenidos en el Hospital Militar Central de las FF AA durante el año 2019. Las características socio demográficas encontró que el 74% de los pacientes fue mayor de 50 años, predominando la población femenina con el 65%. El 80,6 % eran Colecistitis agudas y 19,4% colecistitis crónicas, 74% de las intervenciones fueron de Urgencia y 26% fueron programadas. El tiempo quirúrgico el promedio fue 86 minutos. La valoración intraoperatoria del procedimiento más frecuentes según clasificación de Nassar encontrado fuel el grado III con 64% seguido del grado IV con un 23%. Conclusión: El desarrollo de mayores habilidades y destrezas es de vital importancia así como la aplicación de los conocimientos adquiridos durante la formación de la carrera nos permitirá ofrecerles a los pacientes una mayor seguridad a los pacientes cuando se presenta una colecistectomía difícil.
