There are controversies about the possible benefits of off pump coronary artery bypass grafting (OPCABG) compared to on-pump coronary artery bypass grafting (ONCABG). For a better perspective on this important issue, we performed a meta-analysis of randomized controlled trials, comparing the two techniques. The objective of this study was to verify which technique applied in Coronary Artery Bypass Surgery, OPCABG or ONCABG, provides better results through a meta-analysis of published randomized trials comparing the two techniques. We carried out a computer-based literature search in PubMed, Embase, B-on and Science Direct from March 2009 to January 2010. The studies covered were recovered according to predetermined criteria. A systematic review of randomized clinical trials was performed in order to evaluate the differences between the two revascularization techniques (OPCABG versus ONCABG) regarding mortality and morbidity. Selected studies did not include patients at high risk and long-term longitudinal evaluations. The meta-analysis focused on nine randomized clinical trials, corresponding to a total of 75,086 patients, and compared OPCABG to ONCABG. Regarding mortality, a reduction of 18% in the risk of cardiovascular mortality (OR: 0.82, 95%CI: 0.70 to 0.98, p = 0.03) and 27% in the risk of stroke postoperatively (OR: 0.73, 95%CI: 0.63 to 0.85, p = 0.0001) were observed, both in favor of OPCABG. Concerning the occurrence of complications associated with the procedure, no significant differences were found between the two surgical techniques, particularly with regard to the occurrence of kidney complications (OR: 0.97, 95%CI: 0.84-1.14, p = 0, 74) and sepsis (OR 0.98, 95%CI: 0.64-1.51, p = 0.93, respectively). Off-pump CABG significantly reduces the occurrence of major cardiovascular events (mortality and CVA) compared to on-pump CABG surgery.
BackgroundAortic valve sclerosis (AVS) is characterized by increased thickness, calcification and stiffness of the aortic leaflets without fusion of the commissures. Several studies show an association between AVS and presence of coronary artery disease.ObjectiveThe aim of this study is to investigate the association between presence of AVS with occurrence of previous coronary artery disease and classical risk factors.MethodsThe sample was composed of 2,493 individuals who underwent transthoracic echocardiography between August 2011 and December 2012. The mean age of the cohort was 67.5 ± 15.9 years, and 50.7% were female.ResultsThe most frequent clinical indication for Doppler echocardiography was the presence of stroke (28.8%), and the most common risk factor was hypertension (60.8%). The most prevalent pathological findings on Doppler echocardiography were mitral valve sclerosis (37.1%) and AVS (36.7%). There was a statistically significant association between AVS with hypertension (p < 0.001), myocardial infarction (p = 0.007), diabetes (p = 0.006) and compromised left ventricular systolic function (p < 0.001). Conclusion: Patients with AVS have higher prevalences of hypertension, stroke, hypercholesterolemia, myocardial infarction, diabetes and compromised left ventricular systolic function when compared with patients without AVS. We conclude that there is an association between presence of AVS with previous coronary artery disease and classical risk factors.
This study was carried to evaluate the efficiency of the Bitterlich method in growth and yield modeling of the even-aged Eucalyptus stands. 25 plots were setup in Eucalyptus grandis cropped under a high bole system in the Central Western Region of Minas Gerais, Brazil. The sampling points were setup in the center of each plot. The data of four annual mesurements were colleted and used to adjust the three model types using the age, the site index and the basal area as independent variables. The growths models were fitted for volume and mass of trees. The efficiency of the Bitterlich method was confirmed for generating the data for growth and yield modeling.Keywords: Growth and yield models, permanent sampling points, Bitterlich method. Introdução: A fibromialgia é caracterizada como uma doença crónica na qual a principal característica é a dor músculo-esquelética generalizada, associada a pontos dolorosos com localizações anatomicamente precisas quando sujeitas a palpação. Em situações normais estes estímulos não provocariam dor. Predominantemente o género feminino é o mais afetado e frequentemente está associada a sintomas como fadiga, dificuldades cognitivas, perturbações de humor, cefaleias, alterações gastrointestinais e a distúrbios do sono. A relação sono e dor, sintoma primordial da fibromialgia é consistente, sendo os distúrbios do sono muito comuns sugerindo que estes têm uma elevada relevância no agravamento da sintomatologia. Objetivo: Avaliação através de questionários a relação entre qualidade de sono e sintomatologia da fibromialgia. Métodos: Estudo analítico, observacional e transversal, com uma amostra do tipo não probabilístico, realizado em 38 indivíduos. O estudo implicou o autopreenchimento de inquéritos nomeadamente Informação Geral, Escala Visual Analógica da Intensidade da Dor, Questionário Impacto da Fibromialgia, Índice Qualidade de Sono de Pittsburgh e Questionário qualidade de vida SF-12. A análise estatística foi executada com recurso ao programa SPSS® (Statistic Product and Service Solution). Resultados: Verificou-se que as variáveis PSQI global e SF-12 apresentaram uma correlação negativa fraca com significância marginalmente significativa (p=0,072) e uma correlação positiva moderada estatisticamente significativa foi verificada entre a depressão e ansiedade (p<0,001). As restantes variáveis não apresentaram quaisquer associações. Conclusão: A sintomatologia da fibromialgia está relacionada com os distúrbios do sono, devendo estes pacientes realizar uma avaliação do sono tanto subjetiva como objetiva. Da mesma forma, deve ser incentivada uma intervenção multidisciplinar de maneira a potenciar o aumento da qualidade de vida destes pacientes, melhorando e adaptando o seu acompanhamento clínico e adotando abordagens não apenas farmacológicas.
