Resumo OBJETIVO Analisar as notificações de incidentes relacionados à segurança do paciente. MÉTODOS Estudo transversal com abordagem quantitativa, baseado nos dados do Gerenciamento de Risco de um complexo hospitalar, localizado no noroeste paulista, de agosto/2015 a julho/2016. RESULTADOS Foram analisadas 4.691 notificações. O enfermeiro foi a categoria profissional que mais notificou (71%), seguido do médico (8%). O período mais frequente em que ocorreram as notificações foi o diurno. Houve diferença significativa da proporção de notificações entre os dias da semana. As notificações foram classificadas por motivo, com destaque para os medicamentos (17%), seguido de lesões de pele (15%) e flebite (14%). A maior frequência de notificações ocorreu nas unidades de Internação. Quanto à gravidade 344 eventos ocasionaram dano ao paciente, sendo a maioria de intensidade leve (65%). CONCLUSÃO As notificações espontâneas são uma importante fonte de informações e evidenciam a magnitude do problema relacionado aos incidentes em saúde.
Introdução: O aparecimento de alterações da pele é uma das consequências mais comuns que acometem pacientes hospitalizados, principalmente de longa permanência. A manutenção da integridade da pele dos pacientes internados tem por base o conhecimento de que medidas de cuidados relativamente simples podem garantir que a pele do paciente se mantenha íntegra durante seu período de internação. A proteção da integridade cutânea é considerada como indicativo de qualidade dos cuidados, pois a presença de lesões de pele ou perilesão está associada ao aumento do tempo de internação e à carga de trabalho para equipe de saúde, além de ocasionar aumento dos custos hospitalares. Objetivo: Este estudo objetiva analisar as características das notificações de lesão de pele de um hospital universitário público no interior de São Paulo. Material e Métodos: Estudo descritivo, retrospectivo, com abordagem quantitativa, com base nos dados do sistema de notificação, no período de agosto de 2015 a julho de 2016. Resultados: Foram notificados 697 eventos adversos relacionados a integridade da pele, sendo 657 (94,26%) lesões por pressão. As LPP foram classificadas por estágios, sendo o estágio 2 o de maior ocorrência, seguido do estágio 1. As unidades que mais notificaram foram a unidade de terapia intensiva e a clínica medica e cirúrgica. A lesão por pressão é um dos principais eventos adversos causados pela assistência à saúde. Os ferimentos originados pelo contato do paciente com a superfície do leito, de poltronas e dispositivos, como sondas e cateteres, são causa de grande desconforto para o paciente, além de aumentar riscos de infecções, o tempo de hospitalização e os custos do tratamento. A Lesão por Pressão (LPP) é considerada um dos indicadores negativos de qualidade assistencial dos serviços de saúde e de enfermagem e sua prevenção é importante, considerando o contexto do movimento global pela segurança do paciente. Conclusão: Os eventos de LPP estão entre os incidentes evitáveis, ou seja, poderiam ser prevenidos através da adoção de medidas preventivas e estratégias institucionais. O relato desses eventos é fundamental para analisar as condições em que ocorreram os incidentes, identificar suas consequências e buscar melhores práticas de prevenção e tratamento.
