El neumomediastino se define como la presencia de aire en el mediastino, puede ocurrir de manera espontánea en individuos sanos sin desencadenantes, en cuyo caso la condición se denomina neumomediastino espontáneo. El diagnóstico se puede confirmar mediante radiografía de tórax o tomografía computarizada de tórax, y en algunos casos puede ser difícil de manejar debido a la ausencia de pautas claras de diagnóstico y manejo, es una entidad clínica poco frecuente, sin embargo, los médicos de urgencias deben considerar diagnósticos diferenciales que, de no tomarse en cuenta, pueden resultar fatales para nuestros pacientes. Presentamos el caso de un paciente de 17 años con neumomediastino espontáneo (síndrome de Hamman) que acudió al servicio de urgencias de nuestro hospital por enfisema cervical subcutáneo, se le realizó un tratamiento conservador, tras 24 horas de observación fue dado de alta.
