Objetivo: Expor o conceito, diagnóstico e tratamento diante de perfurações radiculares. Revisão bibliográfica: As perfurações são comunicações endo-periodontais causadas por processos patológicos ou através de iatrogenias durante procedimentos odontológicos. Geralmente, as perfurações radiculares estão associadas a processos infamatórios nos tecidos periodontais circunvizinhos, o que implica na necessidade de uma avaliação periodontal, pois as condições de saúde dos tecidos irão definir a melhor alternativa de tratamento, que pode ser cirúrgico ou não, ou até mesmo a combinação de ambos. Com auxílio de exames complementares para a determinação da sua localização e seu tamanho, os quais irão estabelecer direções de tratamentos precisas, como também o tempo entre o acidente e seu reparo, irão favorecer o diagnóstico e o sucesso do tratamento. Considerações finais: Esta revisão teve como finalidade estabelecer e discutir direções terapêuticas contemporâneas para o tratamento de comunicações do canal radicular. Assim, os aspectos essenciais para reparar estas lesões incluem o seu diagnóstico, prognóstico, e uma discussão sobre os materiais sugeridos para o selamento de perfuração do canal radicular.
Objetivo: Revisar a literatura acerca do insucesso endodôntico com doença apical pós-operatória e a presença de instrumento fraturado e material obturador em contato com a região apical, e que respondem de forma positiva ao retratamento de canal, sem necessidade de intervenção cirúrgica. Revisão bibliográfica: A literatura revisada demonstrou que o insucesso endodôntico é observado quando ocorre o desenvolvimento, persistência ou agravamento de sinais e/ou sintomas de infecção endodôntica após o tratamento de canal inicial. As modalidades de tratamento para as patologias periapicais pós-operatórias baseiam-se no retratamento via ortógrada e na cirurgia perirradicular. A presença de instrumento fraturado em contato com os tecidos perirradiculares bem como o extravasamento de material obturador, são acidentes operatórios que podem acontecer durante a instrumentação e obturação de canais radiculares, respectivamente, que comprometem o sucesso quando associados à uma infecção. Considerações finais: Desta forma, quando detectados no diagnóstico da doença pós-operatória, são em geral tratados de forma cirúrgica objetivando remoção do material presente na área apical.
Objetivo: Revisar e discutir o diagnóstico diferencial e tratamento das lesões endo-periodontais. Revisão bibliográfica: As lesões endodôntico-periodontais afetam ao mesmo tempo a polpa e o periodonto. Como mostram semelhança embriológica, anatômica e de microbiota, o seu diagnóstico acaba se tornando uma inspiração para os cirurgiões dentistas. A polpa dentária e o ligamento periodontal estão intimamente relacionados, possuem interrelações embrionárias, anatômicas e funcionais. Há uma maior comunicação entre esses dois tecidos, sendo assim, a ocorrência de doença em um dos tecidos pode ser difundida para o outro, pois essas conexões servem de caminho para os microorganismos e seus subprodutos transitarem entre a polpa e o periodonto. A lesão endo-perio é uma condição caracterizada pela associação da doença periodontal e pulpar no mesmo elemento dental. Para avaliação da lesão e correto diagnóstico e estabelecimento do melhor plano de tratamento, é necessário conhecer os métodos de classificação das lesões endo-perio visando um melhor prognóstico. Considerações finais: Por intermédio desse conhecimento, o cirurgião dentista chegará o diagnóstico e o tratamento corretos, revertendo em possíveis chances de sucesso ao tratamento das lesões endodônticas-periodontais. Pertinente à multiplicidade dessas infecções, uma conduta interdisciplinar entre endodontistas, periodontistas e microbiologistas é aconselhada.
