BackgroundA principal reason for low use of public health care services is the perception of inferior quality of care. Studying health service user (HSU) experiences with their care and their perception of health service quality is critical to understanding health service utilization. The aim of this study was to define reference points for some aspects of health care quality and to analyze which HSU experiences resulted in perceptions of overall low quality of care.MethodsData from the National Health Survey 2006 were used to compare the experiences of HSUs with their ambulatory care at Ministry of Health and affiliated institutions (MOH), social security institutions (SSI) and private institutions (PrivI). Reference points of quality of care related to waiting time and expenditure were defined for each of the three types of institutions by analyzing HSU experiences rated as 'acceptable'. A multivariable logistic regression model was used to identify the principal factors associated with the general perception of low quality of care.ResultsA total of 11,959 HSUs were included in the analysis, of whom 37.6% (n = 4,500) HSUs received care at MOH facilities; 31.2% (n = 3,730) used SSI and 31.2% (n = 3,729) PrivI. An estimated travel and waiting time of 10 minutes respectively was rated as acceptable by HSUs from all institutions. The differences between the waiting time rated as acceptable and the actual waiting time were the largest for SSI (30 min) in comparison to MoH (20 min) and PrivI (5 min) users. The principal factors associated with an overall perception of low quality of care are type of institution (OR 4.36; 95% CI 2.95-6.44), waiting time (OR 3.20; 95% CI 2.35-4.35), improvement of health after consultation (OR 2.93; CI 2.29-3.76) and consultation length of less than 20 minutes (2.03; 95% CI 1.60-2.57).ConclusionsThe reference points derived by the HSUs' own ratings are useful in identifying where quality improvements are required. Prioritizing the reduction of waiting times and improving health status improvement after consultation would increase overall quality of care ratings.
Objetivo. Identificar la percepción de los usuarios de los servicios de salud ambulatorios en México sobre la calidad de atención y los factores asociados con dicha percepción. Material y métodos. De los datos obtenidos por las Encuestas Nacionales de Salud y Nutrición 2006 y 2012 se analizó la percepción de la atención recibida y de mejoría en salud después de la atención; se elaboró un modelo multivariado para identificar los factores de los servicios asociados con la percepción de mejoría. Resultados. La buena percepción de atención se elevó de 82 a 85%, y la de mejoría del estado de salud, de 79 a 81%, entre los años 2006 y 2012. La explicación sobre la enfermedad y tratamiento, la consulta sin cita previa, la oportunidad de elegir el servicio de salud y menor tiempo de espera se asociaron con la percepción de mejoría. Conclusiones. Existen avances en la calidad de atención percibida en la atención ambulatoria en México. Es necesario fortalecer estrategias organizacionales para brindar atención con mayor oportunidad y responder a las expectativas de los usuarios.
The utilization of an institution of delivery that differed from the one with which the women were affiliated indicates the existence of a de facto functional integration between health institutions in Mexico.
Through quantitative and qualitative methods, in this article the authors describe the perspectives of indigenous women who received antenatal and childbirth medical care within a care model that incorporates a non-governmental organisation (NGO), Partners in Health. They discuss whether the NGO model better resolves the care-seeking process, including access to health care, compared with a standard model of care in government-subsidised health care units (setting of health services networks). Universal health coverage advocates access for the most disadvantaged and vulnerable populations as a priority. However, the issue of access includes problems related to the effect of certain structural social determinants that limit different aspects of the obstetric care process. The findings of this study show the need to modify the structure of organisational values in order to place users at the centre of medical care and ensure respect for their rights. The participation of agents outside the public system, such as NGOs, can be of great value for moving in this direction. Women's participation is also necessary for learning how they are being cared for and the extent to which they are satisfied with obstetric services. This research experience can be used for other countries with similar conditions.
Objetivo. Analizar la participación de hospitales mexicanos en el proceso de certificación (equivalente a la acreditación en otros países). Material y métodos. Estudio transversal, analiza resultados de 136 establecimientos auditados entre 2009 y 2012. Se identificaron estándares con calificación excelente (9.0-10.0), aprobatoria (6-8.9) y no aprobatoria (0-5.9). Con un modelo logístico multinomial se calculó la probabilidad de obtener calificación no aprobatoria, aprobatoria y excelente. Resultados. La calificación promedio general fue 7.72, más alta en hospitales de cirugía ambulatoria (9.10), que en hospitales generales (7.30) y de especialidad (7.99). Todos los establecimientos públicos obtuvieron calificación aprobatoria. Los hospitales auditados en 2011 tuvieron mayor riesgo de obtener calificación aprobatoria (RRR= 4.6, p<0.05) y excelente (RRR= 6.6, p<0.05). Conclusiones. El alcance del proceso de certificación en México ha sido limitado, con mayor participación del sector privado. La cédula de evaluación aplicada en 2011 favoreció la obtención de resultados aprobatorios y de excelencia. Se recomienda homologar el proceso en su totalidad con el empleado por la Joint Commission International (JCI).
Objetivo. Analizar el posible efecto de los modelos de certificación y de los incentivos implementados en la participación de establecimientos de atención médica (EAM) en la certificación del Consejo de Salubridad General entre 1999- 2017. Material y métodos. Se colectaron documentos oficiales, impresos y en línea, sobre la certificación de EAM y se solicitó información a diversas instancias relacionadas mediante mecanismos de transparencia. Se analizó la participación de EAM en los períodos político-administrativos entre 1999-2017. Resultados. El promedio anual de participación entre 1999-2000 fue de 259.5 EAM; entre 2013-2016, de 72.5. La participación de EAM públicos es decreciente. En 2017, los EAM certificados eran <1%. Conclusiones. No se identificaron efectos positivos ni sostenidos de ajustes al modelo, ni de los incentivos implementados. Se observa disminución de la participación en los distintos periodos político-administrativos. Debe evaluarse profundamente el Sistema Nacional de Certificación de EAM y su posible efecto en la calidad clínica.
