The recent upward trend in athletes requiring surgical reconstruction of the anterior cruciate ligament (ACL) has been a well-documented phenomenon. What has not been as widely publicized, however, is the intense pressure that these same athletes are under to return to their chosen sport in time frames that are shorter than ever. I therefore read the article ''No Association of Time From Surgery With Functional Deficits in Athletes After Anterior Cruciate Ligament Reconstruction'' by Myer et al 3 in the October issue and received an enhanced insight into the multiplicity of variables that lie behind being a responsible clinician as well as a genuine patient advocate.
