Zusammenfassung. Hintergrund: Bislang werden im Bereich der klinischen Psychologie zur Analyse von Längsschnittdaten vor allem Varianzanalysen mit Messwiederholung (RM-ANOVA) verwendet. Mehrebenenmodelle für Längsschnittdaten sind demgegenüber in ihren Voraussetzungsannahmen weniger restriktiv und in ihren Analysemöglichkeiten weitaus flexibler. Fragestellung: Wie können Mehrebenenmodelle für Längsschnittdaten in der Psychotherapieforschung sinnvoll eingesetzt werden? Methode: Aufbauend auf dem Beitrag von Keller (2003) werden Probleme klassischer Verfahren der Datenauswertung am Beispiel der Wirksamkeitsevaluation einer kognitiv-verhaltenstherapeutischen Gruppentherapie erläutert und alternativen Modellen gegenübergestellt. Ergebnisse: Es wird gezeigt, wie lineare und nichtlineare Veränderungen beschrieben und getestet sowie eine zusätzliche Analyseebene im Gesamtmodell integriert werden können. Schlussfolgerung: Mehrebenenmodelle für Längsschnittdaten bieten eine adäquate Strategie, um typischen Anwendungsfällen der klinisch-psychologischen Forschung in optimaler Weise Rechnung zu tragen.
Zusammenfassung. Theoretischer Hintergrund: Auf Grund hoher Prävalenzen und deutlicher einhergehender sozio-ökonomischer Folgen stellen multiple somatoforme Beschwerden ein ernst zu nehmendes Problem dar. Fragestellung: Wie effektiv ist eine ambulante kognitiv-behaviorale Gruppenbehandlung im Vergleich zu einem Entspannungsverfahren und einer Wartegruppe in Bezug auf körperliche Beschwerden, Angst, Depressivität und subjektive Gesundheit. Methode: Es wurde eine randomisierte Kontrollgruppenstudie (N = 77) durchgeführt. Die manualisierten Interventionen umfassten acht 90-minütige Termine, die wöchentlich stattfanden. Die manualisierte kognitiv-behaviorale Intervention baut auf dem Behandlungsansatz von Rief und Hiller (1998) auf. Ergebnisse: Sowohl die kognitiv-behaviorale Behandlung als auch die Entspannungsgruppe führten zu einer Verbesserung der somatoformen Beschwerden (SOMS-7). Die kognitiv-behaviorale Behandlung hatte darüber hinaus einen positiven Einfluss auf die Angst (HADS-D) und die subjektive psychische Gesundheit (SF-12). Schlussfolgerungen: Diese kurze, kognitiv-behaviorale Gruppenbehandlung hat positive Effekte auf ambulante Patientinnen und Patienten mit somatoformen Symptomen, die über die Effekte einer Entspannungsgruppe hinausgehen.
Zusammenfassung. Ziel der vorliegenden Arbeit ist es, einen Überblick über den bisherigen empirischen Forschungsstand zur Wirksamkeit der Sporttherapie bei somatoformen Beschwerden zu geben. Auf Grund des defizitären Forschungsstandes bei somatoformen Beschwerden im engeren Sinne werden auch Studien zur Sporttherapie bei Fibromyalgie, dem chronischen Müdigkeitssyndrom und dem Reizdarmsyndrom berücksichtigt. Für die Fibromyalgie und das chronische Müdigkeitssyndrom ist die Wirksamkeit von Sporttherapie als evidenzbasiert anzusehen. Gute Evidenzen liegen für ein sporttherapeutisches Angebot vor, welches entweder ein Ausdauer- oder ein Kombinationstraining (Ausdauer und Stretching/Krafttraining) beinhaltet. Für somatoforme Beschwerden im engeren Sinne ist der Forschungsstand zum gegenwärtigen Zeitpunkt zu gering, um eine abschließende Aussage treffen zu können. Implikationen für weiteren Forschungsbedarf werden dargelegt.
Hintergrund: Patienten mit somatoformen Stöungen leiden häufig komorbid unter Depressionen oder Angststörungen. Ziel der Studie ist es, deren Einfluss auf den Erfolg kognitiv-verhaltenstherapeutischer Therapie bei somatoformen Störungen zu klären. Methode: In einem längsschnittlichen Untersuchungsdesign wurde an 106 Patienten, die an einer ambulanten Gruppenintervention zur Bewältigung somatoformer Beschwerden teilgenommen haben, die Bedeutung komorbider Depression und Angststörungen für die Prädiktion des Therapieerfolgs untersucht. Ergebnisse: In bivariaten Analysen zeigten sich signifikante Zusammenhänge zwischen komorbider Angst, Intensität der somatoformen Störung und dem kurzfristigen und katamnestischen Therapieerfolg. Das Vorliegen einer Depression war nicht mit dem Therapieerfolg assoziiert. In multiplen Regressionsanalysen konnte jedoch nur eine hohe Intensität der somatoformen Störung als Prädiktor für den Therapieerfolg bestätigt werden. Schlussfolgerung: Auch schwerer beeinträchtigten Patienten mit somatoformen Störungen und Patienten mit komorbider Depression oder Angststörungen kann eine ambulante kognitiv-verhaltentherapeutische Therapie zur Symptombewältigung empfohlen werden.
