Os defeitos craniofaciais e do terço médio da face após ressecção tumoral representam um grande desafio para os cirurgiões plásticos devido à complexidade das estruturas envolvidas, pela configuração tridimensional do defeito e pelo grande impacto funcional e estético que implicam. Portanto, a utilização dos transplantes microcirúrgicos tornou-se uma grande opção para as reconstruções envolvendo a região craniofacial. Métodos: Análise retrospectiva de pacientes submetidos à ressecção de tumores da região craniofacial e que foram submetidos à reconstrução com retalhos microcirúrgicos no período de maio de 2012 a maio de 2017. Resultados: O retalho microcirúrgico mais utilizado foi o retalho miocutâneo do músculo reto abdominal, com 50% dos casos. Os vasos faciais foram os mais utilizados para anastomose com 68,2% dos casos. E em 90,9% obteve-se sucesso do tratamento. Conclusão: Tendo em vista a magnitude do defeito residual após a ressecção tumoral, a reconstrução com retalhos microcirúrgicos apresenta-se como opção segura e com elevado índice de sucesso, devolvendo ao paciente a forma e função da região.
