RESUMO:Este estudo teve como objetivo compreender o significado de vivenciar a doença mental na família com vistas a identificar os recursos adaptativos da mesma. Trata-se de um estudo exploratório descritivo com abordagem qualitativa, realizado em um Centro de Atenção Psicossocial do interior do Estado do Rio Grande do Sul. A amostra do estudo constituiu-se de 10 familiares do cotidiano de portadores de transtorno mental. Os resultados apontam as dificuldades de manejo da família em situações de crise, a culpa pela doença; a solidão e o desamparo, quando do aparecimento dos primeiros sintomas; os sentimentos, como o medo, a tristeza, a vergonha e a raiva, assim como, o afeto e o cuidado presentes. Conclui-se que o suporte de informações para os familiares amenizaria o sentimento de culpa, diminuindo em grande parte o sofrimento dos familiares, assim como, a necessidade de a família ser co-partícipe do tratamento e da reabilitação.ABSTRACT: This study aimed to better understand what it means for a family to live with mental illness, seeking to identify adaptive resources. It is an exploratory, descriptive, qualitative research that was carried out at a Psycho-Social Care Center in Rio Grande do Sul, Brazil, from June to July of 2005. This sample was comprised of 10 family members of people who have a mental illness. Data analysis was carried out via ordering, classification, and final analysis. The results point out the difficulties the family has in handling crises; the guilt they feel due to the illness; the loneliness and abandonment felt when the first symptoms appear; and feelings such as fear, sadness, shame, and anger; as well as affection and care present. I conclude that providing information support for family members would ease the feeling of guilt, as well as the family's need to be a co-participant during treatment and rehabilitation. RESUMEN:El objetivo del presente estudio fue comprender el significado de vivenciar la enfermedad mental en familia e identificar los recursos adaptativos. Se trata de un estudio exploratorio descriptivo cualitativo, realizado en un Centro de Atención Psico-social en Rio Grande do Sul, Brasil, de junio a julio de 2005. La muestra del estudio se constituyó de 10 familiares de portadores de trastorno mental. El análisis de los datos fue realizado por ordenación, clasificación y análisis final. Los resultados señalan: dificultades de manejo de la familia en situaciones de crisis; culpa por la enfermedad; soledad y desamparo al aparecimiento de los primeros síntomas; sentimientos como miedo, tristeza, vergüenza y rabia; así como, afecto y cuidado presentes. Se concluye que el soporte de informaciones para los familiares reduciría el sentimiento de culpa, así como, la necesidad de la familia ser copartícipe del tratamiento y de la rehabilitación. Esta pesquisa teve como objetivo geral investigar o preparo dos acadêmicos de enfermagem frente à morte. A metodologia é de natureza qualitativa, realizada através de 15 entrevistas semi-estruturadas com acadêmicos de Enfermagem do sexto e sétimo semestres de uma instituição de ensino superior localizada no norte do Rio Grande do Sul. Os resultados apontam as dificuldades dos acadêmicos de enfermagem entrevistados em lidar com o processo morte-morrer dos pacientes assistidos em estágio; a dificuldade de lidar com os sentimentos de ansiedade, culpa e impotência gerados pela experiência; a fuga do contato com os familiares, através de cuidados emimentemente técnicos e burocráticos; a falta de apoio do professor supervisor em campo de estágio, quando da morte de um paciente e o desconhecimento das fases psicológicas da morte. Conclui-se que os acadêmicos de enfermagem não estão preparados para vivenciar o processo morte-morrer de seus futuros clientes devido às poucas oportunidades de discutir o tema na graduação.
The lives of persons living with mental illness are affected by psychological, biological, social, economic, and environmental factors over the life course. It is therefore unlikely that simple preventive strategies, clinical treatments, therapeutic interventions, or policy options will succeed as singular solutions for the challenges of mental illness. Persons living with mental illness receive services and supports in multiple settings across the health care continuum that are often fragmented, uncoordinated, and inadequately responsive. Appropriate assessment is an important tool that health systems must deploy to respond to the strengths, preferences, and needs of persons with mental illness. However, standard approaches are often focused on measurement of psychiatric symptoms without taking a broader perspective to address issues like growth, development, and aging; physical health and disability; social relationships; economic resources; housing; substance use; involvement with criminal justice; stigma; and recovery. Using conglomerations of instruments to cover more domains is impractical, inconsistent, and incomplete while posing considerable assessment burden. interRAI mental health instruments were developed by a network of over 100 researchers, clinicians, and policy experts from over 35 nations. This includes assessment systems for adults in inpatient psychiatry, community mental health, emergency departments, mobile crisis teams, and long-term care settings, as well as a screening system for police officers. A similar set of instruments is available for child/youth mental health. The instruments form an integrated mental health information system because they share a common assessment language, conceptual basis, clinical emphasis, data collection
The research investigates the matrix support in mental health in Primary Health Care in order to identify the guidelines, professional principles and values that permeate the process. The study used a qualitative approach with triangulation of techniques, and was performed with professionals from the Family Health Strategy. The results reveal a work process which is based on personalized relations, guided by the broadening of clinical practice and mediated by principles, such as inter-disciplinarity, bonding, comprehensiveness, accessibility, co-responsibilization and longitudinality. The values that underpin the relationship with the specialist are organized around the bond, trust and honesty. Difficulties in taking patients to Psychosocial Care Centers arise as obstacles to networking. The bond with the specialist, the structure of Family Health teams and the exchange of experiences and knowledge are identified as facilitating factors. The conclusions drawn are that the networking and the systematic and longitudinal support by specialists are key to decentralization in mental health.
Autonomia e cidadania na reabilitação psicossocial: uma reflexãoAutonomy and citizenship in psychosocial rehabilitation: a reflection
RESUMO O artigo aborda a descentralização em saúde mental por meio do Apoio Matricial na Atenção Primária à Saúde (APS). A pesquisa teve como objetivo geral investigar o Apoio Matricial em saúde mental na APS na perspectiva dos especialistas, levando em conta as diretrizes do Sistema Único de Saúde (SUS) e da Reforma Psiquiátrica. Trata-se de uma pesquisa qualitativa, realizada mediante entrevistas semiestruturadas e grupo focal com apoiadores matriciais em saúde mental. Os resultados evidenciam o aspecto processual da construção conjunta de intervenções sustentadas pela corresponsabilização e pela cogestão; os arranjos organizacionais e operacionais fundamentados em ações de supervisão, discussão de casos clínicos e atendimento conjunto. As relações personalizadas com as equipes de referência, o viés de saúde pública dos especialistas e o investimento em sensibilizar os profissionais generalistas e especialistas foram identificados na pesquisa. Conclui-se que o Apoio Matricial se sustenta por meio de tecnologias relacionais, na sistematicidade dos encontros e na longitudinalidade, devendo ser tais questões consideradas no cuidado interprofissional.
