Physical activity (PA) interventions have been shown to improve the health of people living with HIV (PLWH), yet treatment dropout poses an important challenge. We conducted a meta-analysis to investigate the prevalence and predictors of treatment dropout in PA interventions in PLWH. Electronic databases were searched for records up to September 2016. Randomized control trials of PA interventions in PLWH reporting dropout rates were included. Random effects meta-analysis and meta-regression analyses were employed. In 36 studies involving 49 PA intervention arms, 1128 PLWH were included (mean age = 41.6 years; 79.3% male; 39% White). The trim and fill adjusted treatment dropout rate was 29.3% (95% CI = 24.5-34.7%). There was a significant lower dropout rate in resistance training interventions compared with aerobic (p = 0.003) PA interventions, in studies utilizing supervised interventions throughout the study period (p < 0.001), and in studies using adequately qualified professionals (p < 0.001). Exerciser/participant variables that moderated higher dropout rates were a lower percentage of male participants (β = 1.15, standard error (SE) = 0.49, z = 2.0, p = 0.048), a lower body mass index(BMI) (β = 0.14, SE = 0.06, z = 2.16, p = 0.03), and a lower cardiorespiratory fitness (β = 0.10, SE = 0.04, z = 2.7, p = 0.006). The dropout from PA interventions is much higher in PLWH than in many other populations with chronic morbidities. Qualified professionals (i.e., exercise physiologists, physical educators, or physical therapists) should be incorporated as key care providers in the multidisciplinary care of HIV/AIDS and should prescribe supervised PA for PLWH in order to enhance adherence and reduce the burden of HIV/AIDS. Special attention should be given men, those with a higher BMI, and those with a lower cardiorespiratory fitness.
RESUMOO uso da terapia anti-retroviral combinada (TARV) aumentou a expectativa de vida dos portadores do vírus da imunodeficiência humana (HIV) e, como conseqüência, torna-se constante o estudo do processo fisiopatológico da infecção e das estratégias de intervenção que possam melhorar a qualidade de vida destes indivíduos. O treinamento concorrente, definido como a associação dos componentes aeróbios e força na mesma sessão de exercícios pode ser inserido neste contexto. O objetivo deste artigo é revisar os aspectos centrais da infecção pelo HIV e as evidências existentes sobre o treinamento concorrente com séries múltiplas e simples nos parâmetros imunológico, virológico, cardiorrespiratório e neuromuscular de pessoas infectadas pelo HIV. O método utilizado foi uma revisão sistemática, com artigos publicados entre 2000-2007, nas bases de dados scielo e pubmed e também em dissertações e teses da biblioteca digital da educação física, esporte e saúde. As evidências científicas encontradas nos 8 estudos analisados indicam que o treinamento concorrente, tanto com séries múltiplas quanto séries simples, melhora os parâmetros cardiorrespiratório e neuromuscular; no entanto, os resultados dos parâmetros imunológico e virológi-co ainda não estão completamente esclarecidos, principalmente sobre os protocolos com séries simples. Considerando o número restrito de trabalhos apresentados, sugere-se o desenvolvimento de estudos com treinamento concorrente em vários desfechos clínicos para pessoas infectadas pelo HIV.Palavras-chave: HIV, treinamento concorrente, exercício físico. ABSTRACTThe highly active antiretroviral therapy (HAART) increased life expectancy in individuals infected with the human immunodeficiency virus (HIV). Alternative strategies have been used in order to improve their quality of life and minimize the HAART effect; among these we can name physical exercise. Concurrent training, the combination of aerobic and strength training in the same exercise session, is used to improve aerobic and strength capacities with shorter time of training and stands out within the used methods. It is crucial to understand this syndrome's characteristics for the best application of physical exercise in the HIV infected patients. Therefore, the aim of this study is to review the main HIV infection characteristics and the evidence available on concurrent training with single and multiple sets in immunological, virological, cardiorespiratory and neuromuscular parameters in subjects living with HIV/AIDS. A systematic review of papers published from 2000-2007 in the Scielo and Pubmed database, as well as theses and dissertations on Physical Education, Sports and Health fields from the digital library totalizing 8 studies, was performed. SummaryOne of the most recent phenomena related to the acquired immunodeficiency syndrome (AIDS) is the emergence of a new vulnerable population: the elderly. One of the factors that account for this increase is the development of combination antiretroviral therapy (ART), which has provided better quality of life and life expectancy for HIV-positive patients. However, ART is associated with adverse effects such as dyslipidemia, diabetes mellitus and insulin resistance, which are risk factors for cardiovascular disease. Due to the impact of ART on lipid and glucose metabolism, many studies were published involving HIV infection and cardiovascular disease, as well as their risk factors and the use of ART, but few of them reported on the cardiotoxicity of this therapy in the elderly. The objective of this study is to review the main metabolic changes caused by the use of antiretroviral therapy and its impact on an increased risk of cardiovascular disease in elderly people with HIV.
Human immunodeficiency virus (HIV)-infected subjects have increased levels of oxidative stress which could impair immunological function and therefore contribute to the progression of AIDS. These characteristics are usually evaluated at rest and responses to exercise have yet to be evaluated. The aim of the present study was to assess the effect of a bout of aerobic exercise followed by resistance exercises on antioxidant system in HIV-infected and non-HIV subjects. There were included 14 cases (HIV-positive) and 14 controls (HIV-negative). The exercise protocol consisted of a single session of 20 minutes on a cycloergometer followed by a set of six resistance exercises. The activity of glutathione S-transferase (GST) and catalase were measured in plasma samples, total glutathione (TGSH) and thiobarbituric acid reactive substances (TBARS) were measured in erythrocytes. T CD4+ cells, T CD8+, viral load, complete blood count, and white blood count were also assessed. All measurements were performed at three times: baseline, after aerobic exercise, and after resistance exercises. At baseline, the HIV group had lower GST activity than controls, but after the exercise session GST values were similar in both groups. Compared to the control group TGSH was significantly lower in the HIV group at baseline, after aerobic and resistance exercises. The control group presented higher TBARS values after aerobic exercise compared to the HIV group. The neutrophil count was lower in the HIV group after aerobic and resistance exercises. These data indicate that HIV-infected subjects had lower antioxidant activity at rest. Physical exercise stimulated the enzymatic activity similarly in both groups.
Objective: Study sleep disorders in a Group of Elite Military Police Offi cers. Method: Cross-sectional study in which 22 elite military police offi cers from the Brigada Militar were assessed, corresponding to the total operational headcount of the group studied. Results: There were four main fi ndings in this study. First, a high prevalence of sleep disorders was observed (100% presented some sleeprelated disorder or complaint) and bad sleep quality (63.6%) among these offi cers. Second, a high prevalence (27.3%) of obstructive sleep apnea syndrome was observed among the police offi cers studied, who presented greater daytime sleepiness. Third, among the police offi cers who presented some occupational accident, a higher prevalence of daytime sleepiness, bad sleep quality and obstructive sleep apnea syndrome was found. Fourth, offi cers with a bad quality of sleep presented worse quality of life scores. Conclusion: Considering the high prevalence of sleep disorders among military police offi cers and the different consequences resulting from a bad quality of sleep, sleep-related questions, questionnaires to screen for sleep disorders and, depending on the case, polysomnography should be included in the routine health evaluation of Elite Military Police Offi cers. ResumoObjetivo: Estudar os distúrbios do sono em um Grupo de Policiais Militares de Elite. Método: Estudo transversal, que avaliou 22 policiais militares de elite da Brigada Militar, o que corresponde a totalidade do efetivo operacional do grupo estudado. Resultados: Os principais achados deste estudo foram quatro. Primeiro, observou-se uma elevada prevalência de distúrbios do sono (100% apresentaram algum distúrbio ou queixa relacionada ao sono) e de má qualidade de sono (63,6%) entre esses policiais. Segundo, observou-se uma elevada prevalência (27,3%) de síndrome da apneia obstrutiva do sono entre os policiais estudados sendo que estes apresentaram maior sonolência diurna. Terceiro, dentre os policiais que apresentaram algum acidente de trabalho constatou-se uma maior prevalência de sonolência diurna, de má qualidade do sono e de síndrome da apneia obstrutiva do sono. Quarto, policiais com má qualidade de sono apresentaram piores escores de qualidade de vida. Conclusão: Considerando-se a elevada prevalência de distúrbios do sono entre os policiais militares e as diversas consequências advindas de um sono de má qualidade, sugere-se a inclusão de questões relacionadas ao sono, de questionários de triagem para distúrbios do sono e dependendo do caso, do exame de polissonografi a, dentro da inspeção bienal de saúde dos Policiais Militares de Elite. A infecção pelo vírus da imunodeficiência humana (HIV) é acompanhada por alterações estruturais e funcionais relacionadas ao sistema imunológico. Além disso, o aumento do estresse oxidativo (EO) nos portadores do HIV, caracterizado por diminuição nos níveis de glutationa (GSH), aumentos na glutationa oxidada (GSSG), na razão GSSG/GSH e lipoperoxidação, bem como redução da atividade de enzimas antioxidantes - catalase, superóxido dismutase (SOD) e glutationa peroxidase (GPx) - é uma conseqüência da evolução dos pacientes infectados com HIV. As células do sistema imunológico necessitam de altas concentrações de antioxidantes para manter o balanço redox e preservar a sua integridade e função. Quando ocorre a depleção dos antioxidantes, há diminuição da resposta imunológica e aumento na replicação do HIV. O uso da terapia anti-retroviral combinada (TARV) melhorou significativamente a evolução clínica dos pacientes, porém, mesmo assim, alguns continuam apresentando EO aumentado e outros efeitos da TARV, como alterações no metabolismo lipídico e muscular. O treinamento físico é utilizado como intervenção não farmacológica nos pacientes infectados pelo HIV para proporcionar melhoria nos parâmetros antropométricos, aeróbios, musculares e psicológicos, porém, há carência de estudos sobre a sua utilização em relação ao estresse oxidativo. Nesta revisão, foram analisados os tópicos referentes ao estresse oxidativo nos pacientes HIV positivos e os possíveis benefícios do exercício físico na capacidade antioxidante. O treinamento físico é uma estratégia auxiliar para os pacientes, com ou sem uso da TARV, uma vez que melhora os aspectos cardiorrespiratórios, musculares, antropométricos e psicológicos sem induzir a imunossupressão. Referindo-se ao estresse oxidativo, infere-se, a partir dos dados em indivíduos HIV negativos, que o treinamento físico pode gerar adaptações que minimizam os efeitos deletérios provocados pelo EO através de melhorias nos níveis das defesas antioxidantes enzimáticas e não enzimáticas.
Com a cronicidade da infecção pelo HIV, a partir do uso da terapia antirretroviral combinada (TARV), o indivíduo torna-se predisposto às alterações metabólicas e corporais denominadas síndrome lipodistrófica, que têm como consequência o risco aumentado para doenças cardiovasculares. Tornam-se necessários estudos com viabilidades terapêuticas com o intuito de melhorar a qualidade de vida destes pacientes. Neste contexto, o treinamento físico deve ser abordado como terapêutica para minimizar os efeitos deletérios da TARV. O objetivo desse artigo foi revisar na literatura as evidências científicas sobre o treinamento físico na síndrome lipodistrófica. O método utilizado foi uma revisão sistemática com a localização de artigos publicados entre 2000 e 2008 nas bases de dados PubMed e SciELO. As evidências científicas encontradas nos estudos analisados indicam que a maioria dos estudos, independente do tipo de treinamento, obtiveram melhoria significativa na composição corporal. Dessa forma, tornam-se necessário mais estudos investigando o treinamento físico no perfil lipídico, na resistência a insulina e nas alterações glicêmicas em pessoas vivendo com HIV/Aids.
