Coube a William Brinton, em 1857, relacionar, pela primeira vez, ansiedade e úlcera gastroduodenal; 60 anos mais tarde, Kaufmann apresentou o "elemento psíquico como um fator importante" no desenvolvimento da referida doença. Mais recentemente, a partir de contribuições diversasde internistas, gastroenterologistas, psiquiatras e psicanalistas-passou-se a realçar o papel do psíquico, elevando-o às culminâncias de possível agente etiológico da doença ulcerosa. Estudou-se o considerável acréscimo de úlceras pépticas em estatísticas que partem, mais ou menos, de 1913 e alcançam os ápices de freqüência nas duas guerras mundiais. Mais grave que o aumento do número de doentes mostrava-se, durante a última guerra, a ascensão quase vertical dos acidentes hemorrágicos dos ulcerosos (não raro, mortais), fato verificado em vários países. A elevação da incidência, mesmo em tempo de paz, foi relacionada com a crescente complexidade da cultura, acentuando os estudos de tensão e multiplicando as razões de ansiedade. Cumpria apresentar demonstrações em sentido inverso: povos primitivos ou de baixo nível cultural estariam indenes à doença. Mesmo do Brasil, dos hospitais da Fordlândia, saiu documentação dessa tese: em 26.000 hospitalizados, apenas dois ulcerosos, ambos alienígenas. Simultaneamente, trabalhos de setor psiquiátrico assinalavam um fato notável. Os grandes ataques histéricos, as manifestações neuróticas tempestuosas, tão conhecidas na primeira conflagração, quase desapareceram na segunda, quando a psiquiatria de guerra passou a registrar considerável aumento da sintomatologia de linguagem visceral: taquicardias, dispepsias, desordens na cinética intestinal, etc. O comportamento humano mudara sua linguagem. Interiorizava-se esta ou, como querem os behaviouristas, surgia um comportamento interior visceral, nova forma de expressão das emoções.
