Esta pesquisa objetivou descrever os registros científicos brasileiros acerca da adesão à terapia antirretroviral (TARV) por pessoas que vivem com HIV/AIDS. Tratou-se de um estudo descritivo, de revisão integrativa de literatura, realizado na base LILACS e na biblioteca virtual SciELO, acessadas por intermédio da Biblioteca Virtual em Saúde (BVS). Como resultados, obtiveram-se as categorias: conceitos de adesão ao tratamento antirretroviral, condições de adesão/não adesão/abandono à terapia antirretroviral, a influência da terapia antirretroviral na vida de alguns dos diversos grupos populacionais e ações de enfermagem para a promoção da adesão à terapia antirretroviral. Concluiu-se que as dificuldades de adesão à TARV mostram-se oriundas, sobretudo ao baixo nível educacional, ao déficit de suporte familiar e social, consumo de bebida alcoólica e drogas ilícitas e efeitos colaterais proporcionados por tais medicamentos. Assim, a enfermagem brasileira deve ser capaz de assistir aos sujeitos que necessitam da TARV em seus fatores psicossociais e demandas de saúde, instrumentalizada, sobretudo, pela sistematização da assistência.
