Covid-19 virüsü dünya üzerinde büyük bir etki bırakmıştır ve yayılmaya devam etmektedir. Daha fazla yayılmasını engellemek için koronavirüs hastalarına erken tanı koymak oldukça önemlidir. Her ne kadar akciğer X-Işını görüntüsü tanısı ile çözüm en hızlı ve en kolay yöntem olsa da ortalama bir radyoloğun X-Işını verilerini kullanarak tanı koymadaki doğruluğu tamamen mesleki deneyimine dayanmaktadır. Yani, daha deneyimsiz radyologların hata yapma olasılığı daha fazladır. Bu nedenle tutarlı sonuçlar verebilen bir yapay zekâ modeli üretilmesi istenmektedir. Çalışmamızda göğüs X-Işını görüntüleri ve sıradan kan ölçüm verileri kullanılarak sınıflandırma yapılmış ve sonuçları karşılaştırılmıştır. X-Işını verileri hem açık kaynak çalışmalardan hem de yerel bir hastaneden anonim olarak toplanmıştır ve yaklaşık 7200 görüntüye sahiptir. Kan ölçümü sonuçları da yine aynı yerel hastaneden toplanmıştır. Göğüs X-Işını verilerinin tanısı için yaygın olarak kullanılan evrişimsel sinir ağı algoritmalarından ResNet, SqueezeNet, DenseNet ve VGG kullanılmıştır. Sonuçlar, SqueezeNet modelinin daha yüksek AUC değeri vermesiyle birlikte, diğer algoritmaların da %85 üstünde bulma ve tutturma değeri sağladığını göstermektedir. Covid-19’un kan ölçümlerinden tanısı için ise çok katmanlı yapay sinir ağı ve destek vektör makinası kullanılmıştır. Kan ölçüm verileri kullanarak sınıflandırma kısıtlı bir veri kümesi üzerinde yapılmış olsa da yapay sinir ağı ve destek vektör makinası için doğruluk oranları sırasıyla %76 ve %82 olarak bulunmuştur. Genelleme yapılırsa X-Işını yoluyla tanının kan ölçümü yoluyla yapılan tanıdan daha uygulanabilir olduğu ve Covid tanısında yapay zekânın insanlardan daha doğru sonuç çıkardığı sonucuna ulaşılmıştır.
The Covid-19 virus has made a major impact on the world and is still spreading rapidly. A reliable solution to prevent further damage, early diagnosis of coronavirus patients are incredibly important. While chest X-Ray diagnosis is the easiest and fastest solution for this, an average radiologist has only a 75% to 85% accuracy when evaluating X-Ray data, thus it is desirable to achieve an accurate artificial network for this. Throughout this study, chest X-Ray data and blood routine test data are utilised and compared. X-Ray data consists of 5000 chest X-Ray images which are gathered from an open-source research and from a local hospital in which both have anonymous data. The blood test results were also taken from the same hospital. For the chest X-Ray diagnosis we utilised two of the popular convolutional neural networks, which are Resnet18 and Squeezenet and concluded that Resnet18 provided slightly more accurate results, while both having almost 98% accuracy. For blood test diagnosis, a feed-forward multi layer neural network was used. Even though it was worked on an insufficient dataset, 72% accuracy was obtained, thus making it a feasible option for further research. Hence, we concluded that in general chest X-Ray diagnosis is preferable over routine blood test diagnosis and the usage of AI yields better approximate results than humans.
Introduction Acute toxic leukoencephalopathy (ATL) is a rare complication of cancer treatment, with symptoms varying from mild cognitive impairment to coma. Recognition and management of ATL are important because in most cases, the cessation of the responsible agent is essential. Case report We report a case of a 57-year-old male with relapsed right colon cancer who had multiple steps of chemotherapy, admitted to the emergency department (ED) with confusion and inability to talk, 4 days after FOLFIRI and bevacizumab treatment. To exclude cerebrovascular events cranial computed tomography and diffusion-weighted magnetic resonance imaging were evaluated. There was bilateral and symmetric diffusion restriction on white matter, which was consistent with ATL. Management and outcome Supportive treatment such as optimization of blood pressure and metabolic control was applied since there is no specific treatment for ATL other than cessation of the responsible agents. 12 days after the admission to the ED his neurologic symptoms were normalized and there was no diffusion restriction on control imaging. Discussion ATL is a rare complication of cancer treatment and responsible agents are increasing in number due to the development of cancer treatment. ATL is associated with drugs that are used frequently such as 5-fluorouracil. ATL is mostly reversible, but the progression of neurologic symptoms was also reported. The diagnosis and cessation of the responsible agent are important in management.
