Introdução: A asma é uma doença inflamatória crônica que representa um importante problema de saúde pública. No Brasil, estudos sobre a mortalidade por asma em idosos são escassos, de amplitude local ou restritos a curto período de análise. Objetivo: Este estudo se propôs a analisar a mortalidade por asma em idosos no país. Método: Trata-se de estudo ecológico com dados de óbitos por asma entre os anos de 2000 e 2019. Os dados foram acessados no Sistema de Informações sobre Mortalidade segundo variáveis sociodemográficas. A idade foi agregada em intervalos de 60 a 74 anos, 75 a 89 anos e ≥90 anos. Taxas de mortalidade ajustadas foram calculadas por idade e frequência relativa dos óbitos. O padrão de ocorrência dos óbitos foi caracterizado por idosos (≥ 60 anos de idade), segundo características socioeconômicas e demográficas no Brasil por idade, por sexo e intervalos de tempo, segundo regiões, unidades federadas e Brasil. Além disso, este estudo propõe analisar as desigualdades socioeconômicas e de acesso e uso de serviços de saúde e sua relação com as desigualdades nos riscos de morte entre as regiões, UF e grupos de sexo e idade do país. Resultados: Constatou-se que a asma é uma importante causa de óbito entre os idosos, acometendo mais severamente mulheres, estando relacionado também a fatores socioeconomicos e acesso a serviços de saúde dessas pessoas. Conclusão: Apesar dos avanços no tratamento da asma e a existência de diretrizes para o seu adequado manejo, a doença permanece pobremente controlada, especialmente em idosos em contextos de maior fragilidade assistencial.
