A síndrome do Desconforto Respiratório Agudo (SDRA) é uma síndrome clínica grave, caracterizada por lesão pulmonar difusa aguda. A posição prona foi sugerida para corrigir a hipoxemia nos pacientes com SDRA grave. Objetivo: Fazer uma análise retrospectiva dos efeitos da posição prona nos parâmetros da mecânica respiratória e gasométricos dos pacientes com SDRA grave internados na Unidade de Terapia Intensiva (UTI) do Hospital Alberto Cavalcanti (HAC), no período de janeiro de 2016 a junho de 2017. Método: Foi feita a análise descritiva de sete pacientes com SDRA grave submetidos à ventilação em posição prona. Foram avaliados dos efeitos da posição prona nos parâmetros ventilatórios e gasométricos, além de eventos adversos relacionados a ela. As análises estatísticas foram realizadas no SPSS 22.0. Valor-p<0,05 foi significativo. Resultados: Cinco (71,4%) pacientes eram do sexo masculino. A idade média (±DP) foi 44,7±18,8 anos. Seis (85,7%) foram internados por doenças clínicas, 4 (57,1%) apresentaram SDRA extrapulmonar. O APACHE II foi 24±8,87. Os pacientes permaneceram internados na UTI por 26,3±17,3 e no hospital por 53,3±20,1 dias. Houve um (14,3%) óbito hospitalar. A ventilação prona foi realizada durante 16 horas, intercaladas com oito horas de ventilação em posição supina, por 3,43±2,23 (1-7) dias. Houve melhora da complacência pulmonar estática (29,8±10,3 vs. 45,7±14,9, valor-p=0,036), relação PaO2/FIO2 (89,1±24,4 vs. 288,4±56,2 valor-p<0,0001) e da PaCO2 (52,9±8,6 vs. 37,7±9,7, valor-p=0,013), com baixa incidência de eventos adversos graves. Conclusão: A posição prona foi eficiente e segura nesta população.
