La parálisis Obstétrica del plexo braquial (POPB) es una lesión por tracción del plexo braquial que genera parálisis flácida del miembro torácico, generalmente asociada a distocia de hombros y en raros casos en recién nacidos por cesárea. (1-4).La incidencia de la POPB varia entre 0.4-3 por cada 1000 recién nacidos, en los últimos años se ha visto una disminución debido a el entendimiento de la patología y el cuidado durante el trabajo de parto (1,3,5,6).El principal factor de riesgo para esta patología es la distocia del hombro el cual puede provocar tracción del plexo braquial (PB). Otros factores de riesgo asociados son: la utilización de fórceps, macrosomía, tiempo de parto prolongado, presentación pélvica, desproporción cefalopélvica, multiparidad y diabetes gestacional (1-3,6,7)Aunque en la literatura refiere que el 90% de los casos mejoran de forma espontánea (3 meses – 2 años), se estima que entre un 20-30% de los niños afectados quedan con un déficit residual (1,3-5)
El pulgar en gatillo es una patología de etiología idiopática en la que el tendón flexor largo del pulgar se encuentra engrosado proximal a la polea A1 generando una discrepancia de tamaño que impide el deslizamiento del tendón a través dicha polea, Se presenta de manera global en 3 de cada 1000 RNV siendo mas común en población hispana, su diagnóstico es clínico por lo que no se requieren estudios de extensión, la resolución espontánea de esta patología se presenta en el 30% de los casos aproximadamente, porcentaje que puede aumentar si se realiza terapia física; sin embargo los pacientes que no presentan mejoría deben ser referidos al ortopedista pediátrico para recibir tratamiento quirúrigico.
En México, las deformidades congénitas de las extremidades pélvicas representan un reto terapéutico asociado con pronóstico sombrío.1 En concreto, las hemimelias tibial y peronea, y las hipoplasias femorales proximales, por su alto costo y complejidad técnica, suelen terminar en amputación o persistencia de la deformidad en la vida adulta.2 Ambas situaciones repercuten en la calidad de vida de los pacientes, que se refleja en la falta de integración a la sociedad e incapacidad para tener una vida autosustentable e independiente.
Las apofisitis por tracción son lesiones por sobre uso que se presentan exclusivamente en el paciente pediátrico como resultante del micro trauma repetitivo generado por la tracción de los tendones a nivel de las apófisis, principalmente durante la fase rápida de crecimiento en pacientes que practican actividades deportivas, ya que durante este periodo el crecimiento óseo y muscular no son armónicos, los tendones relativamente cortos generan mayor fuerza de tracción sobre las apófisis creando cargas submaximas de tracción repetida causante de inflamación y lesión de las apófisis.; las apofisitis por tracción en la rodilla son Osgood Schlatter y Sinding Larsen Johansson
