A obesidade, por sua caracterização e etiologia multifatorial, é uma condição que tem merecido atenção e estudos de diversas áreas de especialidades, particularmente a psiquiatria e a psicologia. Os problemas emocionais são geralmente percebidos como conseqüências da obesidade, embora conflitos e problemas psicológicos possam preceder o desenvolvimento dessa condição. A depressão e a ansiedade são sintomas comuns, e depressão maior pode ser mais freqüente nos gravemente obesos. No tratamento psicoterápico, a terapia cognitiva vem mostrando eficácia por objetivar a organização das contingências para mudanças de peso e comportamentos, em princípio, relacionados ao autocontrole de comportamentos alimentares e contexto situacional mais amplo. Embora o tratamento farmacológico da obesidade não deva ser a primeira opção terapêutica, seu uso está plenamente aprovado. Atenção deve ser dada para as drogas realmente aprovadas para uso em longo prazo e as de ação central, podendo atuarem como desencadeadoras de quadros psiquiátricos.Palavras-chave: obesidade, terapia cognitiva, agentes antiobesidade.
A cirurgia bariátrica é o método mais eficaz no tratamento da obesidade mórbida e controle do peso em longo prazo. As cirurgias antiobesidade podem ser procedimentos que limitam a capacidade gástrica, ou que interferem na digestão ou, ainda, uma combinação de ambas as técnicas. A maneira mais objetiva para classificar a obesidade é o Índice de Massa Corpórea (IMC). Pessoas com IMC acima de 40 são portadoras de obesidade mórbida. As principais indicações para a cirurgia bariátrica são: obesos com IMC maior que 40 kg/m² ou IMC acima de 35 kg/m², associado com doenças clínicas descompensadas pela própria obesidade. O papel do psicólogo é o de avaliar se o indivíduo está apto emocionalmente para a cirurgia e auxiliá-lo quanto à compreensão de todos os aspectos decorrentes do pré e pós-cirúrgico. Quanto a possíveis contraindicações psiquiátricas para a cirurgia antiobesidade, não há consenso na literatura.Palavras-chave: Cirurgia bariátrica, obesidade mórbida, aspectos psiquiátricos da obesidade.
ResumoO transtorno da compulsão alimentar periódica (TCAP) foi descrito pela primeira vez nos anos 1950. Contudo, sua elevação à categoria diagnóstica apenas ocorreu em 1994, quando foi incluído no apêndice B do DSM IV, com critérios provisórios para seu diagnóstico. Trata-se de uma síndrome caracterizada por episódios recorrentes de compulsão alimentar, sem qualquer comportamento de compensação para evitar um possível ganho de peso. Incertezas quanto a seus parâmetros diagnósticos como caracterização da quantidade de alimentos ingeridos, duração de um episódio de comer compulsivo, ou mesmo o valor da perda de controle sobre a ingestão alimentar, tornam difíceis uma homogeinização de um grupo sindrômico. Desta forma, estudos epidemiológicos podem revelar diferentes dados de caracterização da população portadora deste transtorno. Isto reforça a necessidade da manutenção de estudos para avaliação desta patologia.Palavras-chave: Transtorno da compusão alimentar periódica, binge eating disorder, obesidade. AbstractBinge eating disorder was first described in 1955. However, its upgrade to a diagnostic category only occurred in 1994, when it was included in appendix B of DSM IV, with provisory criteria. It is characterized by recurrent episodes of binge eating, without any compensatory behavior to prevent a possible weight gain. Uncertainties about the diagnostic criteria like the amount of food ingested, the duration or the value of the loss of control during a binge eating episode make its characterization difficult. Then, epidemiological studies may reveal the characterization of this disorder. This means that more studies are needed for an appropriate evaluation of this pathology.

Bruxismo durante o sono (BS) é uma parassonia caracterizada por movimentos involuntários e estereotipados com ranger dos dentes durante o sono. A prevalência do BS é igual nos dois sexos, variando de 3% a 20% na população geral, sendo mais comum nos jovens. O BS secundário é causado por transtornos neurológicos ou pode estar associado a transtornos primários do sono e antidepressivos inibidores seletivos da recaptação da serotonina. Já o BS primário apresenta fatores de predisposição genética ou psicológica, má oclusão dentária, disfunção leve dos gânglios da base e combinações desses fatores. Os principais sinais e sintomas do BS incluem o ruído característico de ranger dos dentes, desgaste dentário, dor local, hipertrofia dos músculos masseteres e temporais, cefaléias, disfunção da articulação temporomandibular, sono de má qualidade e sonolência diurna. O diagnóstico clínico de BS é feito por meio da história do paciente, do cônjuge e do exame odontológico. A polissonografia documenta a presença de episódios de ranger dos dentes, permitindo identificar alterações da arquitetura do sono, presença de microdespertares, abalos mioclônicos de membros inferiores, roncos e distúrbios respiratórios sonodependentes. O tratamento deve ser direcionado para os fatores etiológicos com base no BS secundário. Não existe tratamento-padrão para o BS primário, devendo este ser individualizado ao paciente. O tratamento odontológico do BS primário e secundário com placas de repouso tem como objetivo prevenir danos das estruturas orofaciais e aliviar dor craniofacial. O tratamento comportamental inclui técnicas de relaxamento, abstinência de cafeína e tabaco. O tratamento farmacológico do BS primário e secundário emprega drogas agonistas dopaminérgicas, benzodiazepínicos ansiolíticos, buspirona, hipnóticos não-benzodiazepínicos, como o zolpidem, relaxantes musculares, certos antidepressivos, como mirtazapina, nefazodona, trazodona, bupopriona e drogas antiepilépticas, como a gabapentina. Aplicações locais de toxina botulínica nos músculos masseteres e temporais podem ser utilizadas em casos de bruxismo intenso não-responsivo à terapêutica convencional.
