Objetivos: Caracterizar as Reações Transfusionais (RT) imediatas e analisar os meios para diagnóstico dessas reações. Metodologia: Trata-se de uma pesquisa com delineamento longitudinal retrospectivo, exploratório e descritivo, realizado em um Hospital Universitário Público de Aracaju – SE, Brasil. Todos os aspectos éticos foram respeitados aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Sergipe, a coleta de dados ocorreu durante o período de 2019 e 2020. Os dados relativos as reações transfusionais foram extraídas do sistema de notificação do hospital e foram caracterizados segundo protocolos nacionais e internacionais, quanto à imputabilidade de uma Reação Transfusional com critérios clínicos, laboratoriais e com o vínculo temporal. A análise estatística realizada dos dados foi descritiva e inferencial de acordo com as variáveis e objetivos dos pesquisadores. Resultados: A data do incidente transfusional e a notificação no sistema conferiu um tempo médio de 3,91 dias para que os profissionais notificassem as RT. O histórico transfusional não foi preenchido em 85% (67) das notificações. A possibilidade de recidivas de RT nesses pacientes ocorreu entre 1 a 18 dias do evento inicial e apenas 27% (n=3) estavam em unidades distintas. Considerações finais: As reações transfusionais quando notificadas demonstraram uma fonte rica para um cuidado mais adequado para pacientes submetidos ao tratamento transfusional com hemocomponentes.
Objetivo: Descrever o processo de fundamentação teórica da construção de 2 (dois) Procedimento Operacional Padrão voltado para as medidas de mitigação para as Reações Transfusionais imediatas. Metodologia. Trata-se de um estudo teórico com características qualitativas, descritivas e exploratórias, a qual foi realizada uma imersão na literatura especializada nacional e internacional para subsidiar a produção de um Procedimento Operacional Padrão voltado para as medidas de mitigação das Reações Transfusionais imediatas, desenvolvido entre os meses de março a julho de 2020 realizado em um Hospital Universitário de Sergipe com base nas fragilidades do processo apontadas pela literatura e pela prática laboral. Foi levado em consideração que as RT podem decorrer de três condicionantes: doador, paciente e cuidados prestados. Resultados. O processo de construção do instrumento ocorreu em três momentos: no primeiro, consistiu na revisão da literatura em para localizar evidências científicas qualificadas e atualizadas, sobre o tema estudado; no segundo, foi analisado todo o processo de hemotransfusão do hospital; e no terceiro, foi reunido os registros e/ou anotações acerca das fragilidades processuais e laborais observadas como demandas de profissionais da saúde Considerações Finais: O alcance do cuidado de excelência necessita da ampla divulgação do conhecimento de forma periódica à equipe interdisciplinar, do despertar da necessidade de notificar, da explicitação das fragilidades do processo com os envolvidos para que a comunicação e a identificação do paciente sejam aliadas, do monitoramento da eficácia e das medidas preventivas otimizadas no manejo.
Introdução: A terapia transfusional possui um papel curativo e preventivo e requer avaliação, acompanhamento, monitoramento e análise das etapas a fim de evitar as subnotificações e falhas assistenciais que resultam em Reação Transfusional impactando negativamente nas taxas de morbidade, mortalidade e na ascensão de custos hospitalares. Objetivo: Desenvolver o módulo de software Hemomonitor com sistema de alertas para monitoramento dos pacientes em uso de transfusão. Método: Trata-se de uma pesquisa aplicada realizada em um Hospital Universitário, cuja trajetória metodológica seguiu três etapas: 1) Caracterização do processo transfusional no local do estudo; 2) Atualização acerca dos softwares disponíveis sobre o tema por meio de uma revisão de literatura; 3) Desenvolvimento de software com base na literatura especializada. Resultados: Os fluxos de atividades desempenhadas foram deflagrados a partir da extração de dados multivariados da base de dados dos sistemas AGHUx e +Exames, utilizados pela instituição para transformá-los em informações. Foi necessário parametrizar as alterações dos exames laboratoriais e reproduzí-los, certificar a solicitação do hemocomponente com a assinatura do médico responsável pelo paciente, construir um sequenciamento de solicitação de hemocomponente, permitir o compartilhamento das informações e o interfaceamento entre os setores, padronizar os exames comprobatórios mediante à suspeita transfusional, identificar a amostra do paciente e da bolsa do hemocomponente de forma informatizada e apropriada com a legislação, parametrizar os laudos que serão gerados a partir das provas de compatibilidade, permitir a rastreabilidade, parametrizar alterações dos sinais vitais para estabelecer o vínculo temporal, definir as trilhas de auditoria para capacitar as equipes quanto à celeridade dos processos, eficiência e aproveitamento substancial dos recursos. Conclusão: O Hemomonitor otimiza a monitorização e acompanhamento em tempo real dos pacientes em uso de transfusão com atualização dessas informações a cada três minutos na intranet do hospital, auxiliando na antecipação de condutas eficazes para a mitigação das Reações Transfusionais.

Conclusão: A implementação do módulo Hemomonitor e a aplicação em uma unidade piloto permitiu observar que a sua utilização pode melhorar a monitorização e acompanhamento em tempo real dos pacientes em uso de HC, auxiliando na antecipação de condutas eficazes para mitigação das RT.
