The present case report describes a modified technique of coronectomy for mandibular third molar with association of Platelet-Rich Fibrin Graft (PRF). The panoramic radiograph showed proximity of the roots of the vertical positioned 48 tooth with the superior wall of the mandibular canal, since the erasure of the superior cortical of the mandibular canal was observed. Due to the risk of injury to the inferior alveolar nerve, the technique of coronectomy was chosen. After the realized the coronectomy, in order to promote the regeneration of soft and hard tissues and also to prevent pulp changes that could lead to pain, pulp necrosis and infection, the PRF graft was used to fill the surgical defect formed by removal of the crown. Good healing evolution was observed and after 12 months of clinical and radiographic follow-up there was complete bone formation in the area where the crown was removed and no complications were observed.
Lesões do nervo hipoglosso e laríngeo, recorrente em suas vias extracranianas unilaterais ou bilaterais, causam um déficit motor na língua com perda transitória, parcial ou total da fala, que pode estar associada à disfagia leve ou grave, caracterizando a síndrome de Tapia. Essa condição incomum costuma estar relacionada à manipulação das vias aéreas durante o ato anestésico-cirúrgico, sendo poucos os casos na literatura correlacionados com cirurgias maxilofaciais, o que explica, em parte, o desconhecimento dessa complicação por um número significativo de profissionais. Descrevemos um relato de caso de síndrome de Tapia bilateral estabelecida após procedimento para avanço maxilomandibular. Trata-se de um paciente do gênero masculino, 55 anos, com diagnóstico de retrognatismo e retromaxilismo. O paciente foi submetido a um avanço maxilomandibular com intubação nasotraqueal. Evoluiu com síndrome de Tapia bilateral, recuperando-se dos sinais e sintomas desta complicação, sendo reoperado para corrigir resultados estéticos obtidos na primeira intervenção que estava abaixo do aceitável, não evoluindo após esta segunda cirurgia com a síndrome. Concluímos que medidas preventivas devem ser tomadas, como evitar movimentos bruscos na região da cabeça e hiperextensão prolongada do pescoço durante a cirurgia, minimizando o trauma da cânula traqueal sobre os tecidos que recobrem os nervos hipoglosso e laríngeo recorrente.
Lesions of the hypoglossal and laryngeal nerve, recurrent in its unilateral or bilateral extracranial pathways, cause a motor deficit in the tongue with transient, partial or total loss of speech, which may be associated with mild or severe dysphagia, characterizing Tapia syndrome. This unusual condition is usually related to airway manipulation during anesthetic-surgical surgery, and few cases in the literature are correlated with maxillofacial surgeries, which partly explains the ignorance of this complication by a significant number of professionals. We describe a case report of bilateral Tapia syndrome established after a procedure for maxillomandibular advancement. This is a 55-year-old male patient diagnosed with retrognatism and retromaxillism. The patient underwent a maxillomandibular advancement with nasotracheal intubation. He evolved with bilateral Tapia syndrome, recovering from the signs and symptoms of this complication, being reoperated to correct aesthetic results obtained in the first intervention that was below acceptable, not evolving after this second surgery with the syndrome. We conclude that preventive measures should be taken, such as avoiding sudden movements in the head region and prolonged hyperextension of the neck during surgery, minimizing the trauma of the tracheal cannula on the tissues covering the hypoglossal and recurrent laryngeal nerves.
Saber reconhecer um paciente com má oclusão associa- da a deformidade esquelética maxilofacial é importante para a correta indicação da cirurgia ortognática, possibilitando resultados funcionais (oclusão e respiração) e estéticos (har- monia facial) adequados. Este trabalho tem como objetivo discutir através da apresentação de casos clínicos os prin- cipais aspectos ligados à cirurgia ortognática como: idade mínima para sua realização; camuflagem ortodôntica ou tratamento ortodôntico-cirúrgico; quando operar a maxila, a mandíbula ou ambos; e a importância da relação multi- disciplinar que envolve o tratamento de pacientes portadores de deformidades dentofaciais.
