ObjectiveTo investigate orbital vascular resistance by Doppler velocimetry in patients
with steady-state sickle cell anemia, as well as to characterize its
relationship with biomarkers of hemolysis.Materials and MethodsThis was a cross-sectional study of two groups: 71 outpatients with sickle
cell anemia; and 32 age- and gendermatched, healthy subjects (control
group). All participants underwent Doppler velocimetry of the orbital
arteries and laboratory tests.ResultsAll biochemical laboratory markers were abnormal in the sickle cell anemia
patients (p < 0.0001 vs. controls). In the patient
group, Doppler velocimetry revealed increased flow velocities in the
ophthalmic artery and reduced flow velocities in the central retinal artery,
as well as high values for the resistance index (RI) and pulsatility index
(PI) in both arteries (p < 0.0001 vs. controls).
Biomarkers of hemolysis were found to correlate significantly with the RI
and PI indices. In the ophthalmic artery, the reticulocyte count showed a
moderate direct correlation with RI and with PI. In the central retinal
artery, hemoglobin showed a strong inverse correlation with RI and with
PI.ConclusionOrbital vascular resistance, as evaluated by Doppler velocimetry, is elevated
in patients with steady-state sickle cell anemia and shows a significant
correlation with biomarkers of hemolysis.

A investigação acerca das pessoas deslocadas internamente (isto é, no interior dos seus países, mas fora dos seus espaços de vida habituais) se mostra um desafio, em virtude dos volumes de pessoas atualmente envolvidas e das condições com que se confrontam, da incapacidade (ou desinteresse) dos Estados nacionais em garantir proteção aos seus cidadãos, e do princípio de neutralidade internacional, que dificilmente permite intervenções externas no sentido da salvaguarda dos direitos humanos. Analisamos o conceito de cidadania, Estado soberano e a noção do não intervencionismo, assim como problematizamos os desafios colocados pela globalização, pela amplitude dos fenômenos migratórios e desrespeitos aos direitos humanos. Acreditamos que a análise pode servir a um maior entendimento sobre os fenômenos migratórios e fragilidade institucional, além da revisão de conceitos. Pode ainda servir para uma revisão crítica da legislação e da possibilidade de institucionalização de órgãos transnacionais de proteção.
RESUMO:Torna-se relevante a revisão da proteção internacional dos refugiados pela diversidade de causas que geram fluxos migratórios forçados nos dias atuais, como fatores ambientais, construção de grandes infraestruturas, ocupação da terra para o agronegócio, dentre outras. Uma análise crítica deste aparato jurídico pode garantir mais eficiência no cumprimento dos direitos humanos, dar um "efeito útil" à legislação existente, bem como problematizar o surgimento e participação de novos atores (movimentos sociais locais ou transnacionais, comunidade envolvida), para além do Estado, na proteção dos direitos dos refugiados.
