Aim Youth mental health is of paramount significance to society globally. Given early onset of mental disorders and the inadequate access to appropriate services, a meaningful service transformation, based on globally recognized principles, is necessary. The aim of this paper is to describe a national Canadian project designed to achieve transformation of mental health services and to evaluate the impact of such transformation on individual and system related outcomes. Method We describe a model for transformation of services for youth with mental health and substance abuse problems across 14 geographically, linguistically and culturally diverse sites, including large and small urban, rural, First Nations and Inuit communities as well as homeless youth and a post‐secondary educational setting. The principles guiding service transformation and objectives are identical across all sites but the method to achieve them varies depending on prevailing resources, culture, geography and the population to be served and how each community can best utilize the extra resources for transformation. Results Each site is engaged in community mapping of services followed by training, active stakeholder engagement with youth and families, early case identification initiatives, providing rapid access (within 72 hours) to an assessment of the presenting problems, facilitating connection to an appropriate service within 30 days (if required) with no transition based on age within the 11 to 25 age group and a structured evaluation to track outcomes over the period of the study. Conclusions Service transformation that is likely to achieve substantial change involves very detailed and carefully orchestrated processes guided by a set of values, principles, clear objectives, training and evaluation. The evidence gathered from this project can form the basis for scaling up youth mental health services in Canada across a variety of environments.

This study confirms poor symptomatic and functional outcome after first-episode psychotic mania. It suggests possible usefulness of early intervention strategies in bipolar disorders and need for developing specific interventions addressing anxiety, depression and substance abuse comorbidity.

This study aimed to determine if, following two years of early intervention service for first-episode psychosis, three-year extension of that service was superior to three years of regular care. We conducted a randomized single blind clinical trial using an urn randomization balanced for gender and substance abuse. Participants were recruited from early intervention service clinics in Montreal. Patients (N5220), 18-35 years old, were randomized to an extension of early intervention service (EEIS; N5110) or to regular care (N5110). EEIS included case management, family intervention, cognitive behaviour therapy and crisis intervention, while regular care involved transfer to primary (community health and social services and family physicians) or secondary care (psychiatric outpatient clinics). Cumulative length of positive and negative symptom remission was the primary outcome measure. EEIS patients had a significantly longer mean length of remission of positive symptoms (92.5 vs. 63.6 weeks, t54.47, p<0.001), negative symptoms (73.4 vs. 59.6 weeks, t52.84, p50.005) and both positive and negative symptoms (66.5 vs. 56.7 weeks, t52.25, p50.03) compared to regular care patients. EEIS patients stayed in treatment longer than regular care patients (mean 131.7 vs. 105.3 weeks, t53.98, p<0.001 through contact with physicians; 134.8 6 37.7 vs. 89.8 6 55.2, t56.45, p<0.0001 through contact with other health care providers) and received more units of treatment (mean 74.9 vs. 39.9, t54.21, p<0.001 from physicians, and 57.3 vs. 28.2, t54.08, p<0.001 from other health care professionals). Length of treatment had an independent effect on the length of remission of positive symptoms (t52.62, p50.009), while number of units of treatment by any health care provider had an effect on length of remission of negative symptoms (t522.70, p50.008) as well as total symptoms (t522.40, p50.02). Post-hoc analysis showed that patients randomized to primary care, based on their better clinical profile at randomization, maintained their better outcome, especially as to remission of negative symptoms, at the end of the study. These data suggest that extending early intervention service for three additional years has a positive impact on length of remission of positive and negative symptoms compared to regular care. This may have policy implications for extending early intervention services beyond the current two years.

Background: Physical inactivity is a key contributor to the global burden of disease and disproportionately impacts the wellbeing of people experiencing mental illness. Increases in physical activity are associated with improvements in symptoms of mental illness and reduction in cardiometabolic risk. Reliable and valid clinical tools that assess physical activity would improve evaluation of intervention studies that aim to increase physical activity and reduce sedentary behaviour in people living with mental illness. Methods: The five-item Simple Physical Activity Questionnaire (SIMPAQ) was developed by a multidisciplinary, international working group as a clinical tool to assess physical activity and sedentary behaviour in people living with mental illness. Patients with a DSM or ICD mental illness diagnoses were recruited and completed the SIMPAQ on two occasions, one week apart. Participants wore an Actigraph accelerometer and completed brief cognitive and clinical assessments. Results: Evidence of SIMPAQ validity was assessed against accelerometer-derived measures of physical activity. Data were obtained from 1010 participants. The SIMPAQ had good test-retest reliability. Correlations for moderatevigorous physical activity was comparable to studies conducted in general population samples. Evidence of validity for the sedentary behaviour item was poor. An alternative method to calculate sedentary behaviour had stronger evidence of validity. This alternative method is recommended for use in future studies employing the SIMPAQ. Conclusions: The SIMPAQ is a brief measure of physical activity and sedentary behaviour that can be reliably and validly administered by health professionals.

Background: Limited knowledge exists on telepsychiatry in specialized services for firstepisode psychosis (FEP), despite its potential for improving service access and engagement.Objective: To explore access and use of technology, obstacles to attending clinic appointments, and perspectives of young adults with FEP on using telepsychiatry as part of outpatient services. Methods: A cross-sectional, descriptive survey study was conducted between July and October 2017 with young adults between the ages of 18 and 38 recruited from a specialized program for FEP in an urban Canadian setting. Data were analysed using descriptive statistics and content analysis.Results: Among 51 participants (mean age = 26.1, SD = 4.2; 59% male; 20% experiencing housing instability), more than half (59%, n = 30) rarely or never used mainstream video chat (e.g., Facetime). The majority (78%, n = 40) reported obstacles to attending appointments, with several (37%, n = 19) identifying two or more. Almost half (49%, n = 25) were very favorable towards telepsychiatry and a quarter (25%, n = 13) were somewhat favorable. Participants expressed several concerns about telepsychiatry, including loss of human contact and confidentiality.Conclusions: To our knowledge, this is a first study on the perspectives of individuals with FEP about telepsychiatry. Despite experiencing obstacles to attending appointments and expressing receptivity towards telepsychiatry, participants did not have access to these services.
